Peak systolic (S) to lowest end-diastolic (D) ratios (S/D) of umbilical velocimetry have been used to assess downstream placental vascular resistance and predict adverse pregnancy outcome. The purpose of this study is to assess S/D ratios in patients undergoing cesarean section for clinical fetal distress. Fifty-six patients were identified who had umbilical velocimetry performed during antepartum fetal surveillance (nonstress testing and amniotic fluid index) within 7 days of undergoing cesarean section for fetal distress at Women's Hospital (Los Angeles, CA). The mean gestational age at delivery was 36.5 +/- 2.5 weeks. Thirty (53.6%) patients had elevated S/D ratios (greater than 3), 24 (42.9%) had abnormal amniotic fluid indices, and 20 (35.7%) had abnormal nonstress testing. Group 1 (N = 30) patients delivered small-for-gestational-age (SGA) fetuses and group 2 (N = 26) patients delivered appropriately grown (
AGA
) fetuses. In group 1, 24 (80%) patients had abnormal S/D ratios and 16 (53.3%) had abnormal amniotic fluid indices, compared to only 6 (23.1%) with abnormal S/D ratios and 8 (30.8%) with abnormal amniotic fluid indices in group 2 (p less than .05). In contrast, 14 (53.8%) of the 26 patients in group 2 had abnormal nonstress testing compared to only 6 (20%) of the 30 patients in group 1 (p less than .05). Intra-uterine growth retardation is an important public health problem in many developing countries. The authors conducted a case-control study of low birth weight (LBW) in three teaching hospitals and a population survey in Ahmedabad city, India during 1987-1988. To identify and quantify risk factors for small for gestational age births, we divided the low birth weight and control infants into small for gestational age (SGA, n = 617) and appropriate for gestational age (
AGA
, n = 1851) using an Indian birth weight by gestational age standard. Logistic regression was used to estimate adjusted odds ratios for important risk factors. Prevalence of risk factors was estimated from a community sample survey of mothers (n = 1102) who had delivered in the past year. Attributable risks were calculated from odds ratios and prevalence data. The most important risk factors for SGA was poor maternal nutritional status (weight < 51 kg) with an attributable risk of 42 per cent. Other significant risk factors were anaemia, primiparity, poor obstetric history, lack of antenatal care and
hypertension
during pregnancy, and birth defects, each of which contributed only moderately to the attributable risk. The analysis indicates that improvement of maternal nutrition and antenatal care might prevent a substantial portion of SGA births in this and similar populations.

The purpose of this study was to determine whether infants sufficiently affected by maternal diabetes or

to exhibit abnormal growth (macrosomia, growth retardation) would also display significant alteration in timing of pulmonary maturity (delay or acceleration, respectively). We studied 874 consecutive women with fetal pulmonary maturity testing prior to delivery. Patients were stratified by birth weight into fetal size categories (small for gestational age [SGA], appropriate for gestational age [

], large for gestational age [LGA]). Cases were compared based on maternal disease, fetal size categories and pulmonary maturity testing results. Pulmonary maturity rates based on both phosphatidylglycerol (PG) and lecithin/sphingomyelin ratio (L/S) did not differ between term LGA infants of diabetic mothers (97%) and term LGA (80%) or
AGA
(97%) infants of non-diabetic, non-hypertensive mothers. When compared based on PG alone, there was no difference between the rate of positive PG in term

