Human sympathetic nerves have a high-affinity norepinephrine uptake system. This uptake system is inhibited competitively by chlorpromazine but not by molindone, which suggests that molindone will not interact adversely with guanethidine, an antihypertensive drug that enters sympathetic nerves via the high-affinity uptake system. Accordingly, patients with concomitant schizophrenia and hypertension were treated simultaneously with molindone and guanethidine; there was no evidence of an adverse drug interaction. The data indicate that molindone and guanethidine can be used in combination safely and effectively.

The distribution of some risk factors on different psychiatric diseases in 1726 hospitalized patients were investigated within a prevalence study. To all tested factors there was a contrary behaviour of oligophrenic patients opposite to the group with a schizophrenia, organic and affective psychotics. Hypertension only was found more frequently in non psychotic psychopathia and oligophrenia than in psychosis. Therefore the prevalence of hypertriglyceridemia, hypercholesterinemia, diabetes, specific Ecg-changes, smoking of cigarettes and severe physical inactivity in oligophrenics was less except in all other group of diseases, also in consideration of the different frequency of psychopharmacal treatment. Electrocardiographical signs for coronary heart diseases also show a similar distribution of frequency. Likewise in psychiatric patients seems to be an agreement with the coronary morbidity and the somatic risk factors.

In 1969, a Pacific Northwest American Indian community cohort (n = 100) was interviewed for the presence of physical and psychiatric illnesses. The same community was studied again in 1988. This study describes the outcome among the original 100 subjects. The schedule for Affective Disorders and Schizophrenia Lifetime Version (SADS-L) served as the basic interview instrument, supplemented by data from medical records, death certificates, and medical and community informants. Twenty-five subjects had died, 13 from cardiovascular disorders and seven from alcohol-related illnesses. Among the 46 subjects re-interviewed, hypertension, heart disease, and diabetes had become significant sources of medical morbidity. Alcoholism was the most significant cause of psychiatric morbidity, particularly among males. This study indicates that greater attention should be focused upon prevention and treatment of alcoholism, cardiovascular disorders, and diabetes in this community and in other American Indian populations.

The past decade has seen important progress in understanding the localization, pharmacology, and function of serotonin (5-HT) receptor subtypes. At least seven subclasses have been shown to exist, and evidence is emerging to suggest further subclassification. Serotonin is involved in numerous physiological processes (e.g. feeding, sleep, pain, sexual behavior, temperature regulation) and pathophysiological ones. Serotonin reuptake blockers have been found effective in the alleviation of depression and attacks of panic, and are at varying stages of clinical evaluation in the treatment of obsessive compulsive disorder, chronic pain, and bulimia nervosa. Selective potent serotonin receptor agonists and antagonists show promise in the treatment of migraine, nausea and vomiting, schizophrenia, anxiety, hypertension, and Raynaud's disease.

The intensive research in molecular pharmacology over the past few years has provided completely new insights into the mechanisms underlying pharmacological specificity. At the molecular level, signal transmission from the receptor to the cellular effector mechanism manifests far greater diversity than formerly supposed. In part, this diversity is due to the combination of separate molecular entities into a pattern specific for a given cell type. An added complication is the frequent crosstalk between different receptor signal transmissions. Thus, where specificity is concerned, only rarely is it a question of a specific molecular entity that can be targeted for the action of pharmacological agents. Moreover, disease may be due to an imbalance in signal transmissions that can only be combated by a multiple targeting approach. Accordingly, although the techniques of molecular biology are invaluable in generating new understanding of the mechanisms underlying drug specificity, they may prove disappointing as applied to actually tailoring specific drugs for the treatment of such major human ailments as hypertension or schizophrenia.

A 34-year-old black male with hemoglobin AS was admitted for renal failure, polydipsia, hypertension, schizophrenia, mental confusion, and visual hallucinations. Abnormal electrolytes were corrected by dialysis, but blood specimens were reported as hemolyzed with hyperkalemia. Peaked T waves on electrocardiographic analysis were followed by cardiac arrest. An autopsy revealed sickled cells in the visual cortex and other symptomatic organs, but normal erythrocytes in most of the vascular tree. These findings suggest true progressive sickle cell crisis in a hemoglobin AS patient.

