Fungal wound infections have become more common because of the increased use of immunosuppressive and antineoplastic agents, prosthetic devices and grafts, broad-spectrum antibiotics, and
hyperalimentation
. Severe burns, renal failure, and other debilitating conditions also predispose to invasive
mycoses
. An aggressive diagnostic approach is particularly important, and tissue biopsy specimens are often necessary to establish a diagnosis. Meticulous surgical technique and minimization of the predisposing factors are crucial in the prevention of these infections, and prophylactic antimycotic agents may be of value in selected high-risk patients. Some mycotic wound infections can be managed effectively without systemic therapy; but when systemic agents are needed, combination antifungal therapy may provide improved results without increased drug toxicity.

Candida sepsis has become one of the most common and dangerous forms of hospital acquired infection. The recommended drug for parenteral treatment of Candida sepsis is amphotericin B, however, its toxic effects preclude its usage in many patients, particularly in the presence of renal failure. A less toxic antifungal agent is 5-fluorocytosine. A patient with Candida albicans sepsis was treated successfully with 5-fluorocytosine by intravenous administration. The
fungal infection
developed during the course of acute renal failure, repeated surgical intervention, intravenous
hyperalimentation
, gastrointestinal bleeding and five months of antibiotic therapy. The clinical symptoms receded rapidly and cultures became sterile after one week of intravenous treatment. The predisposing factors, difficulties in prevention and diagnosis of

With the increased number of immunocompromised patients there has been a concomitant increase in patient morbidity and mortality due to fungi. The etiologic microorganisms vary depending upon the type of immune dysfunction. Patients with malignancies and chemotherapy-induced neutropenia commonly are infected with Candida and Aspergillus. Other ubiquitous fungi such as Rhizopus, Fusarium, and Trichosporon are more frequently implicated as agents of disease in these patients. Patients with cell-mediated immune dysfunction such as acquired immune deficiency syndrome (AIDS) are susceptible to mucocutaneous candidiasis and pulmonary and disseminated cryptococcosis. Histoplasmosis and coccidioidomycosis have been particularly lethal infections in AIDS patients. Contributing factors such as broad-spectrum antibiotic use, intravenous catheterization, malnutrition,
hyperalimentation
, multiple surgical procedures and/or trauma, and steroids used either singly or in combination may also predispose patients to invasive
fungal disease
. Definitive diagnosis is often difficult to establish and usually requires invasive biopsy. Delay of culture results due to the time required to process specimens and to allow the fungus to grow also contributes to the poor results of therapy. Biopsy of skin lesions represents a useful technique for making a diagnosis. Recent advances in antifungal therapeutics promise to change the current approach to treatment for several of the
mycoses
. The availability of new oral azoles with spectra of activity that include aspergillosis and cryptococcosis, which currently require treatment with parenteral amphotericin B, may prove practical for prolonged oral therapy of otherwise lethal

Although abdominal candidiasis in critically ill surgical patients is becoming increasingly common, optimal management has not been defined. We treated 16 patients with abdominal candidiasis over a 36 month period. Violation of the gastrointestinal tract mucosa was the most common precipitating event (13 patients). Predisposing factors included: CVP catheters, broad spectrum antibiotics, and parenteral
hyperalimentation
in all patients, H2-blockers/antacids in 14 patients, as well as malnutrition (7 patients), DM (3 patients), alcoholism (3 patients), and steroids/chemotherapy (3 patients). Candida was isolated from an abscess in seven patients, peritoneal fluid in six patients and both in three patients. In four patients abdominal candidiasis was preceded by positive cultures from blood or two peripheral sites which had not been treated. All patients were treated with amphotericin B (146-4000 mg) without any major adverse effects.
Fungal infection
was eradicated in ten patients; three patients succumbed to candidiasis. Patients treated within seven days required less Amphotericin B and appeared to have a better outcome than those having delayed treatment. The authors conclude that abdominal candidiasis is a potentially lethal infection in critically ill surgical patients that should be aggressively treated. Amphotericin B can be safely administered and concurrent antibiotics need not be stopped.

Autopsy findings of 133 patients who died following fungemia or with invasive
fungal infection
were reviewed. Common clinical factors included antibiotic therapy, chemotherapy, corticosteroid administration,
hyperalimentation
, malignancy, and bone marrow transplantation.
Fungal infection
was seldom diagnosed antemortem and fungemia was detected in only 24 patients (18%). Ocular involvement occurred in 14 patients (Candida 11, Aspergillus 2, and Cryptococcus 1). The eye was the fifth most commonly involved organ at autopsy among patients with candida infection. Ocular involvement occurred with a significantly greater frequency in patients with Candida tropicalis than with Candida albicans infections (P less than 0.05). Although only about 10% of patients with fungal infections had ocular involvement, all those with ocular lesions had widely disseminated disease. Realizing the potential toxicity of antifungal therapy, we recommend that screening ophthalmologic examinations be performed on patients with fungemia or patients at high risk for development of
fungal infection
. The presence of ocular lesions consistent with
fungal disease
, in the appropriate setting, is a strong indication for investigation of possible systemic
fungal infection
and therapy once a definitive diagnosis is established.

Two premature neonates with birth weight less than 1,200 g developed systemic candidiasis during treatment with multiple antibiotics and parenteral
hyperalimentation
. Clinical findings included signs of necrotizing enterocolitis in one patient and multiple fungal renal cortical abscesses in the other. The Candida antigen, mannan, was present in the sera of both patients at the time of clinical deterioration. Multiple blood cultures and urine and stool samples from both patients grew Candida albicans. Systemic antifungal therapy was given for a 6-week period and was associated with prolonged antigenemia despite negative findings on follow-up cultures. Antifungal therapy was stopped soon after antigen was no longer detected. Both patients recovered without evidence of further

. Candida antigen detection may be useful in the diagnosis and follow-up of premature infants with disseminated candidiasis.

and broad spectrum antibiotic therapy via a central venous catheter. The patient was treated with 10 mg/kg fluconazole from week 16 of gestation for a total duration of 50 days. Adverse effects did not occur and the rest of the pregnancy proceeded favourably for both the mother and the baby.

This retrospective study was designed to investigate the related factors and outcome of systemic
fungal infection
in very low-birth-weight (VLBW) infants. Medical records of infants admitted to the neonatal intensive care unit of National Cheng Kung University Hospital between January 1990 and June 1994 were reviewed. Of the 262 VLBW infants, 15 (5.7%) had fungemia (14 Candida 1 Cryptococcus) during the study period. Among the fungemic infants, 60% also had urinary tract infection; 18% had central nervous system infection. Their mean birth weight was 1079 +/- 78 g (504-1474 g), and the gestational age was 28.6 +/- 0.6 weeks (23-32 weeks). Thirteen of them (87%) had respiratory distress syndrome and patent ductus arteriosus, while 60% had chronic lung disease. The percentage of antibiotic usage, parenteral
hyperalimentation
, endotracheal intubation, placement of central venous line and steroid therapy were 100%, 100%, 73%, 67% and 36% respectively. The mean age at diagnosis of fungemia was 40.5 +/- 4.8 days (10-76 days). Common clinical manifestations were respiratory deterioration (93%), poor feeding (58%) and fever (53%). The frequency of side effects of amphotericin B in decreasing order were: hypokalemia (54%), hyponatremia (31%) and decreased urine amount (23%). The mortality rate was 40%.
