The physical-disease charcteristics of 125 skid row and 736 non-skid row male alcoholics were compared in detail to determine whether skid row alcoholism is characterized by a distinct medical, as well as a social, profile. Trauma, tuberculosis, venereal disease, and malnutrition were more common in the skid row alcoholics. Epilepsy, peripheral neuritis, acute brain syndromes, chronic brain disease, and lifetime recordings of all nervous system illnesses also occurred more frequently in the skid row group, as did
gastritis
, gastrointestinal hemorrhage, ulcer surgery, and postgastrectomy syndrome. Fatty liver,
hypertension
, ischemic heart disease, cardiomyopathy, and cardiovascular illnesses of all kinds, however, were less common. The skid row medical profile is, in part, the product of a unique sociologic environment. Thus, skid row alcoholism may be viewed as a distinct sociomedical entity.

The aim of this study was to evaluate the long-term prognosis for patients suffering from
gastritis
associated with Helicobacter pylori infection, and in particular the proportion of cases progressing to peptic ulcer. The study was carried out in one urban general practice. One hundred and three patients who had presented with dyspepsia over the 1973-80 period and who were found to have a macroscopically normal endoscopy were reassessed between seven and 14 years later. Gastric antral biopsies had been taken routinely at endoscopy and were subsequently re-examined for the presence of H pylori. The patients' medical records were examined to establish their consulting rates over the follow-up period and whether they suffered from any other medical conditions. Patients were interviewed to assess the course of their dyspeptic symptoms. Comparison of patients who were unequivocally H pylori positive with those who were negative revealed no significant differences in the consultation rate for gastroenterological symptoms, in the proportion of patients referred to a hospital consultant or for further gastroenterological investigations or in the proportion reporting that their symptoms had improved. However, a statistically highly significant relationship was found between H pylori infection and
hypertension
. The results of this study have shown that there is a good prognosis for non-ulcer dyspepsia whether or not H pylori infection is present. The association between H pylori
gastritis
and
hypertension
clearly merits further investigation.

This study examined the physical complaints, mental complaints and psychiatric disorders in software engineers. Subjects were 101 male software engineers. They were evaluated by a semistructured interview. Psychiatric diagnosis was based on DSM-III (Diagnostic and Statistical Manual of Mental Disorders, third edition). The results are summarized as follows: 1) Physical complaints were observed in 68% of the subjects with 25% of the subjects complaining of physical ill-health. This study showed that 12% of the subjects had
hypertension
, 12%,
gastritis
or gastro-duodenal ulcer, 14%, allergic disease, and 19% miscellaneous diseases. 2) Of the subjects 62% had mental complaints and 31% mental ill-health. Depressive symptom was the most frequent mental complaint with 32% of the subjects diagnosed as DSM-III. The common diagnoses were adjustment disorders (19%), major affective disorders (6%), psychological factors affecting physical condition (5%) and dysthymic disorder (3%). Only one subject sought professional help from a psychiatrist for relief of mental complaints. The data suggest the severity of mental ill health in software engineers, but methodological limitations preclude a firm conclusion at this time. Further studies should be made on the mental health state in software engineers.

The author summarizes results of a biorhythmological analysis of the frequency of exacerbations of main diseases of the gastrointestinal tract and cardiovascular system. A seasonal character was established for some of these diseases--gastric and duodenal ulcer, chronic
gastritis
, ischemic heart disease, arterial
hypertension
, neurocirculatory dystonia of the hypertensive type. It is recommended to use these results for chronoprophylaxis of the mentioned pathological conditions.

Mid-luteal phase total urinary estrogen excretion and pregnanediol levels were monitored in 112 self-selected, regularly menstruating women who had been sterilized 2 or more years before, and compared to 55 controls. The controls had neither used contraception or ovulation identification methods within 3 months, nor had they had gynecologic surgery. The mean total estrogen excretion of the study subjects was significantly lower than that of the controls, 29.6 vs 36.0 mcg/24 hours (p0.0001). When the data were presented as histograms, 25% of the study group had estrogen means below the 10 percentile value for controls. The histograms were skewed to the right for both groups. Results for pregnanediol analyzed in the same way did not differ significantly, although 23.4% did have pregnanediol excretion below the 10 percentile level controls. The study subjects were categorized into 3 groups: those with abnormal uterine bleeding, physical problems, and psychological problems. Some other observations included the negative correlation between estrogen and pregnanediol excretion and serum cholesterol levels; as association of low estrogen with abnormal menstrual bleeding; and an association of low estrogen and pregnanediol with physical problems. The authors suggested a theory of localized
hypertension
and tissue damage to explain poor ovarian function in these sterilized women. Female hormone production has been linked with some of the problems found in these groups, such as weight gain, osteochrondritis,
gastritis
, irritable bowel syndrome, dysmenorrhagia and breast adenosis.

