Suprapublic prostatectomies performed on 89 patients from January 1977 to December 1981 were retrospectively evaluated as regard to operative risk, preoperative and postoperative complications. The patients were from 56 to 87 years old with a peak distribution in the seventies. Common preoperative complications were
urinary tract infection
, coronary sclerosis followed by
hypertension
. The operation time for resection and weight of resected adenoma averaged 46.5 minutes and 35.1 g, respectively. Average blood loss during operation was 179 ml. The most frequent postoperative complication was acute epididymitis. There were no operative deaths. Prostatic incidental carcinoma was found in 3 cases of resected adenomas by histological examination. There were statistically significant correlations between adenoma weight and duration of postoperative
urinary tract infection
(r = 0.38, p less than 0.01) and between operation time and blood loss (r = 0.42, p less than 0.01). In contrast, preoperative
urinary tract infection
had no influenced on duration of postoperative
urinary tract infection
or postoperative gross hematuria.

In 259 subjects at risk to have inherited autosomal dominant polycystic kidney disease (PKD), the frequency of symptoms consistent with
urinary tract infection
, haematuria, back and abdominal pain,
hypertension
, renal stones, and end-stage renal failure was evaluated. The diagnosis of PKD was made in 140 of these subjects (54 per cent). At the time of the study, 36 per cent of males and 7 per cent of females with PKD were asymptomatic, normotensive, and denied any previous problems. In patients younger than 30 years, 66 per cent of males but only 11 per cent of females were asymptomatic. In female patients,
urinary tract infection
(69 per cent) and
hypertension
(61 per cent) were the most frequent clinical manifestations. In contrast, in males with PKD, these problems were present in only 19 per cent and 42 per cent, respectively. Frequency of other clinical manifestations was similar in women and men with PKD. End-stage renal failure was present in 5 per cent of the 81 patients younger than age of 40, in 33 per cent of the 27 patients 40-49 years old, and in 47 per cent of the 32 patients aged 50 years or more. Physical examination was unreliable in estimating kidney size in most patients, particularly in early stages of the disease.
Hypertension
and symptoms such as haematuria and back pain, but not urinary tract infections, correlated well with renal size measured by radiograms.

, 4 cases of cholelithiasis, 3 cases of heart diseases and 7 cases of other diseases. The clinical efficacy of CTX in 34 cases of RTI, 11 cases of
UTI
, 8 cases of BTI and 5 cases of other infection was excellent in 11 cases, good in 27 cases, fair in 12 cases, poor in 4 cases and unclear in 4 cases. The over all clinical effectiveness was 70.4%. No adverse reaction was observed except for 2 cases (general fatigue in 1 case and eruption and itching in another). These results obtained should support the usefulness of CTX.

in two. One patient died two months after the nephrectomy, and one allograft was lost because of acute tubular necrosis. Both patients with erythrocytosis had prompt return of the hematocrit level and RBC mass to normal. were classified as excellent in six patients, good in nine, and poor in four. Native renal-vein renin ratios of patients with excellent or good responses were not statistically different when compared with those of poor responders.

treated by renal autotransplantation and 3 cases with extensive renal calculi treated by renal bench surgery and autotransplantation. The cases with renovascular

were due to fibromuscular dysplasia in the trunk of renal artery, and return to normotension and improvement of renal function were obtained within a few days after the operation. In 2 of the cases with extensive renal calculi, the contralateral kidney was contracted. In all cases, the stones were removed completely and

The performance of a nurse practitioner, under the supervision of an internist, in managing inpatients in a rehabilitation hospital was assessed by comparison with three similar groups managed by internists. Criteria were chosen to reflect both quality of care and cost-effectiveness. The medical records of 25 patients discharged consecutively on each service were examined to determine length of stay, laboratory and x-ray costs per day, number of consultations, and mean score on six index conditions. The index conditions score was designed to reflect proper recognition and management of the following common medical problems:

This study reports an evaluation of care given at an urban multidisciplinary community family practice clinic. By means of an "indicator-condition" approach, the criteria and rating system developed for the Burlington Randomized Controlled Trial (BRCT) were applied to 103 randomly selected charts demonstrating 124 episodes of care given for seven specific "conditions": otitis media,

, prenatal care, care of the newborn up to the age of 12 months, immunization up to the age of 24 months, depression and

. Overall, 83 (67%) of the episodes of care studied were rated adequate or superior. The proportion of such episodes varied from 33% for

to 81% for care of the newborn. No statistically significant differences were found between these results and those of the BRCT. A total of 48 instances of inadequate care were noted, of which 21 (44%) were omissions in patient management. Inadequate preventive care and care of chronic diseases was more common than inadequate care of acute infectious diseases. Ureteropelvic junction (UPJ) obstruction is a common cause of hydronephrosis in infants. Newborns with severe obstruction often have marked improvement following correction; therefore early diagnosis and operation is important. From 1973 to 1983, 21 patients were operated on for UPJ obstruction diagnosed under 6 weeks of age. Six patients (29%) had antenatal ultrasonographic diagnosis. The remaining patients were diagnosed by IVP or radionuclide scan for palpable renal enlargment or for associated anomalies. Seventeen had unilateral and four had bilateral obstruction. Twenty-three pyeloplasties, one primary nephrectomy, and one cutaneous pyelostomy with subsequent nephrectomy were done. All pyeloplasties were dismembered, with tailoring of the renal pelvis. Postoperative renal function was followed with radionuclide scan or IVP. Postoperative complications included a single
urinary tract infection
in three patients and two bowel obstructions. One early postoperative death occurred in an infant with bilateral obstruction who developed congestive heart failure secondary to severe uncontrollable
hypertension
. There were two other unrelated late deaths. Documented functional improvement with minimal complications follow unilateral or simultaneous bilateral pyeloplasty in newborns with UPJ obstruction.
