Cerebral atherosclerosis
without luminal narrowing has been found macroscopically and by angiographic examinations in some patients with cerebral hemorrhage. In order to clarify the histology of non-stenotic atherosclerosis of the cerebral vessels, we examined cleared specimens and serial sections of the main trunks of the cerebral arteries. The middle cerebral artery was selected in 20 cases of cerebral hemorrhage and 7 cases of cerebral infarction. Non-stenotic atherosclerosis was found frequently in cases of cerebral hemorrhage, while most patients with cerebral infarction showed stenotic cerebral atherosclerosis. We counted the numbers of medial smooth muscle cells in 10 autopsied cases of cerebral hemorrhage and 6 of cerebral infraction. The mean numbers of smooth muscle cells per unit area in the patient with cerebral hemorrhage were less than those in cerebral infraction. In cerebral hemorrhage, the main trunks of the cerebral arteries were dilated, probably as a result of the damage to medial muscle cells and higher blood pressure during the course of intimal thickening. hypertension
spreads to the peripheral, small arteries through the main trunks without luminal narrowing of the cerebral vessels.

Frequency of cerebrovascular diseases (CVD) and their underlying vascular lesions were analyzed in 724 autopsy cases, aged 40 years and over, in the community of Hisayama, Japan during the period 1961 to 1981. Cerebral infarction (CI) was more frequently found at autopsy than cerebral hemorrhage (CH) with a ratio of infarction and hemorrhage of 4.4. Small CI occupied 75.7% of the cases with CI. The cases with any type of CVD showed more severe atherosclerosis of the major cerebral arteries than did those without CI or CH.

of those with large and medium CI was the greatest, and with decreasing severity in those with small CI and with CH sequentially. Fibrinoid necrosis of the intracerebral small arteries was frequently found in cases with
hypertension
and particularly associated with CH. The decline in frequency of CH was confirmed; however, changes in frequency of CI were not evident. Fibrinoid necrosis was also reduced, although the severity of cerebral atherosclerosis showed no definite change. The decline of CH seemed to be ascribed to the reduction of fibrinoid necrosis of the intracerebral small arteries.

Cerebral atherosclerosis
vascular stenosis is a common etiology for ischemic stroke and a major factor in recurrent stroke and vascular mortality. Recent studies suggest that renalase plays a role in
hypertension
and ischemic stroke, and may be involved in atherosclerosis. The aim of the present study was to investigate whether there were correlations between single-nucleotide polymorphisms (SNPs) in the renalase gene and severity of intracranial cerebral atherosclerotic vascular stenosis in ischemic stroke patients as determined by imaging. A total of 212 ischemic stroke patients and 244 healthy controls from the north Chinese Han population were enrolled in this study. Polymerase chain reaction and ligase detection reaction were used for SNP analysis. We classified the case samples by severity of the intracranial cerebral atherosclerotic vascular stenosis. Allele, genotype, and haplotype were analyzed in cases and controls, and logistic regression was used to adjust for bias due to conventional stroke risk factors. The allele and the genotype of rs10887800 in the renalase gene were both associated with severe intracranial cerebral atherosclerotic vascular stenosis (p = 0.013 and p = 0.049, respectively). No association was observed with severity for SNP rs2576178 or SNP rs2296545. Our findings show that the SNP rs10887800 in the renalase gene is closely associated with severe intracranial cerebral atherosclerotic vascular stenosis in ischemic stroke patients of north Chinese Han origin.
