An analysis was made of the results of irradiation therapy applied during the period 1965-1976 to 131 patients with
Hodgkin's disease
. The results of radiotherapy only of clinically positive
lymphogranuloma
localizations were compared with those of irradiation of both positive and negative nodes. The evaluation criteria used were the numbers of 3-year long primary remissions, 3-year long survival rates, frequency of onset and localization of relapses. Better therapeutic effects were achieved by radiotherapy as regards duration of primary remission, survival rate and frequency of relapses in originally clinically negative nodes proximate to positive nodes. However, the number of trans-diaphragmatic and extralymphatic relapses did not decline. A 5-year survival rate regardless of the irradiation method and clinical stage in 131 patients (1965-1974) was attained by 70.2%. The discussion bears on problems of an optimum radiotherapy and combined radiochemotherapy, and the significance of diagnostic laparotomy in
lymphogranuloma
.

The authors from 11 institutions in the Czech Socialist Republic, grouped into a Complex Rationalisation Brigade for the diagnosis and therapy of the malignant
lymphogranuloma
, present results of a retrospective study in 430 patients with
Hodgkin's disease
. The results were obtained after the data treatment on a digital computer and concern with a general characteristic of the group of patients, that involved 57,2% men and 42,8% women with a peak of the age distribution between 20 and 30 years and a second peak between 60 and 70 years. In the clinical data there is an analysis of the rate of occurring the lesion in particular groups of nodes, where the nodes of mediastinum and of the neck were involved most frequently. In the therapeutic procedures the radiotherapy, cytostatic therapy and their combination are evaluated. The 5-year survival in the whole group is of 41%. Out of this, in the first five years of the period of interest and in the second five years, the numbers of the surviving patients were of 33 and 47% respectively. In the Tables, there are complete remissions, mean times of the remission and longest time of the remission for different methods of the therapy. When evaluating the relapse of the disease, it was possible to observe not only the dependence on the total time of irradiation but also on the time distribution of the dose. The patients with involved nodes above the diaphragm have a better prognosis than those with lesions of nodes in the subdiaphragm region.

Three cases of endobronchial
lymphogranuloma
are reported. The clinical and histological aspects of lung localization of
Hodgkin's disease
are outlined. Special attention is paid to a case of concomitant broncho-oesophageal involvement of the lymphogranulomatous process and therapeutic problems are reviewed.

In a group of 10 patients (six with acute lymphatic leukaemia, one patient with Ewing's sarcoma, one patient with lung cancer, one patient with a myeloma and one patient with
Hodgkin
's
lymphogranuloma
) treatment with leucocytic growth factors (Neupogen Hoffman La Roche) was started only after the onset of leukopenia. Even after this mode of administration of leucocytic growth factors septic conditions in leukopenic patients were more easily controlled and it was thus possible to complete chemotherapy with planned doses in the planned time interval. In five patients with acute lymphatic leukaemia the leucocytic growth factors participated in the achievement of remission, in patients with myeloma and
lymphogranuloma
they contributed to marked improvement of the general condition and reduction of the time spent in hospital.

Cytological, immunohistological and electron microscopical observation of 21 percutaneous fine needle punctures of retroperitoneal, pelvic and abdominal lymph nodes after borderline lymphography and computer tomography and 6 punctures of tumours after tomography allowed classification of primary metastases from the small pelvis in 14 patients and characterized tumours in 4 patients, which could not be demarcated by sonography. We distinguished yolk sarcoma metastasis, prostate gland cancer metastasis, three cases of nodular metastases of seminoma cells, and two metastases of melanoma. Malignant cells of
Hodgkin
's
lymphogranuloma
and non-Hodgkin's lymphoma were distinguished in seven samples of fine needle puncture. We found malignant cells of adenocarcinoma, T-immunoblastoma, pancreas carcinoma and histiocytosis X in four punctures of tumours. Fine needle puncture processed for electron microscopy with buffered fixation and harvested into Lowicryl K4M resin through centrifugation makes it possible to detect even the minimum of cells present, preserves the structure of cells and enables to correlate cytological findings in semithick sections with correspond ultrastructure in followed series of semithin sections.
