The data are reported on 1000 patients operated upon for lung cancer, in 290 of them concomitant cardiovascular lesions have been revealed. The postoperative mortality was found to be higher in patients with postinfarction
cardiosclerosis
,
hypertension
of stage II--III at the age under 59, while in chronic coronary insufficiency, arrhythmia and impaired conductivity the mortality rate was not increased. In patients over 60 the postoperative mortality was higher than in young patients, however, a concomitant cardiovascular pathology rendered no appreciable effect. A 5-year survival in concomitant cardiovascular pathology was 25% versus 33.3% in control patients, that is attributed by the authors to a large number of "compromise" saving operations in patients with concomitant cardiovascular lesions.

Total cholesterol and triglycerides as well as their distribution among the different hyperlipoproteinemia types were studied in three patients groups: 47 normotensive patients with myocardial infarction, 35 normotensive patients with
cardiosclerosis
(chronic ischemic heart disease), 29 hypertensive patients without symptoms or signs of clinical atherosclerosis. Their results were compared to those of 45 normal controls. There was no decrease in HDL cholesterol nor increase in LDL cholesterol in patients with chronic or acute ischemic heart disease. A large percentage of patients from these groups had normolipoproteinemia. The most prominent lipidic changes were observed in hypertensive patients: no patient had a HDL cholesterol level above normal values, thirty three per cent had a HDL cholesterol level below 35 mg/dl. A high percentage of patients with acute myocardial infarction or
hypertension
exhibited atypical lipoproteinemia anomalies (hyper HDL triglyceridemia, hyper LDL triglyceridemia, hyper VLDL cholesterolemia) when they could have normolipoproteinemia. This suggested lipoproteinic metabolism disturbances in such cases.

While studying the functional activity of poly- and mononuclear leucocytes in 75 patients with various clinical types of ischemic heart disease (angina of effort, angina decubitus, arrhythmias, cardiac insufficiency at the background of atherosclerotic
cardiosclerosis
and atherosclerotic
hypertension
), decline in the functional activity of the phagocytizing cells has been revealed, this being considered to be a pathogenetic prerequisite for atherosclerosis progression.

As many as 26 patients with arterial
hypertension
(AH) associated with postmyocardial infarction
cardiosclerosis
were examined for assessing the efficacy of a new preparation belonging to a panel of calcium antagonists, viz. isradipine (<<Lomir>> by SANDOZ). Of note, a positive result was observed in 88%, which fact was manifested by normalization of AH without any side effects. Half the patient population showed improved tolerability of physical loads by lowering AH increments and reduction in the myocardium oxygen demand. Thus, use of <<Lomir>> in a combined treatment of patients surviving myocardial infarction and suffering from AH, furthers reconvalescence and improves tolerability of physical loads.

The 24-h profile of blood pressure (BP) was studied in 28 patients (21 males and 7 females) with congenital heart failure (CHF) of NYHA class II-III (ejection fraction < 45%). The patients were 46 to 76 years of age and had postinfarction
cardiosclerosis
. They had not received ACE inhibitors before. Two groups were formed basing on the presence of
hypertension
. Perindopril was administered in a single daily dose of 2 mg or higher if demanded to reduce symptoms of CHF and/or to normalize BP. The treatment continued for 3 months. The 24-h BP profile was assessed using portable device SpaceLabs 90207 (USA). In CHF patients with
hypertension
perindopril significantly lowered mean 24-h, day and night BP and its loads, reestablished two-phase circadian rhythm of AP and corrected BP variability. In CHF patients free of
hypertension
significant changes of the profile were not registered. It is evident that unwanted changes in the BP 24-h profile due to perindopril were absent in CHF normotensives.

This study assessed the diagnostic value of X-ray characteristics of the heart and pulmonary circulation in CHD patients with aggravated arterial
hypertension
in three age groups (22-40, 41-59, and 60 years or older). The study covered 246 patients with acute myocardial infarction (140 hypertensives and 106 normotensives) and 182 with postinfarct
cardiosclerosis
(88 patients with arterial
hypertension
and 94 without this condition). All the patients underwent teleradiography, 12-lead EGG, echocardiography, and coronary ventriculography. While comparing the two study groups, the X-ray criteria show deteriorated left and right cardiac functions and altered aorta due to both its atherosclerotic changes and lower left ventricular contractility.