infants of non-hypertensive, non-diabetic mothers (75%) and that seen in the other pregnancy groups (33-80%). Breakdown by gestational age revealed no significant differences in maturity rates between the study groups. ; Arg1273Arg) has previously been shown to be associated with hyperinsulinemia in nondiabetic Mexican-American subjects. Here, we have investigated the association of this SNP with type 2 diabetes mellitus (T2DM) in French Caucasian subjects. We have observed an increased frequency of the A allele (37.1% vs 27.6%, P=0.0048; odds ratio 1.54), of the AA genotype (15.7% vs 9.8%; P=0.025), and of the combined AA/AG genotypes (58.5% vs 45.5%, P=0.0098; odds ratio 1.69) in patients compared with controls. This association is stronger in the subgroup of patients with age of diagnosis of diabetes equal to or less than 45 years: A allele 43.2% (P=0.0003 compared with controls; odds ratio 1.99), AA genotype 21.4% (P=0.0032), and combined AA/AG genotypes 65.1% (P=0.0022; odds ratio 2.23). Unexpectedly, the G allele is strongly associated with arterial
hypertension
in obese diabetic subjects (GG vs AA odds ratio 19.97). In conclusion, we have observed an association of an SNP in exon 31 of the SUR1 gene with T2DM. Ghrelin has orexigenic effects. It is present in umbilical cord plasma in full-term neonates, raising the prospect that ghrelin plays a role in fetal and neonatal energy balance. We measured ghrelin in small (SGA), appropriate (
AGA
), and large (LGA) for gestational age neonates and evaluated whether ghrelin levels are modulated by neonatal insulin and glucose concentrations. Plasma concentrations of ghrelin, insulin, and glucose were measured in cord blood sampled at birth in 123 SGA,
AGA
, and LGA neonates (gestational age, 24-41 wk) born to mothers with and without diabetes. Ghrelin was detected in samples from all infants. Its concentration was 40% higher in SGA neonates (mean +/- SD, 2436 +/- 657 pg/ml) compared with
AGA
(1738 +/- 380) and LGA (1723 +/- 269) neonates. There was a positive correlation between ghrelin and gestational age in
AGA
/LGA (r = 0.23; P < 0.05) and a negative correlation in SGA (r = -0.67; P < 0.005) neonates. Therefore, the difference in ghrelin between SGA and

The cost of disease mongering to the individual and the community is expected to be high. Some authors have argued that medicalisation of illnesses may not be a problem and the real problem may be the lack of medicines. Doctors will play a key role in combating disease mongering. Disentanglement from the pharmaceutical industry and development of a capacity for critical analysis are required. Educating patients and empowering them to make decisions are important. Several initiatives have been undertaken to combat disease mongering. Initiatives at the level of the patient and the physician are especially important. Studies on the extent and knowledge of disease mongering among doctors and medical students, and their economic and social consequences are urgently required.

is considered to be associated with coronary heart disease but the explanation of this association remains unknown.
Hypertension
is highly prevalent in patients with coronary heart disease. Essential hypertension is linked to hyperaldosteronism and spironolactone, an antihypertensive drug which is a mineralocorticoid receptor antagonist, has been used for a long time in the treatment of androgenic alopecia. We recently observed in a double transgenic mouse model that overexpression of a mineralocorticoid receptor targeted to the skin induced the development of alopecia. We prospectively studied the association of
hypertension
and androgenetic alopecia in Caucasian men. Two hundred and fifty Caucasian men aged 35-65 years were consecutively recruited by 5 general practitioners (50 per practitioner). Data collected included age, androgenetic alopecia score with a simplified Norwood's score (0-4), blood pressure or history of
hypertension
, smoking, history of diabetes mellitus or hyperlipidemia, familial history of androgenetic alopecia, and treatment. Chi-square, Fisher exact tests and linear regression model were used for statistical analysis.
Hypertension
was strongly associated to androgenetic alopecia (p < 0.001). Linear regression tests confirmed that this association was independent of age : odds ratio was 2.195 (95% CI : 1.1-4.3). Familial history of androgenetic alopecia was also strongly associated with androgenetic alopecia : odds ratio was 10.870 (95% CI : 4.3-27.1). Other variables (diabetes mellitus, hyperlipidemia, smoking, treatment) were not associated with androgenetic alopecia. We were limited by a relatively small study sample but in this study androgenetic alopecia was strongly associated with
hypertension
. Association of androgenetic alopecia and hyperaldosteronism warrants additional studies. The use of specific mineralocorticoid receptor antagonists could be of interest in the treatment of androgenetic alopecia.
