Pneumothorax, hydrothorax, hydromediastinum, and
cardiac tamponade
are uncommon, although not unusual, complications of central venous catheter placement. We report a case of hydromediastinum with bilateral pleural and pericardial effusions, occurring in a patient after placement of a Silastic double-lumen central venous catheter for
hyperalimentation
.

Cardiac tamponade
from central venous catheterization occurs in less than 1% of cases. This report describes the first case of
cardiac tamponade
associated with total parenteral nutrition during pregnancy.
Cardiac tamponade
with subsequent nutrition during pregnancy.
Cardiac tamponade
with subsequent cardiac arrest occurred on the seventh day of central
hyperalimentation
for hyperemesis gravidarum. Maternal cardiopulmonary resuscitation was accomplished after pericardiocentesis, but maternal brain death and fetal death occurred. Diagnostic and therapeutic issues unique to
cardiac tamponade
during pregnancy are discussed.

Complications from central venous catheterization for administration of parenteral feeding solutions are common and well documented. In a recent series, complications from catheter placement were reported as 4.8%. We report the case of a patient with
cardiac tamponade
while receiving parenteral
hyperalimentation
through a polyurethane central venous catheter.

Several complications are associated with the use of central venous catheters for total parenteral nutrition;
cardiac tamponade
is one of the most severe. The recent use of flexible materials made of silastic polymeric silicone has great decreased the incidence of this complication, but has not eliminated wholly. We report the case of a patient with fatal
cardiac tamponade
, while receiving parenteral
hyperalimentation
trough a silicone central venous catheter placed in the right jugular internal vein, secondary to superior vena cava perforation. The physiopathological mechanisms, precautionary measures and treatment are discussed.

Pyopneumopericardium caused by transdiaphragmatic perforation of gastric ulcer is very rare. A 60-year-old man was admitted to our hospital because of chest pain with high fever. The chest computed tomography (CT) revealed hydropneumopericardium. The patient was diagnosed as purulent pericarditis. So emergent pericardiectomy and pericardial drainage were performed to relieve
cardiac tamponade
. Two findings enabled us to diagnose the causation of pyopneumopericardium as gastropericardial fistura. The first finding was that endoscopic examination of upper gastrointestinal tract revealed a deep peptic ulcer in the dome of gastric fundus. The second finding was that a dye solution which was injected into the pericardial cavity via the drainage tube leaked out into the gastric cavity through the ulcer. This patient improved successfully by the treatment of intravenous
hyperalimentation
including antibiotics and omeprazole. We think that expedient diagnosis and surgical drainage are essential for successful patient outcome.
