Fifteen of 42 formalin-fixed lymph-node biopsies with
Hodgkin
lesions of different histological types and stages gave positive staining for immuno-globulins by the peroxidase/antiperoxidase method. The immunoglobulins in all the positively stained
Hodgkin
cells were of IgG type, but staining for
IgA
, D and E was negative. One of the 15 biopsies was positive for IgM. Thirteen of the 15 positive biopsies were positive for both kappa and lambda. Sequential double staining using two different substrates for peroxidase showed that most of the
Hodgkin
cells simultaneously contained light chains of both kappa and lambda type. It is concluded that
Hodgkin
cells do not contain a "monoclonal" immunoglobulin product.

Malignant plasma cell lymphomas have rarely been studied in the literature. We report two personal cases, the first survived 9 years, in the second case, histo-immunofluorescence revealed the presence of
IgA
kappa immunoglobulin. 59 cases have until now been reported to the best of our knowledge. They were reported under the term plasmocytoma. Analysis of data in the literature shows that plamocytomas resemble clinically and in prognosis, non-
Hodgkin
malignant lymphomas, of which Lukes and Lennert have recently given a new classification. Histo-immunofluorescence will permit in the future better understanding of the immunoglobulins secreted. Treatment is not yet standard. Three methods are used, depending on the spread of the tumour (surgery, radiotherapy and chemotherapy).

Under examination were the levels of serum immunoglobulins G, A, M, and D in 25 patients with
lymphogranulomatosis
and in 22 patients with nonlymphogranulomatous malignant lymphomas. A reliable increase in IgG concentration was found in both groups, but it was more pronounced in cases with malignant lymphomas. The
IgA
and IgD level also tends to rise but somewhat more sharply in malignant lymphomas. The level of IgM in
lymphogranulomatosis
patients was not significantly changed, while in nonlymphogranulomatous malignant lymphomas it was raised a little.

The light chain of serum IgE from 4 untreated patients with
Hodgkin's disease
with elevated IgE levels was studied by an immunoadsorbent technique. Serum IgE was found to contain both kappa and lambda light chains in all cases studied. In addition an association between serum levels of IgE and that of
IgA
, IgG, and IgM was demonstrated. These findings make it unlikely that increased serum IgE in
Hodgkin's disease
is of monoclonal origin and support the view that serum IgE in such patients reflects a general disturbance in the regulation of their humoral immune response.

The following sera were tested for EB virus-specific
IgA
: serial sera from 61 cases of infectious mononucleosis (IM) and from 195 EBV IgG positive healthy students; single sera from each of 1469 persons of different ages, 63 cases of untreated
Hodgkin's disease
, and 22 neonates. EBV specific
IgA
was found in the sera from 88% of cases of IM, from 18.5% of EBV IgG positive healthy students, and in 13.5% of repeat samples from the same students three years later. The incidence of EBV
IgA
varied from 5 to 30% at different ages in single sera from EBV IgG positive persons aged 2 to 70 years. The higher percentages occurred in the age groups where recent sero-conversion rates were high. Fifteen percent of sera from cases of
Hodgkin's disease
were positive for EBV
IgA
, an incidence similar to that for healthy adults in the age group 25-45 years. None of the EBV IgG positive sera from neonates gave a positive reaction for EBV IGA.

To define the contribution of aggressive lymphoma treatment to the risk of post-splenectomy septicemia, we investigated the humoral immunity of 44 patients with
Hodgkin's disease
. Specific antibody against Haemophilus influenzae Type b was significantly reduced (mean, 147 ng per milliliter, P less than 0.01) in patients receiving combined treatment (radiotherapy and chemotherapy), whereas single treatment reduced titers marginally (chemotherapy) or not at all (radiotherapy). Untreated patients had normal values (396 ng per milliliter), and splenectomy was without effect. In some patients who received combined treatment, titers were reduced to levels seen in infants. IgM levels were likewise normal in untreated patients. Chemotherapy, however, significantly reduced IgM levels (P less than 0.025), an effect potentiated by prior splenectomy. IgG,
IgA
, alternate-pathway activity, C3, C4 and CH50 were all normal or elevated. Aggressive treatment with chemotherapy and radiation impairs humoral defense against encapsulated micro-organisms, and thus magnifies the risk of post-splenectomy septicemia in patients with
Hodgkin's disease
.