American former prisoners of war (POWs) are an aging group who seek health care with increasing frequency. To examine the prevalence of long-term physical and emotional consequences of captivity in this population, the authors analyzed medical and psychiatric examination data for 426 former POWs. Detailed psychiatric diagnostic criteria were used to assess the POWs' mental health. Compared with general population groups, POWs had moderately elevated lifetime prevalence rates of depressive disorders and greatly elevated rates of posttraumatic stress disorder (PTSD), although their rates of hypertension, diabetes, myocardial infarction, bipolar disorder, schizophrenia, and alcoholism were not elevated. POWs who lost more than 35 percent of their body weight during captivity had higher rates of anxiety disorder, depressive disorders, PTSD, and schizophrenia, compared with other POWs.

Previous studies have focused on the role of the central nucleus of the amygdala (CeA) in cardiovascular and other amygdaloid functions. The combined retrograde tracing/immunohistochemical method was used to test for the presence of enkephalin, neurotensin, neuropeptide Y, and catecholamine neurons within the nucleus of the solitary tract that send efferents to the CeA. After injections of retrograde tracer into the CeA, retrogradely labeled neurons were observed within the caudal, medial nucleus of the solitary tract. Most CeA-projecting neurons were located ipsilaterally within the medial nucleus of the solitary tract at the level of the area postrema. Retrogradely labeled enkephalin- and neurotensin-immunoreactive neurons were found within the medial nucleus of the solitary tract at this level, while retrogradely labeled neuropeptide Y-immunoreactive neurons were found within the medial nucleus of the solitary tract rostral to the area postrema. About 60-74% of CeA-projecting cells were also immunoreactive for tyrosine hydroxylase. Approximately 9% of retrogradely neurons were phenylethanolamine-N-methyltransferase immunoreactive. The results provide evidence that within the nucleus of the solitary tract, peptidergic CeA-projecting neurons have a topographic distribution. In addition, noradrenergic neurons within the A2 group, rather than adrenergic neurons of the C2 group, provide the bulk of catecholaminergic input to the CeA from the nucleus of the solitary tract. Cell counts indicate that each of these peptides may be colocalized (to varying extents) within catecholamine-producing neurons. Also the catecholaminergic and enkephalinergic contribution to the ascending pathway from the nucleus of the solitary tract to the CeA distinguishes it neurochemically from the descending pathway. Thus, although there are afferent and efferent connections between the nucleus of the solitary tract and CeA, their peptidergic/neurotransmitter connections are not necessarily reciprocal. Input from nucleus of the solitary tract peptidergic and catecholaminergic neurons to the CeA may be important in the etiology of a number of pathophysiological conditions including hypertension, gastric ulcers, and schizophrenia.

The authors describe an autopsy case of glioblastoma occurred after 38 years received lobotomy. The patient was a 72 year-old male, who received lobotomy at 34 year old against schizophrenia. CT scan taken at 72 year old showed irregular low density areas without mass effect in the bilateral frontal white matter adjacent to the anterior horn. After 4 months, the signs of intracranial hypertension were observed and his consciousness was disturbed abruptly. CT scan revealed ring enhancement with marked mass effect in the left frontal lobe. A biopsy specimen from the tumor showed a picture of anaplastic astrocytoma. Family rejected the remission maintenance treatment. The patient died 3 months later the onset. At autopsy, a large tumor occupied in the left frontal lobe was recognized. The tumor demarcated poorly from the cerebral tissue and invaded into the left anterior cingulate gyrus and the corpus callosum. Histologically, tumor cells composed of fibrillary, gemistocytic and multinucleated astrocytes. GFAP, NSE and vimentin were found in large cells. Histological diagnosis was glioblastoma. It was suggested that the tumor occurred from the region around a cyst of prefrontal lobotomy in the left frontal lobe. In the right frontal lobe, a large cyst in size of 30-18 mm was present in the centrum semiovale. The wall of cyst was composed of layer of glial scar tissue. The origin of the cyst was discussed.

Accurate volume determination of the encephalic ventricles is of importance in several clinical conditions, including Alzheimer's presenile dementia, schizophrenia, and benign intracranial hypertension. Previous studies have investigated the accuracy with which magnetic resonance imaging (MRI) can be used in clinical practice to evaluate the encephalic ventricles. However, adequate evaluation of pathological conditions depends on a sufficient amount of morphometric data from normal subjects. To begin establishing this data base for "normal" subjects, we evaluated the MRI scans of 38 subjects found to have no apparent pathology and calculated the ventricular volume in each case by using methods previously developed in our laboratory. The results were then compared with published volumes determined from studies that used either ventricular casts or computerized tomographic scans. The average total ventricular volume for all 38 subjects was 17.4 cm3, while that for males was 16.3 cm3 and that for females was 18.0 cm3. A small but significant correlation was found between age of subject and ventricular volume, with ventricular size increasing with age.
