During the past 15-20 y, the incidence of overweight and obesity in the United States has grown rapidly. The processes that underlie this alarming trend remain largely unspecified. We hypothesize that degradation of the ability to use certain orosensory cues to predict the caloric consequences of intake may contribute to overeating and excessive weight gain. The results of two preliminary studies with rats are consistent with this hypothesis. In one study, the ability of rat pups to regulate their caloric intake after consuming a novel high-calorie, sweet food was disrupted if they had received prior training with sweet tastes that failed to predict the caloric consequences of eating. Another study found that altering the normal predictive relationship between food viscosity and calories led to increased body weight in adult rats. Dietary factors that degrade the relationship between sweet tastes, food viscosity and calories may contribute to overeating and weight gain.

Dietary restraint is defined as the cognitive tendency to restrict intake and is often accompanied by the breakdown of restraint, referred to as disinhibition, leading to overeating and overweight in adults. Given recent evidence suggesting that dietary restraint and disinhibition are emerging as early as middle childhood, this study examined the validity of the restraint construct as measured in girls between the ages of 5 and 9. Dietary restraint was assessed longitudinally by questionnaire and validity was established by correlating restraint with measures previously reported to be related to restraint including weight concerns, body esteem, self-reported dieting, and measures of dietary intake when girls were ages 5, 7, and 9. Participants were 153 girls from predominately middle class, and exclusively non-Hispanic white families living in central Pennsylvania. Correlational data were used to assess relationships between dietary restraint and weight concerns, body esteem, dietary intake, and dieting. Results from this study indicate that there is evidence for the validity of the dietary restraint construct among girls by age 9. Specifically, dietary restraint was highly and positively associated with body mass index, weight concerns and body dissatisfaction and negatively correlated with dietary intake, findings similar to those reported in the adult literature.

Binge eating disorder (BED), characterized by ingestion of very large meals without purging afterwards, is found in a subset of obese individuals. We showed previously that stomach capacity is greater in obese than in lean subjects, and in this study, we investigated capacity in obese individuals with BED. We also determined ad-libitum intake of a test meal until extremely full. Furthermore, we measured various appetitive hormones (insulin, leptin, glucagon, CCK, ghrelin) and glucose before a fixed meal and for 120 min afterwards. An acetaminophen tracer was used to assess gastric emptying rate. We compared three groups of overweight women: 11 BED, 13 BE (subthreshold BED), and 13 non-binge-eating normals. The BED individuals had the largest stomach capacity as assessed by either maximum volume tolerated (P=.05) or by gastric compliance to pressure (P=.02) using an intragastric balloon. Although test meal intake did not differ between groups, it correlated (P=.03) with gastric capacity. The BED group showed a tendency (P=.06) to have greater area under the curve (AUC) and had higher values at 5 and 60 min (P<.05) for insulin compared to normals. Moreover, the BED subjects had lower ghrelin baselines premeal, and lower AUC for ghrelin, which then declined less postmeal than for the normals (P<.05). None of the other blood values differed, including glucose, leptin glucagon, and CCK, as well as acetaminophen, reflecting gastric emptying. The lower ghrelin in BED, although contrary to what was expected, is consistent with lower ghrelin in obesity, and suggests down-regulation of ghrelin by overeating. The lack of differences in CCK is consistent with the lack of differences in gastric emptying rate, given that CCK is released when nutrients reach the intestine. The results show that BED subjects have a large gastric capacity as well as abnormalities in meal-related ghrelin and insulin patterns that may be factors in binge eating.

Large prospective studies show a significant association with obesity for several cancers, and the International Agency for Research on Cancer has classified the evidence of a causal link as 'sufficient' for cancers of the colon, female breast (postmenopausal), endometrium, kidney (renal cell), and esophagus (adenocarcinoma). These data, and the rising worldwide trend in obesity, suggest that overeating may be the largest avoidable cause of cancer in nonsmokers. Few obese people are successful in long-term weight reduction, and thus there is little direct evidence regarding the impact of weight reduction on cancer risk. If the correlation between obesity and cancer mortality is entirely causal, we estimate that overweight and obesity now account for one in seven of cancer deaths in men and one in five in women in the US.

Self-report questionnaires of child eating behavior have demonstrated poor agreement with child interview methods and parent report. However, no study has investigated the relationship between child interview and parent report. Therefore, we compared results from a diagnostic interview, the Eating Disorder Examination adapted for Children (ChEDE) to those from a questionnaire, the Adolescent Version of the Questionnaire on Eating and Weight Patterns-parent version (QEWP-P), in a nontreatment sample of overweight and normal weight children. Both instruments were administered to 88 overweight (BMI >or= 85th percentile) and 79 normal weight (BMI<85th percentile) children, age 10.2 +/- 1.7 years, recruited from the community. The ChEDE and QEWP-P were not concordant in terms of the type of eating episodes that occurred in the past month. Using the ChEDE as the criterion method, the QEWP-P had reasonably high specificity, but low sensitivity for the presence of binge episodes (sensitivity 50%, specificity 83%) or objective overeating (sensitivity 30%, specificity 79%) during the past month. ChEDE subscales were, however, significantly related to items assessing eating-related distress on the QEWP-P. While parent report of child eating behaviors may provide some general information regarding eating psychopathology in young nontreatment-seeking children, they do not accurately reflect the results of a structured interview.