Seventy-one previously untreated patients with non-
Hodgkin
lymphomas were studied with several readilyvailable tests of immune function: number of peripheral blood lymphocytes, serum immunoglobulins, and delayed hypersensitivity to six recall antigens. The results were correlated to histology (Rappaport classification), stage (Ann Arbor classification), the presence of symptoms, and survival. As a group, 38 patients with diffuse lymphomas exhibited marked impairment in reactivity to five of six antigens (p less than 0.03 to p less than 0.001). In addition, lymphopenia and reduced levels of serum
IgA
were found in association with diffuse histiocytic lymphoma. Among patients with diffuse lymphoma, lymphocyte number and skin test reactivity tended to be greater in those with localized disease or without constitutional symptoms, and survival was superior for patients free of symptoms (p less than 0.01). As a group, 33 patients with nodular lymphoma had normal numbers of lymphocytes, lower levels of serum IgG and
IgA
, and significant impairment of reactivity to two antigens (streptokinase-streptodornase and mumps; p less than 0.01); reactivity to three other antigens (Candida albicans, coccidiodin, and tuberculin) was normal. Survival for patients with nodular lymphoma was superior (p less than 0.01) compared to those with diffuse lymphomas. In summary, severe immunodeficiency was found in patients with diffuse lymphoma (particularly diffuse histiocytic lymphoma), and definite but much less severe immunodeficiency was characteristic of patients with nodular lymphoma.

Immunological parameters have been evaluated both in vitro and in vivo in 20
Hodgkin's disease
patients in continuous not maintained complete remission for at least 3 years. IgG,
IgA
and IgM levels were within the normal range. Delayed sensitivity, assayed by a battery of recall antigens, was normal too, in the majority of patients. The number of circulating lymphocytes was slightly inferior to normal controls. T cells, evaluated by the rosette E formation, seems to be diminished; on the contrary, B cells, evaluated by the membrane immunofluorescence, were increased in respect to normal controls. The differences are statistically significant. Timidine uptake of PHA stimulated lymphocytes was diminished too. It is concluded that even if these patients' clinical behaviour is like that of normal population, it is possible to find a disorder in vitro parameters, which could be attributable to long lasting effect of the therapy and/or to the persistence of serum factors acting as anti-T cells antibodies.

The cellular immune response was explored by us with the help of tuberculin and DNCB reactions in 73 cases of
Hodgkin's disease
. Serum immunoglobulins were simultaneously determined. Tuberculin and DNCB reactions had significantly been decreased before treatment. A significant augmentation of the
IgA
is remarkable in tuberculin and DNCB positive groups. The positivity of the cutaneous tests is only temporarily lowered by radiation therapy, the difference being not significant, while IgM values in patients who had undergone radiation therapy are remaining at a lower level. Patients in remission revealed a positivity in tests that was approximating the values before treatment. Lower values were observed in all the three groups of immunoglobulins; the decrease of the IgM-values was significant. In case of a relapse, a significant decrease of positivity developed, accompanied by significantly increased values of
IgA
in tuberculin and DNCB negative groups. Only insignificantly positive values of the cutaneous tests were seen in the course of cytostatic treatment, and the values of the IgM were significantly decreased.

A young woman developed
Hodgkin's disease
(nodular sclerosis) in pregnancy and gave birth to a boy who developed common variable immunodeficiency. Initially there was normal IgG with low
IgA
and IgM, and antibody deficiency. IgG levels fell progressively over 4 years. Cellular immunity was normal. We suggest that this is a further family with immune deficiency presenting with common variable immunodeficiency and lymphoid malignancy.
