A group of 278 patients, over the age of 60 years, and representative of geriatric and general medical admissions to the District General Hospital in Banbury, Oxforshire, was studied to correlate the prevalence of systolic murmurs to age, sex, cardiac failure, ischaemic heart disease, dysrrhythmias,
hypertension
, peripherial
vascular disease
and anaemia. The object was to establish the clinical significance of these murmurs and test a postulate that they could not be dismissed as benign. Seventy-five per cent of the murmurs were judged to be aortic and 12 per cent mitral in origin. The prevalence of systolic murmurs increased with age from 32 per cent at 60-64 years to 57 per cent over 85 years, and was greater in females (44 per cent) than in males (34 per cent). The presence of systolic murmurs was related to the presence of cardiac failure, ischaemic heart disease, dysrrhythmias,
hypertension
, peripheral vascular disease and anemia. Only 8 per cent of patients with systolic murmurs had none of the above-mentioned six cardiovascular abnormalities compared with 36 per cent of patients without such a murmur, while multiple cardiovascular abnormalities were also commoner in the former group. The mortality rate in hospital was similar for patients with or without a systolic murmur.

Ex vivo renal artery reconstruction and autotransplantation is a relatively recent addition to the surgical armamentarium for renal
vascular hypertension
. Ten consecutive patients were considered for this surgical therapy and form the basis of this communication. The patients were treated by a combination of methods including bilateral ex vivo reconstruction, unilateral in situ and contralateral ex vivo reconstruction, and unilateral ex vivo reconstruction and contralateral nephrectomy. Replacement of the diseased segment of the renal artery in all ex vivo reconstruction consisted of arterial autografts including hypogastric artery, splenic artery, common iliac, and external iliac artery. In the ex vivo reconstruction, the ureter was either left intact or was transected and reconstructed by standard ureterovesicle implantation. After surgery all patients became normotensive without antihypertensive medication. Although this is a relatively small series, the uniform good results in these patients with extensive disease suggest that ex vivo renal artery reconstruction is a safe and effective method of treatment. Thus, it should be more widely applicable, expecially in those patients with renal
vascular disease
who were previously thought to be inoperable or eligible for nephrectomy only.

A new strain of genetically obese rat recently obtained in our laboratory exhibits endogenous hyperlipidemia (marked hypertriglyceridemia and moderate hypercholesterolemia) and spontaneous
hypertension
. The animals die prematurely from kidney failure or from the complications of atherosclerosis. A low calorie diet proved to be highly beneficial to these rats. Body weight declined, obesity diminished, the hypertriglyceridemia was almost eliminated, and the hypercholesterolemia was reduced. However, the hypertensive state was not alleviated. Since the life span of the rats was greatly prolonged by a low calorie diet, the latter undoubtedly served to prevent or arrest the development of renal and

2 case histories of retinal damage in users of oral contraceptives (OCs) are discussed. The 1st, a 27-year-old woman who had used Eugynon for a year, had no history of cardiovascular disease of hemicrnaia. Ophthalmic examination showed vision in the right eye as less than 6/120, in the left 6/6, with indications of central serous chorio-retinpathy, with edematous center surrounded by small hemorrhages. After discontinuation of OCs, vision returned to 6/6 and paracentral scatoma disappeared. In the 2nd case a 37-year-old woman who had taken Eugynon and Ovulen for 1 year and Neolyndiol for 2 years complained of the appearance of a wedge-shaped shadow in her right eye. The patient had a history of metrorrhagia and hemicrania on the right side. Vision in both eyes was 6/6, but a paracentral scotoma in the right eye was present, a probable symptom of the patient's hypertensive spastic
angiopathy
. OCs were discontinued and propanol was administered, then discontinued due to symptoms of a scotoma in the left eye. The
angiopathy
disappeared but the ocular lesions remained. Ocular complications occur in about .07% of OC users. The damage results from arterial
hypertension
and changes in the macula lutea. Careful observation of blood pressure and ocular disturbances is the best preventative for ocular damage in OC users.

Herein we describe a case of a 23-year-old male patient with severe

that was refractory to medication, a hypoplastic left kidney and complex renovascular disease. Patient evaluation revealed stenosis of the right renal artery and saccular aneurysms of its three branches. Treatment with converting enzymes was discarded due to the risk of azotemia and the complex
vascular disease
advised against percutaneous transluminal angioplasty. He was therefore submitted to ex situ vascular repair (bench surgery) using the hypogastric artery with its three branches and autotransplantation in the right iliac fossa. The foregoing procedure achieved excellent results. The arterial pressure and overall renal function returned to normal 15 days postoperatively. In our view extracorporeal surgery is the procedure of choice in patients with complex renovascular disease involving the small branches of the renal artery.
