Midwifery training was started in Chile in 1834. In 1877 a pediatric institute was founded, and a professional organization for midwives was formed in 1919. In 1958 midwives were charged with BCG (bacillus Calmette-Guerin) vaccination of infants. A new professional organization was formed in 1962. In 1965 family planning instruction was initiated in maternity wards because of the high mortality rate after illegal abortions. 5 years later programs were launched on cervical, uterine, and breast cancer for midwives. In 1974 midwives became responsible for contraceptive counseling in a campaign to prevent sexually transmitted diseases. After 1973 the new government cut public health care outlays and staff was reduced by attrition. In 1984 the unemployment rate among newly graduated midwives stood at 34%. After privatization of hospitals 20% of the population got private health care paid by insurance. The employment of midwives increased but mostly in administrative and subservient roles assisting doctors in deliveries. A normal delivery costs $893 and a cesarean $1250, while the net earnings of a midwife amount to $176 a month. Caesareans make up 60-70% of all deliveries, and they are a major cause of complications associated with anesthesia. The state-run El Salvador hospital in Santiago has 11,000 deliveries a year. There had not been a maternal death from childbirth in the previous 19 months. Neonatal mortality is 5.3/1000 live births, and 12% of pregnant women have
high blood pressure
and 9% have
hepatosis
. The average stay is 2.5 days. The rate of caesareans (24.5%) is expected to diminish to 15%. Midwives work 12 hours at a stretch under adverse conditions and stress. A new proposal submitted to the University of Chile wants to abolish midwifery training altogether and replace it by some courses in obstetrics, gynecology, and pediatrics in the nursing curriculum.

Necropsy findings of hepatobiliary system from 78 patients with end-stage renal disease maintained on hemodialysis are reported. Ninety percent of the patients exhibited some abnormalities. Multiple abnormalities often coexisted in each patient. Hepatomegaly was found in 50% of the patients and could be attributed to a discernible cause in all but two of the affected patients who had isolated hepatomegaly. Hepatic congestion was also prevalent and was complicated by fibrosis, cardiac cirrhosis, and centrilobular necrosis and hemorrhage in some patients. This was associated with chronic fluid overload,
hypertension
, and/or cardiovascular disease in the affected patients indicating the importance of adequate control of these factors. Mild periportal hepatic fibrosis, fatty metamorphosis, triaditis, hemosiderosis, and cystic changes were also seen with some frequency--the latter were associated with polycystic kidney disease and were complicated by massive intracystic hemorrhage and abscess formation, each in one patient. Chronic active hepatitis was found in three patients and was associated with chronic HBs antigenemia in one patient and presumed non-A, non-B infection in two. Nearly 22% of the patients showed either cholelithiasis at autopsy or before cholecystectomy due to complications. Significant negative findings included lack of acute viral hepatitis, silicone

Using PIXE (proton-induced X-ray emission), we simultaneously determined the concentrations of Se, Ca, Fe, Cu, Zn, Br, and Pb in blood serum from 56 pregnant women, 25 healthy controls, and 31 others with twin pregnancy or some complicating condition (diabetes,
hypertension
, epilepsy,
hepatosis
gravidarum, pre-eclampsia, small baby), and in cord-blood serum from 21 newborns. Pellets, pressed from the serum samples after addition of yttrium as an internal standard, mixing, and evaporating at 30 degrees C with or without reduced pressure (less than 1 kPa), were bombarded by 2.2 MeV protons from a Van de Graaff accelerator in the air and the induced X-rays collected by a Ge(Li) detector. Relative to mean Se values for early six- to 12-week pregnancy (0.045 ppm), those for 35-42 week pregnancy (0.028 ppm) were low (p less than 0.001). Umbilical cord blood serum showed even lower values (0.016 ppm, p less than 0.001)--findings in harmony with the incidence pattern of Keshan cardiomyopathy. Pb crossed the placenta; values for cord serum were not significantly different from those in pregnancy serum. Cu, Zn, Fe, and Ca showed the significant expected patterns in the different groups. Compared with the late-pregnancy controls, Fe was high in mothers of small-birth-weight babies (1.70 ppm, p less than 0.02). Br was high in pre-eclampsia (3.59 ppm, p less than 0.05) and mothers with twins (3.61 ppm, p less than 0.05).
