Limitations of therapeutic options for chronic lymphocytic leukemia (CLL) and non-Hodgkin lymphoma (NHL) have necessitated the development of novel treatments/strategies. Rituximab (chimeric anti-CD20 monoclonal antibody [mAb]) considerably improved therapeutic outcomes for patients with B-cell malignancies, particularly when combined with chemotherapy; outcomes, however, are limited by rituximab resistance or reduced response upon re-treatment. Novel anti-CD20 mAbs are in development that may enhance mAb therapy. Ofatumumab (human anti-CD20 mAb) induces highly potent cell lysis, including in cells with low CD20 expression, and is the most clinically advanced new anti-CD20 mAb. Positive phase III interim data for ofatumumab in fludarabine-refractory CLL that is also refractory to alemtuzumab or less suitable for alemtuzumab due to bulky (>5 cm) lymphadenopathy has led to FDA approval of this agent in this population. Preclinical and early clinical assessment of other novel anti-CD20 mAbs include: ocrelizumab, veltuzumab, GA101, AME-133v, and PRO131921; data suggest potential for improved efficacy over rituximab that will require substantiation in large-scale clinical trials. New treatment strategies and novel anti-CD20 mAbs have the potential to enhance long-term outcomes for CLL and NHL.

Primary hepatic lymphoma (PHL) is a lymphoma presenting with predominant liver involvement at presentation in the early stage of lymphoma. It accounts to less than 0.4% of all extranodal lymphomas and usually occurs in immunocompromised states. It is more commonly of the non-Hodgkin's lymphoma of B cell variety. Hodgkin's lymphoma presenting as PHL has not been reported before. We report a 55-year-old HIV positive male presenting with an SOL in left lateral segments of liver having typical imaging characteristics of fibrolamellar HCC with normal serum AFP levels. Surgical resection was done and the histopathological analysis and immunohistochemistry of the specimen revealed it to be a Hodgkin's lymphoma. The patient was started on chemotherapy (ABVD regimen) along with highly active antiretroviral therapy (HAART). Primary lymphoma of liver can mimic HCC on imaging. Hence, in the background of an immunocompromised state like HIV with normal serum AFP levels, PHL should be suspected and liver biopsy can clinch the diagnosis.

Adalimumab is a human monoclonal antibody which targets tumor necrosis factor (TNF)-alpha. It is produced by recombinant DNA technology, using a mammalian cell expression system and is widely-known to treat a number of immune-mediated conditions, including psoriasis. There has been a growing concern regarding the possible association between TNF-alpha inhibitors and malignancy. In this case report, we describe the case of a 20-year-old woman, known to have been suffering from chronic plaque psoriasis for 12 years, and who developed Hodgkin's lymphoma within five weeks of beginning adalimumab treatment.

Initial extranodal osseous presentation of Classic Hodgkin lymphoma (CHL) is extremely rare. The present study describes a case of CHL in an 11-year-old male presenting with lower back and right hip pain. Radiologic imaging showed a solitary lytic lesion involving the L5 vertebrae and sacrum. Pathologic examination led to a diagnosis of extranodal osseous CHL and further workup proved bone marrow and nodal disease. Diagnosis of extranodal osseous CHL is challenging, especially in this age group and location. When confronted with a solitary lytic bone lesion in a young patient, more common benign and neoplastic entities enter the differential diagnosis and include osteomyelitis, Langerhans cell histiocytosis, epithelioid hemangioma of bone, leukemia, and anaplastic large cell lymphoma (ALCL) among others, with extranodal osseous CHL being one of the less common entities in the differential diagnosis. This case report raises the awareness of extranodal osseous CHL as one of the entities to consider when confronted with a solitary lytic bone lesion in a young patient.
