The children in the population of the district of Prague 4 were screened for the prevalence of
hypertension
. From the age group 6--11 years (1st--5th forms), a representative sample was selected, comprising 2 152 children; of the age group 12--19 years (6th--9th forms and adolescents), 90% of the population (11 323 individuals) were examined. The arbitrarily set criteria of
hypertension
130/80 mmHg in the children aged 6--11 years and 135/80 mmHg in those aged 12--19 years, were found acceptable for identification of potential hypertonics. In the population examined, such or higher pressures were found in 0.5--3.4% of the subjects examined. By thorough clinical and laboratory examinations of children aged 11--15 years with elevated blood pressures the participation of
secondary hypertension
was determined. In comparison with a control group, these children exhibited statistically significantly more frequent diseases, obesity, and faulty regimen of living, as well as
hypertension
in their parents.

The angiotensin antagonist saralasin (1-sar-8-ala-angiotensin II) was given to 27 patients with different forms of
secondary hypertension
. The blood pressure fell in 6 of 7 patients with renal artery stenosis and in 4 of 10 patients with terminal renal failure on regular hemodialysis. No change or a rise in blood pressure was observed in 3 patients with Cushing's syndrome, 4 patients with primary aldosteronism, 3 patients with

and a unilateral small kidney of other than renovascular origin, and 6 patients with terminal renal failure. It can be concluded from the results that angiotensin II is involved in the pathogenesis of renovascular

and in some cases of

accompanying chronic renal failure.

Previous attempts to assess sympathetic nervous system activity in patients with
hypertension
have used a variety of physiologic, pharmacologic and biochemical techniques. Results have been conflicting and confusing. Recently, the activity in plasma of the catecholamine synthesizing enzyme, dopamine-beta-hydroxylase (DBH), has been proposed as an index of sympathetic nervous system activity. Studies of apparently healthy subjects show that high values (greater than 60 units per liter) for plasma DBH activity correlate with pronounced daily lability of blood pressure and frequent readings greater than 130/85 mm of mercury. Studies of patients referred for evaluation of established
hypertension
show significantly higher values for plasma DBH activity in patients with primary hypertension than in those with commonly recognized forms of
secondary hypertension
-that is, renovascular, renal parenchymal and adrenocortical. Therefore, the measurement of plasma DBH activity may be helpful in the study and differential diagnosis of hypertensive diseases. Measurement of DBH in plasma is inexpensive, reproducible and relatively easy to do.

The elevated level was not reduced when blood pressure was brought to normotensive level by administration of thiazide or rauwolfia. The prevalence of primary and

and of heart and kidney involvement was thoroughly studied in 689 hypertensive subjects derived from a blood pressure screening examination of a total population sample of Swedish men (n = 7,452). The prevalence of

was found to be only 5%, the prevalence of surgically curable
hypertension
being even lower. Left ventricular hypertrophy and slight heart enlargement were each found in about one-third of the hypertensive patients, while severe heart enlargement, left ventricular hypertrophy on ECG, proteinuria, abnormal serum creatinine and urinary sediment were each found in about 5%. On the basis of these findings, a minimum pre-treatment workup in uncomplicated hypertension is proposed.

Study of intrarenal vasculature was carried out by using the metallic impregnation technique on whole kidney sections in 31 [corrected] cases of (primary and secondary)

and 10 normal controls. Distinct patterns of intrarenal vasculature were noted in controls and in cases of

. Gradual tapering of vessels, absence of tortuosity and good peripheral vascularisation were noted in controls. Abrupt tapering, tortuosity of vessels and poor peripheral vascularisation were noted in hypertensive cases. In essential hypertension moderate to severe changes of dilatation of the segmental and/or arcuate arteries was noted. The degree of dilatation was related to the level of systolic BP rather than diastolic in cases of essential hypertension.

even if severe, rarely showed significant dilatation lesions. Avascular zones and conglomeration of vessels at poles was seen only in cases of pyelonephritis. This helped in distinguishing these, from cases of glomerulonephritis.
