A clinical and laboratory examination of abnormalities not attributable to atherosclerosis has been performed on 188 male and 126 female adult subjects with hyperlipidaemia. The sample was recruited from 20000 subjects screened at a health control centre who had an initial serum cholesterol and/or triglyceride (TG) concentration above 350 mg/100 ml and 3.50 mmol/l. All were subjectively healthy and had no history of atherosclerotic disease. Known cases of secondary hyperlipidaemia were excluded. Lipoprotein (LP) analysis with preparative ultracentrifugation and electrophoresis was made on all subjects including control group with "nonelevated" serum lipids. Typing of hyperlipoproteinaemia (HLP) was performed according to the modified system of Fredrickson et al. Compared to controls, subjects who had elevated very low density ?LP (VLDL) (types II B, III, IV and V) were more obsese, while subjectI B and women with type IV HLP than in the control groups. Arcus corneae was seen in 29% of control groups. Arcus corneae was seen in 29% of controls and in higher frequencies in types II A and II B. A positive correlation existed between the frequency of arcus corneae and the mean low density LP cholesterol in the different types. Multiple tendon xanthomata (n equals 11) were found exclusively in type II A HLP, palmar xanthomata (n equals 3) only in the presence of floating beta-LP and eruptive xanthomata in one male with type V HLP. The mean ESR was increased in all types of HLP. The mean S-GPT and uric acid concentrations were higher in type IV HLP in both sexes than in the control groups. In men with type IV HLP S-GPT was positively correlated to tvldl tg. the uric acid level was correlated to both the VLDL TG concentration and body weight independently. Of the male subjects with HLP 1/3-1/2 had a diabetic or borderline i.v. glucose tolerance.

Among approximately 20,000 apparently healthy subjects whose serum cholesterol and triglyceride (TG) values were screened at a health examination, those with the most pronounced hyperlipidaemia have been selected for further studies. Thus, 188 males and 126 females, aged 30-65 years, with asymptomatic primary hyperlipoproteinaemia (HLP) and 59 male and 69 female controls with non-elevated serum lipids were studied with regard to frequency of ST segment depressions during exercise to near maximal heart rate. Furthermore, the concentrations of cholesterol and TG were determined in the lipoprotein (LP) classes very low (VLDL), low (LDL) and high (HDL) density LP, separated by preparative ultracentrifugation. From the LP analysis each subject's HLP was classified according to the typing system of HLP recommended by the WHO. The frequency of ST depressions (Minnesota code 4.1-4.3 as well as 4.1-4.4) increased with age, was higher in females than in males and was increased in all types of HLP in males. The percentage frequencies of ST depressions 4.1-4.4 in the various types of HLP were (male/female, p against controls): controls 16/36, type IIA 50 (p less than 0.01)/56, type IIB 64 (p less than 0.01)/75 (p less than 0.01)/75 (p less than 0.05), type III 67 (p less than 0.01)/33 and type IV HLP 40 (p less than 0.01/53. There was no significant difference in the frequencies of ST depressions in subjects with "high" and "low" BP (hypertensives were excluded from the study) or in subjects with "high" and "low" k-value for the i.v. glucose tolerance. Non-smokers had a tendency to higher frequencies of ST depressions than smokers. The association between different LPs and other "risk factors" and the occurrence of ST depressions in HLP were studied further with multiple regression analysis. Invariably age was the best predictor of ST depressions. The LP fraction giving the highest correlation coefficient was LDL cholesterol in both sexes. VLDL TG and LDL TG were also positively and significantly associated with ST depressions. HDL cholesterol was negatively but insignificantly correlated to ST depressions. When age and LDL cholesterol had been entered into the multiple regression, the only factor giving further significance was VLDL TG in males. Probability tables for the occurrence of ST depressions considering age and different levels of LDL cholesterol and VLDL TG were given. The importance of simultaneous consideration of both VLDL TG and LDL cholesterol in ST segment depression was evident from the tables. Of other "risk factors" (BP, glucose tollerance, smoking, ESR) entered into the regression together with only age and the LPs, only ESR contributed with borderline significance to ST depressions.

Since dietary calcium had been reported to reduce plasma lipids, the effects of calcium carbonate (CaCO3, 2 g/day) and the calcium salt of p-chlorphenozyisobutyrate (Ca-CPIB, 2 g/day), both singly and in combination, were studied in outpatients with primary hyperlipidaemia. Three groups of five patients were followed in a double-blind cross-over study, in which placebo and the drugs were given alternately during four-week periods. The main results were: 1) CaCO3 alone did not produce any significant changes in plasma lipids. 2) Ca-CPIB reduced LDL-cholesterol in patients with type IIa and IIb by an average of 29 and 21%, respectively. It also lowered VLDL-triglyceride by 50% in type IIb and by 48% in four out of five patients with type IV. 3) The combination of CaCO3 and Ca-CPIB reduced LDL-cholesterol by 31 and 25% in types IIa and IIb, respectively. It also lowered VLDL-triglyceride by 48-52% in types IIa and by 46% in four out of five patients with type IIb. 4) Three out of five patients with type IV had a rise of LDL-cholesterol while on Ca-CPIB alone; two of the patients had the rise while on the combination. 5) After treatment with Ca-CPIB, either singly or in combination, there was a statistically significant lowering of ESR and of plasma inorganic phosphate and alkaline phosphatase. No clinical side effects were noted.