Parental feeding styles may promote overeating or overweight in children. A comprehensive literature review was undertaken to summarize the associations between parental feeding styles and child eating and weight status. Twenty-two studies were identified. We systematically coded study attributes and outcomes and tested for patterns of association. Nineteen studies (86%) reported at least one significant association between parental feeding style and child outcome, although study methodology and results varied considerably. Studies measuring parental feeding restriction, as opposed to general feeding control or another feeding domain, were more likely to report positive associations with child eating and weight status. Certain associations differed by gender and by outcome measurement (e.g., rate of eating as opposed to total energy intake). Parental feeding restriction, but no other feeding domain, was associated with increased child eating and weight status. Longitudinal studies are needed to test underlying causal pathways, including bidirectional causal models, and to substantiate findings in the presence of other obesity risk factors.

Focus groups were used to gain insight into the nutrition-related attitudes and motivations of overweight African-American adolescents and young adults enrolled in the Atlanta Job Corps Center in preparation for designing an effective and culturally appropriate nutrition intervention for this population. Eighteen students and 18 staff members participated in six different focus groups. Focus group themes suggest that an intervention with this target group should address overeating, proper diet composition, emotional eating, long-term consequences of a poor diet, group work, and incentives. Limiting food portions, offering healthy, appealing food choices, and displaying nutritional content of foods in the cafeteria were also suggested. It was also suggested that the intervention should be ongoing and led by someone who is patient, supportive, and has overcome an overweight problem. An intervention based on these recommendations can be specifically tailored to this target group and, potentially, be effective in changing dietary behaviors and lifestyle.

The majority of studies on eating attitudes, dieting and body size perceptions have focused on young adults and women. This study examined these attitudes in 334 black and white men, ages 55 to 98 years, who were members of the Charleston Heart Study cohort. Associations of the eating attitude variable with race, education and weight status were examined. Eighty-two percent of the overweight white men studied had dieted to lose weight, whereas only 49% of slimmer white men had dieted. In contrast, overweight black men did not diet more than slimmer black men. Overall black men dieted less than white men (37% reported dieting). Black men who were high school graduates were 1.3 times more likely to have dieted than were less educated black men. Overweight white men were over twice as likely as slimmer white men to feel guilty after overeating. This difference was not found in overweight versus slimmer black men. Education was not associated with measured body mass index (BMI) or perceived or ideal body size. However, there were some racial differences in these variables. White men preferred to be slightly thinner than black men (ideal BMI 25.6 vs. 26.1 kg/m2), and the difference between actual and desired BMI was 0.6 kg/m2 greater in white than in black men. These results indicate that effects of race and weight status on eating restraint and body size perceptions seen in younger subjects and in women are also present, at least to some degree, in elderly men.

A questionnaire that assessed a broad range of eating-related characteristics for unselected, normal subjects was factor analyzed in a two-step process proposed by Comrey (1984). Twelve "factored homogeneous item dimensions" were identified first and yielded three primary-level factors in a second factor analysis: Predisposition to Obesity (including Dieting and Preoccupation with, and Fear Of, Gaining Weight), Uncontrollable Urges to Eat (including Eating Momentum Beyond Control, Food a Panacea and Constant Temptation, and Secret Binging), and Predisposition to Anorexia (including Insufficient Eating Obvious to Others, Food Phobia, Inability to Eat, and Vomiting After Meals). The three primary-level factors were positively intercorrelated and exhibited significant positive, though weak, correlations with a measure of trait arousability. Also, weak results tentatively indicated that individuals with more pleasant and/or more arousable temperaments were less likely to be overweight. Subjects reported sharply higher levels of food consumption when feeling "depressed" (i.e., bored, lonely, sad) than when feeling "distressed" (i.e., uncomfortable, anxious, in pain). Uncontrollable Urges to Eat correlated positively and significantly with self-reports of food consumption while depressed, showing that those lacking control over eating ate especially more while feeling bored, lonely, or sad. Predisposition to Obesity correlated negatively and significantly with self-reports of food consumption while distressed, showing that those tending more toward obesity ate less while upset or anxious.

The authors compared eating patterns, disordered eating, features of eating disorders, and depressive symptoms in persons with binge eating disorder (BED; n = 177), with night eating syndrome (NES; n = 68), and in an overweight comparison group without BED or NES (comparison; n = 45). Participants completed semistructured interviews and several established measures. Depressive symptoms were greater in the BED and NES groups than in the comparison group. NES participants ate fewer meals during the day and more during the night than BED and comparison participants, whereas BED participants ate more during the day than the comparison participants. BED participants reported more objective bulimic and overeating episodes, shape/weight concerns, disinhibition, and hunger than NES and comparison participants, whereas NES participants reported more eating pathology than comparison participants. This evaluation provides strong evidence for the distinctiveness of the BED and NES constructs and highlights their clinical significance.