The selective shunt concept has been validated both physiologically and clinically for the nonemergent patient with variceal bleeding. The patency rate of the shunt has been good and long-term control of gastroesophageal bleeding excellent. Furthermore, the transsplenic decompression of the upper stomach and lower esophagus has effectively prevented the problems related to
gastritis
and gastric varices seen with nonshunt procedures. Continued portal perfusion of the liver has been sustained for more than 9 years in some patients, and 9 of 11 patients have portal perfusion exceeding 7 years. The metabolic problems of the portaprival syndrome have been greatly ameliorated, and long-term survival with clinical well-being has been achieved. For the actively bleeding patient, acute renal vein
hypertension
needs to be prevented to provide effective control of hemorrhage. Selective splenocaval shunts have been successful in this regard in a small number of patients.

Four biopsy specimens of antral and body mucosa were taken from the greater and lesser curvature of the stomach in 557 Germans and 46 Turks with no gastric or duodenal lesions. The age-adjusted
gastritis
score in each area was related to various habits and clinical diagnoses. In contrast to the Germans the Turks had advanced antral
gastritis
at an early age which did not progress with age. Patients of low social class were more prone to antral
gastritis
than academics and civil servants.
Hypertension
was associated with a lower incidence of antral
gastritis
. Patients who had had a cholecystectomy had advanced
gastritis
in antral and body mucosa as compared with those with gallstones but intact gallbladders. The incidence of fundic
gastritis
was higher in patients with degenerative joint disease than in the controls. It is concluded that alcohol, nicotine, drug abuse and various diseases do not play an important role in the frequency of occurrence of
gastritis
.

After stating and evaluating the different ulcer theories in reference to the morphological, physiological and biochemical premises, the integrating components of vascular factors for the ulcer genesis were generally exposed. Apart from the magical 50th birthday after which the old age ulcer in terms of Spang should develop, the clinical appearance of the ulcer in old age can correspond with the following morphological equivalents: 1. with increasing age the reduction of the turnover rate of gastric mucosa epithelium 2. with increasing age a general reduction of the lymphatic reticulum tissue and consequently of the immunological defense reaction in the stomach 3. atrophic mucosa in old age with/without
gastritis
4. increase of intestinal metaplasia with age 5. as a result of 1-4 increased qualitative and quantitative faulty composition of the mucus barrier 6. changes in the intramural stomach arteries a) secondary sclerosing proliferating arteries b) loss of elasticity of the elastic muscle accompanying apparatus of the stomach arteries c) primary sclerosis of the preterminal vascular system by diabetes mellitus as well as
hypertension
d) due to the vascular sclerosis as well as elastosis with obliteration of the perivascular shifting layer increased danger of bleeding and the impossibility of vessel invagination as a method of spontaneous blood clotting after erosion.

A study was conducted to determine the prevalence and incidence of stress-associated illnesses and to identify possible sources of stress among students enrolled in the M.D.-Ph.D. program of a midwestern university. Specially constructed questionnaires were completed by 45 M.D.-Ph.D. students, 334 M.D. students, and 42 Ph.D. students at this university. The information obtained from the M.D.-Ph.D. students was compared with the responses of the M.D. students and the Ph.D. students. Male M.D.-Ph.D. students had a significantly higher incidence of
hypertension
, colitis, and asthma, while female M.D.-Ph.D. students reported a higher usage of antacids and occurrence of
gastritis
or ulcers during the time that they were enrolled in their programs. M.D.-Ph.D. students indicated that the fear of flunking out of school and the grading system were more stressful than did the M.D. students. Additionally, relationship with the graduate adviser and the graduate committee was more stressful to M.D.-Ph.D. students than to Ph.D. students.

Gastric and duodenal motor function was studied using ionoballoono-tensiokinesiography method in 77 patients with chronic calculous cholecystitis. Various periodic motor activity (PMA) infringements, both hypo- and hyperkinetic types, were diagnosed, mainly associated with duodenal
hypertension
, duodenogastric reflux and reflux-
gastritis
. Principles of pre- and postoperative correction of the PMA state for the guarantee of cholecystectomy functional full value were determined.
