The relation between education, prevalence of 17 chronic diseases or groups of diseases, and pattern of health care utilisation was evaluated from data from the 1983 Italian National Health Survey, based on 58 462 individuals aged 25 or over randomly selected within strata of geographical area, size of place of residence, and size of household, in order to be representative of the whole Italian population. , myocardial infarction and other heart disease, haemorrhoids or varices, chronic respiratory disease, anaemias,
gastroduodenal ulcer
, cholelithiasis and liver cirrhosis, kidney and urological diseases, arthritis, and psychiatric and neurological disturbances, were consistently less prevalent among more educated individuals. The age and sex adjusted risk estimates for individuals educated in high school or university compared with those with only a primary school education or less ranged between 0.21 for liver cirrhosis and 0.80 for anaemias. The sole exception was allergy, which was more prevalent among the more educated individuals (relative risk = 1.42). General practitioner visits and hospital admissions were reported less frequently by the more educated individuals, but specialist consultations of potential preventive value were less frequent among the less well educated. The results were similar when occupation was utilised as an indicator of social class. Thus, the findings of this national survey provide confirmation and quantitative assessment of considerable differences in health and health service utilisation according to indicators of social class.

) demonstrated that a single therapeutic dose (0.002 mg/kg) of clonidine, administered to individuals with normal hemodynamic parameters, slightly reduced their heart rate (by 7-8%), and arterial blood pressure (by 12-19%), had a modulating effect on blood aggregation, unaffected blood sugar and depressed dramatically basal gastric secretion of acid and pepsin, and, to a lesser extent, that of bicarbonates. This range of clonidine effects suggests that it may be useful in combined cases of arterial
hypertension
and gastric hypersecretion, yet the treatment should be relatively short-term and low-dose, because of a risk of gastric vasoconstriction.

, and anxiety was diagnosed as having nonarteritic anterior ischemic optic neuropathy (AION). The patient presented with unilateral disc edema and a paracentral scotoma. Follow-up examination at 10 weeks revealed resolution of the scotoma. This is one of a few cases in which remission of the visual field defect has been reported.

Using questionnaire and physical screening examination data for a general population of 4,962 adults aged 18 to 61 years enrolled in the Rand Health Insurance Experiment, we calculated the prevalence of 13 chronic illnesses and assessed disease impact. Low-income men had a significantly higher prevalence of anemia, chronic airway disease and hearing impairment than their high-income counterparts, low-income women a higher prevalence of congestive heart failure, diabetes mellitus,

, hearing impairment and vision impairment. Of our sample, 30% had one chronic condition and 16% had two or more. Several significant pairs or "clusters" of chronic illnesses were found. With few exceptions (diabetes,

240 patients operated on for PHPT in the years 1956-79 were compared with 240 sex and age matched randomized controls to examine the present state of health of the patients. Diseases which occurred in both groups were registered to clarify their possible association with PHPT. Recurring urinary tract infections occurred as often in both groups, though renal stones had naturally been more frequent in the PHPT group. Serum creatinine was elevated in 33 PHPT patients and in 9 controls (p less than 0.001), and renal damage was also more severe in the PHPT patients. Malaise and fatigue occurred in 29 PHPT patients and in 14 controls (p less than 0.05). Medically treated
hypertension
was found in 95 PHPT patients and in 53 controls (p less than 0.001). Eight PHPT patients and one control had had pancreatitis (p less than 0.01).
Peptic ulcer disease
and gallstones were registered as frequently in both groups. 16 PHPT patients and three controls had received previous irradiation to the neck region (p less than 0.01). Six PHPT patients, but none of the controls, had had a differentiated non-medullary thyroid carcinoma. Two of the thyroid carcinoma patients had received irradiation. Other malignant diseases were as frequent in both groups. Three controls were hypercalcaemic and had PHPT verified at operation. The prevalence of PHPT in the control group was 1.25%. The state of health was normal in 52% of the PHPT patients and in 68% of the controls (p less than 0.001). The results indicate that PHPT permanently deteriorates the health of the patients and should therefore be diagnosed and treated as early as possible in the course of the disease.

Sixty-six patients were seen from January 1963 to December 1970 in whom a diagnosis of analgesic nephropathy was made. The ratio of women to men was 2.7:1 and women presented at an earlier age. Over 60% of patients had or developed urinary tract infection and over one-third presented with serum urea levels over 300 mg/100 ml.

was present in 60% of patients and almost 20% had a history of
peptic ulcer
; hysterectomy had been done in 35% of the women. Most patients improved initially but more than half of those followed up died within five years of presentation. A direct relation was found between prognosis and the degree of functional renal impairment at presentation.

A postal survey among 2% of men in Leeds showed that the prevalence of urinary stone disease is 3.8%. The prevalence of upper urinary tract and spontaneously passed stones increases progressively from 0.7% in social class 5 to 5.0% in social class 1 but that of bladder stones (0.7% in the group as a whole) is independent of social class. There is an initial peak of upper urinary tract and spontaneously passed stones commencing at age 20 and having a projected prevalence at age 90 of 5.7% and a second peak of bladder stones, commencing about age 50, with a projected prevalence of 1.9%. The prevalence of stone disease increases according to the order: single less than divorced/separated less than married less than widowed men. A family history of stones tends to be higher amongst relatives of stone-formers than amongst the corresponding relatives of control subjects, the male/female ratio being 2:1. The occurrence of urinary stones is significantly associated with that of gallstones,

The plasma and 24-hr urinary levels of cyclic adenosine 3':5'-monophosphate and of cyclic guanosine 3':5'-monophosphate (cGMP) were determined for 19 healthy normal patients, 54 patients with six types of nonneoplastic diseases (cholelithiasis,
peptic ulcer
, coronary heart disease,
hypertension
, regional ileitis, and cirrhosis), and 54 patients with five types of neoplastic disease (cancers of the lung, colon, and breast, acute myelocyte leukemia, and Hodgkin's disease). The cyclic adenosine 3':5'-monophosphate levels of urine and plasma in normal subjects, in noncancer subjects, and in cancer subjects did not differ significantly. The cGMP levels in the noncancer group were similarly unchanged from those in the normal group. However, mean cGMP levels in the urine and plasma of patients with neoplastic diseases were, respectively, 2- and 3-fold greater than the normal values (p less than 0.005 for urine and p less than 0.05 for plasma). Pharmacokinetic studies with [3H]cGMP in nine healthy controls and 15 patients with neoplasia showed that the mean production rate of this nucleotide in patients with metastatic cancer was elevated when compared to normal patients, but many values fell within the normal range. In acute leukemia, the production rate was seven times normal, with four of five patients having values clearly outside the normal range. The plasma clearance rate in patients with neoplasia was not decreased when compared to that in normal patients. It is proposed that an increased production rate, rather than any change in plasma clearance, accounts for the increased levels of cGMP in the plasma and urine of some patients with neoplastic disease.

A retrospective study of 55 patients with panic disorder referred for psychiatric consultation by primary care physicians is presented. Eighty-nine percent of the patients initially presented with one or two somatic complaints, and misdiagnosis often continued for months or years. The three most common presentations were cardiac symptoms (chest pain, tachycardia, irregular heart beat), gastrointestinal symptoms (especially epigastric distress), and neurologic symptoms (headache, dizziness/vertigo, syncope, or paresthesias). Eighty-one percent of patients had a presenting pain complaint.
Hypertension

were the most common medical diagnoses, and depression and alcoholism the most frequently associated psychiatric diagnoses.
