In the syncytiotrophoblastic surface of the toxemic full-term placentae, the sialates and their consequent negative charging were studied by both electron microscopic histochemistry (Ferritin method) and Western blot analysis. Evidence is presented showing (1) that the reactions with both the ferritin labelled Limulus Polyphemus Agglutinin (LPA-ferritin), specific to sialates, and the cationized ferritin, specific to a negative charge, decrease in the specimens of toxemic placentae, (2) that the density of sialates in placentae of severe gestational proteinuria or/and
hypertension
is statistically lower than that of severe gestational edema, and (3) that there is no difference between the blotted bands in normal placentae and toxemic ones regardless of the severity of the
toxemia
. These results indicate that the reduction of the surface negative charging is demonstrated in toxemic syncytiotrophoblasts and appears to be secondary to the decrease in the amount of sialate.

This is a presentation of clinical progress and outcomes of pregnancy in 240 women with cerebral tumors (n-16) and pseudotumors (n-224). Etiologies of cerebral pseudotumors included inflammatory diseases of the nervous system (n-86), cerebrovascular disease (n-81), benign intracranial

(n-20), brain injuries (n-19), late

of pregnancy (n-13) and cranial malformations (n-5). The disease first manifested during the gestation in 103 women. Pregnancy aggravated clinical progress of preexisting disease in 96 (79.3%) of 121 patients with pseudotumors and in 15 (93.7%) of 16 patients with cerebral tumors. Delivery was normal in 124 of 298 pregnancies, premature in 30 pregnancies and operative in 25 pregnancies; 109 pregnancies were terminated by induced or spontaneous abortions. Ten women died before delivery.

Etiology and pathogenesis of focal and segmental glomerulosclerosis (FSGS) in patients with
toxemia
of pregnancy remain controversial. We examined 15 preeclamptic patients presenting with nephrotic syndrome. None of the patients had urinary abnormalities and
hypertension
before pregnancy. Clinically, proteinuria first developed during pregnancy and disappeared completely in all but one patient lost to follow-up after 1-30 months from delivery. Renal dysfunction,
hypertension
and edema rapidly resolved in the postpartum period. None of the patients had a progressive clinical course. Renal biopsy specimens obtained postpartum revealed typical features of preeclamptic nephropathy. In addition, findings compatible with FSGS were observed in 13 patients including 4 in which such lesions were unearthed by additional serial sectioning. These results indicate that FSGS may not only be induced by preeclampsia but also be one of the representative glomerular changes in preeclamptic patients with nephrotic syndrome. A favorable clinical course ensues in a manner similar to that of patients with the garden - variety of preeclampsia.

The relationship between placental and peripheral concentrations of pregnancy-associated plasma protein A (PAPP-A) was investigated, using specific radioimmunoassay techniques. At term, placental concentrations of PAPP-A were significantly lower in diabetic pregnancies; 1.9 +/- 0.2 micrograms/mg (mean +/- SE) placental protein as compared with controls (3.2 +/- 0.3). Similar findings were noted when results were analyzed per total placental weight. Patients with classes C-R (1.2 +/- 0.3) had significantly lower PAPP-A levels than in classes A, B (2.4 +/- 0.3 micrograms/mg protein, P = .02). The mean birth weight of the infants of obese women was 163g greater than that of the control subjects; no difference was observed in infant length or gestational age. These results, while confirming that obesity is an important risk factor, suggest that methodological aspects of the previous studies may have contributed to magnify the severity of the problem.

A number of human studies presented at the workshop indicate that the premature infant at birth is biochemically deficient in docosahexaenoic acid (DHA) in both the brain and liver phospholipids, and that DHA is essential for normal visual acuity. The amount of DHA necessary to maintain normal amounts of the liver and brain phospholipids postnatally is 11 mg/kg daily. Elderly patients on long-term gastric tube feedings and others on long-term intravenous fluids and on total parenteral nutrition are particularly prone to deficiencies of alpha-linolenic acid, eicosapentaenoic acid (EPA) and DHA. The amounts estimated to prevent deficiencies in the elderly are 800-1100 mg/d of alpha-linolenic acid and 300-400 mg/d of EPA and DHA combined. Preliminary data indicate that children with malnutrition and mucoviscidosis, women with

, and elderly people have decreased amounts of DHA in plasma phospholipids. The omega 3 fatty acids lower triglycerides and, at high levels, lower cholesterol. The anti-aggregatory, anti-thrombotic and anti-inflammatory properties of omega 3 fatty acids have been confirmed, and a dose-response curve is emerging. We have obtained some new findings from the observation and management of 15 pregnancies and deliveries, all complicated with IgA nephropathy, which was diagnosed by open renal biopsy before each pregnancy. The classification of IgA nephropathy was from Grade I to Grade IV according to Nomoto et al. . The criteria were based on the classification of
toxemia
of pregnancy of the Committee for Toxemia of Pregnancy. Japan Society of Obstetrics and Gynecology. We referred to laboratory data such as complete blood counts, coagulation tests, blood chemistry tests, urinalysis and renal function tests. We also referred to Amagasaki's criteria which indicate whether the pregnancy and delivery will be normal or not. Three cases with Grade I IgA nephropathy all had normal courses. Nine cases with Grade II satisfied the criteria of Amagasaki for normal delivery, but during the third trimester, proteinuria was recognized in seven cases, edema in one case and
hypertension
in two cases. Three cases with Grade III were all met the criteria for abnormal pregnancy and delivery. Only one case showed proteinuria from the first trimester, but she had no obstetrical complications or deterioration of renal function during her course. There were 12 vaginal deliveries and three Cesarian sections. All infants were in good condition except for one intrauterine fetal death. In view of the above results, we concluded that patients with Grade II IgA nephropathy could not continue their pregnancies safety. A case of 30 year-old female with HELLP syndrome, who had undergone emergency caesarean section under general anesthesia, was reported. HELLP syndrome is characterized by hemolysis, liver dysfunction and thrombocytopenia, besides symptoms of severe

of pregnancy. After an awake orotracheal intubation, anesthesia was maintained with nitrous oxide, oxygen and muscle relaxant. Blood pressure was controlled with intravenous administration of nitroglycerin. Though the eclampsia was recognized several times during and after the operation, the patient and her baby had no complication nor sequela on their discharge. The key in the anesthetic management of caesarean section in a patient with HELLP syndrome is to control

and eclampsia, to consider the presence of liver and kidney dysfunctions, and to improve anemia and bleeding tendency.

A case of bilateral exudative retinal detachment associated with otherwise normal pregnancy is presented. Similar cases, but associated with

of pregnancy from the literature are discussed. As far as we know, this is first reported case where bilateral exudative retinal detachment presented in a normal pregnancy and the re-attachment started promptly after a normal, spontaneous delivery. The re-attachment was complete, and the visual acuity after one year was 20/20 o.u. This case supports earlier theories that

is not the only cause of exudative retinal detachment during pregnancy.
