A 30 year old man with Hodgkin's disease, clinically in remission, presented with blurred vision in one eye due to a choroiditis. He developed headaches 10 days after commencing oral steroids and was subsequently found to have cryptococcal meningitis. The meningitis and choroiditis resolved on antifungal medication. This is the first case of cryptococcal choroiditis recorded in the United Kingdom.

Primary non-Hodgkin lymphoma (NHL) of the nasal cavity is a rare neoplasm. They account for about 0.17-2% of all cases of NHL and 5.8% of all malignant neoplasms of the sinonasal region in adults. A 37-year-old male patient presented with left-sided nasal obstruction and headache which appeared a couple of weeks before the admission. Nasal examination revealed a massive polyposis. After medical therapy with corticosteroids, neither the polyposis nor his complaints showed regression; thus, he underwent a nasal polypectomy operation. Biopsy results indicated an inflammatory nasal polyp. Two weeks later, a rapidly progressive facial swelling, especially in the left periorbital area and blurred vision occurred. Second biopsy, which was also taken from the left nasal cavity suggested again an inflammatory nasal polyp. To rule out any possible malignancy, a third biopsy was conducted in the left maxillary sinus which indicated T/NK cell lymphoma of the sinonasal tract.

Primary cardiac lymphoma (PCL) is defined as a non-Hodgkin lymphoma involving only the heart or pericardium. PCL is extremely rare and often misdiagnosed. We report the case of a healthy 53-year-old male who originally presented with blurred vision secondary to a right intraocular mass. Enucleation of the eye confirmed a ciliary body melanoma and, upon further investigation, the patient was discovered to have a mass in the right atrium suspicious for a myxoma. However, resection of the atrial mass revealed a low-grade B-cell PCL. The occurrence of PCL in an immunocompetent patient being investigated for a visceral malignancy makes this a highly unusual presentation of a rare neoplasm.

Perifollicular granuloma is a unique histologic feature and whether it is associated with immunoglobulin G4 (IgG4)-related disease is controversial. We report a case of a 38-year-old man who presented with worsening left eye pain, proptosis, tearing, gritty sensation, blurred vision and multiple lymphadenopathy. An axillary lymph node resection showed reactive follicular and interfollicular lymph node hyperplasia, and increased eosinophils and plasma cells (at least 80% of IgG(+) plasma cells were positive for IgG4). A distinct feature was the presence of multifocal, perifollicular histiocytic granulomas, which formed a wreath around the entire follicles. The human herpes virus 8 was not detected by immunohistochemistry. In addition, an extensive panel of special stains, immunohistochemistry, and flow cytometry was negative for lymphoma, fungal, or mycobacterial infection. The findings were suggestive of IgG4-related sclerosing disease-associated lymphadenopathy. Further laboratory testing showed a significant increase of serum immunoglobulin E (> 23000 IU/mL) and slight increase of total IgG, but normal serum IgG4. Even though perifollicular granuloma is a nonspecific histopathologic feature and can be seen in other diseases, such as nodular lymphocyte predominant Hodgkin lymphoma, IgG4-related lymphadenopathy should be listed in the differential diagnoses of benign reactive lymph nodes, especially when perifollicular granuloma and plasmacytosis coexist.

Bilateral non-granulomatous anterior uveitis with left vitritis and macular edema were detected in a 19-year-old woman presenting with blurred vision in her left eye. Light microscopic study of the pathologic mediastinal lymph node that was detected via contrast computed tomography imaging during etiologic study revealed nodular sclerosing and mixed cellularity Hodgkin's lymphoma (HL). Ocular findings completely resolved with adriablastin, bleomycin, vinblastine, dacarbazine chemotherapy treatment. Herein, it is emphasized that HL should be remembered as one of the differential diagnoses in patients with ocular inflammatory pathologies such as uveitis and vasculitis. The ocular findings of HL are discussed.