Data on lipid metabolism in 109 cases of endometrial carcinoma and 33 patients in control are presented. Relevant disorders were detected in 72.5% of the study group [stage I obesity--23 (21.1%); II--29 (26.6%); III--25 (22.9%); IV--2 (1.9%)] which was 1.5 times the mean level in controls (51.5%). The abdominal pattern of obesity was predominant (77.7%). Symptoms of
cardiosclerosis
were identified in 93 patients with endometrial tumors (85.3%), 93.5% of the latter group presenting with concomitant hyperglyceridemia. In 68.8% of endometrial carcinoma patients, incidence of arterial
hypertension
was higher than in controls (54.5%). Lipoproteins played a major role in dyslipoproteid pathogenesis involved in obesity and
high blood pressure
. A study of the insertion-deletion (I/D) polymorphism of apolipoprotein AI genes showed two deletion alleles (DD--6%) and one heterozygote (ID--3%) in control group; no deletion alleles were identified in endometrial tumor patients (0.1 (p(0.05). An investigation of the I/D polymorphism of angiotensin-converting enzyme genes identified deletion homozygotes in 30, insertion homozygotes (II)--27, and heterozygotes--41% (control). In endometrial cancer group, the deletion allele distribution was: DD--29; II--28 and ID--45%. Deletion allele frequency in control was 0.485 while in endometrial carcinoma--0.484 (p(0.05), i.e. with out significant difference.

The results of a study dealing with the specific features of silent myocardial ischemia (SMI) in male patients aged over 70 years are presented. Prolonged episodes of silent myocardial ischemia have been ascertained to be more frequently encountered in patients with ischemic heart disease (IHD) as high functional class angina pectoris on exertion, postinfarct
cardiosclerosis
, arterial
hypertension
, atherogenic blood lipid disturbances and they increase a total risk for SMI provided that they are combined. All this leads to destabilization of coronary blood flow and becomes a trigger for the development of prognostically significant cardiac arrhythmias by complicating the course of IHD and increasing the cost of drugs.

Antiarrhythmic activity of oral allapinin was studied in 64 patients with ischemic heart disease. 27 of the patients had undergone coronary artery bypass grafting 2 to 12 months before, 19 patients had postinfarction
cardiosclerosis
, 38--arterial
hypertension
, 30--circulation insufficiency stage I. Arrhythmia presented as ventricular extrasystole (n = 28), paroxysmal atrial fibrillation (n = 18), paroxysmal atrial tachycardia (n = 11), frequent supraventricular extrasystole (n = 7). Arrhythmia continued from 6 months to 8 years. An effective single dose was defined with acute pharmacological test. The treatment course lasted for 21 days. Allapinin proved to be highly effective: ventricular ectopic activity was suppressed in 71.4% patients, atrial tachycardia paroxysms were prevented in 72.7%, paroxysms of atrial fibrillation--in 77.8%. Allapinin tolerance was good. Extracardiac side effects occurred most frequently, but dose lowering was necessary only in 6.2%. The drug was discontinued because of cardiac side effects in 4.7% cases. ECG monitoring is a highly informative method of the treatment efficacy control.

Hemodynamics and ventricular remodeling were studied echocardiographically in 192 men with heart failure (NYHA functional class I), arterial
hypertension
(AH) of stage I-III and clinical picture of ischemic heart disease (IHD). The latter presented in the patients with stable angina pectoris of FC I-II (SAP), unstable angina pectoris (UAP) without foci, paroxysmal atrial fibrillation, acute myocardial infarction (MI), postinfarction
cardiosclerosis
(PC) with SAP or UAP. The control group consisted of 41 healthy men. The patients had AH stage 1. The patients and healthy controls differed significantly by the size of the aorta, left atrium, thickness of the interventricular septum and posterior wall of the left ventricle. There was a significant left-ventucular hypertrophy in the groups with patients with MI, SAP and PC, UAP and PC (p < 0.001). In these groups the type of left ventricular remodeling was characterized as excentric type of left ventricular hypertrophy without its delatation. Normal left ventricular geometry was in healthy men, SAP, UAP, paroxysmal actual fibrillation.