Experimental and clinical researches were done for the purpose of that to treat the hyperlipemia with Dahuang Zhechong Wan (DHZCW). The patients of primary hyperlipemia were divided into two groups. One group was treated with DHZCW, the other with inositol nicotinate as control. DHZCW could reduce the serum total cholesterol (0.05 less than P less than 0.2), triglyceride (P less than 0.05), the blood viscosity (P less than 0.01), reductive viscosity (P less than 0.001) and fibrinogen (0.05 less than P less than 0.2). No significant change of the other item of hemorheology, HDL-c and the subfraction were observed. No significant differential effect between those two groups. The experimental hyperlipidemic model was made by feeding cholesterol and axungia on rabbits. The results showed that DHZCW could reduce the serum total cholesterol (P less than 0.05), triglyceride (P less than 0.05), beta-LP (P less than 0.01), the blood viscosity (P less than 0.05) and plasma viscosity (P less than 0.05), but no significant effect on HDL-c and its subfraction, ESR, index of erythrocytic deformation and hematocrit. DHZCW could change the viscosity of blood of hyperlipemia and reduce the total cholesterol, triglyceride and fibrinogen of the blood. It might have some beneficial effects on atherosclerosis and prevent ischemic cardiovascular and cerebrovascular diseases.

All contributory factors to the unusual occurrence of stroke in young people were evaluated in patients under age 40 admitted to the Stroke Unit of the Austin Hospital in Melbourne, Australia. Over the August 1977 to December 1980 period there were 700 admissions. Of these 14 patients were under the age of 40. There were 7 males and 7 females whose ages ranged from 17-38 years. Each patient was screened for factors which might contribute to premature vascular disease including hypertension, diabetes, smoking, obesity, and hyperlipidemia. In addition, the following tests were performed to exclude an arteritic process: full blood examination; ESR; protein electrophoresis; syphilis serology; and the presence of antinuclear factor. Each of the 14 patients suffered cerebral infarction. A summary of each case is presented in a table. In 9 patients, infarction occurred in the carotid territory of supply. Large cortical infarcts with or without subcortical involvement occurred in cases 1-8, of whom 5 had major vessel occlusion demonstrated angiographically and another had stenosing and ulcerative atheromatous disease at the extracranial carotid bifurcation. In a further 4 patients, infarction occurred within the vertebrobasilar territory and was either confined to the brain stem, the occiptal cortex, or involved both. Angiograms were performed in 2 of these patients and showed irregular narrowing of the vertebral artery which was interpreted as spasm and segmentally narrowing of the basilar artery. The final patient had several ischemic events which included right sided amaurosis fugax, and left frontal, right parieto-occipital and left occipital infarctions. Angiography was normal. All patients survived the stroke and were able to go home. There may be an interrelationship between the pathological findings of Irey et al. (1978) and the effect oral contraceptives (OCs) has on migraine. This is relevant to Case 13. Sustained exposure to OCs may produce the pathological changes described (visible as segmental narrowing angiographically). In 2 patients cerebral infarction was caused by atheromatous or hypertensive occlusive vascular disease. In Case 3 an embolus occluded the middle cerebral artery. Infarction complicating migraine was diagnosed confidently in 4 patients on the basis of typical migrainous symptomatology in the past and accompanying the stroke. Of the 12 patients fully evaluated, there were no cases of polycythemia or thrombocytosis. There were no abnormalities of the clotting factors. Almost every patient had some form of emotional upset, and there were 7 who had significant psychiatric illness and emotional problems of extreme magnitide.

Out of 26 patients with acute pancreatitis, 8 had several signs of bacterial infection such as high nitroblue tetrazolium (NBT) reduction of granulocytes, fever, elevated ESR and leukocytosis with granulocytosis. 2 patients had a high NBT-value without all other clinical signs of infection and 6 had such signs without a high NBT-value. --An NBT-value lower than normal was found in 6 patients, 3 of whom also had other signs of infection. The level of serum lipids, determined in only 3 of the 6, demonstrated concomitant hypertriglyceridemia. Hyperlipidemia is known to decrease granulocyte activity and might have prevented a stimulation to increased NBT-reduction otherwise brought about by bacterial infection. Further, 3 of the 6 patients with low NBT-reductions suffered from a very severe type of pancreatitis and two of them developed pneumonia. --Bacterial infection may thus contribute to a severe clinical course of pancreatitis, especially in patients with hypertriglyceridemia in whom the granulocyte function is depressed.
