Among the 130 primary or secondary malignant fibrous histiocytomas of bone diagnosed and treated at Memorial Hospital for Cancer and Allied Diseases during the previous half century, 20 (15.4%) arose as a direct consequence of irradiation. This type is the commonest secondary osseous malignant fibrous histiocytoma at this institution. It affects the ilium, the scapula, and the distal end of the femur most frequently, predominantly in patients whose age peaks in the fifth decade of life, when their sarcomas developed. Grounds for the irradiation were either nonosseous conditions (13 patients) or preexistent skeletal lesions (seven patients). Reasons for the incidental bone irradiation included

; carcinoma of cervix, breast, or lung; bilateral retinoblastoma, and others; giant cell tumors predominated among the irradiated skeletal lesions. The mean and the median radiation doses were 6,040 and 5,700 rads, respectively. The latent period between irradiation and the appearance of the bone
sarcoma
ranged from 4 to 47 years with a mean of 16.5 and a median of 14.5 years, respectively. The cumulative disease-free survival rate at 3 years was 58%. Although all patients who received their radiation therapy for a preexistent bone lesion survived, only 27% of patients whose bone was normal at the time of irradiation are alive and well at the 3-year mark.

A new radioreceptor assay for insulin-like growth factor-II (IGF-II), using receptors on ovine placental membranes, is described. Half-maximal displacement of specifically bound radioiodinated human IGF-II tracer was seen at 1.0 ng/tube of unlabelled IGF-II. The cross-reactivity of IGF-I was 1%, and insulin was entirely without effect. Measured on serum samples from 100 healthy adults, the mean IGF-II concentration (+/- SD) was 576 +/- 160 ng/ml. Identical mean values were seen for all adult age groups up to 65 years. The mean value for 10 acromegalic adults was 583 +/- 155 ng/ml, and for 9 GH-deficient subjects, 161 +/- 26 ng/ml (P less than 0.001 compared to normals). Of eight patients with chronic renal failure, none had an IGF-II level less than 2SD above the normal mean. No significant effect of renal dialysis was seen. A review of the Memorial Sloan-Kettering Cancer Center experience with second malignancies (SM) after childhood
Hodgkin's Disease
(HD) identified 17 SM in 320 patients who survived more than 1 year from, and were 15 years old or younger at the time of, HD diagnosis (1949 to 1983). Of 254 previously untreated patients, 12 SM were noticed as compared with 0.606 expected on the basis of rates in the general pediatric population (relative risk, 19.8; 95% confidence interval, 10.2 to 34.6). For patients who received multi-agent chemotherapy, the cumulative probability of developing acute nonlymphocytic leukemia (ANLL) or bone
sarcoma
was 6.2% and 5.5%, respectively, at 10 years from the initiation of therapy; the cumulative risk of all SM in this group reached 18.7% at 15 years. For patients who received radiation alone or with single-agent chemotherapy, the cumulative risk of SM rose from 0% at 10 years and 2% at 15 years, to 10.7% at 25 years from the initiation of treatment. The risk of ANLL after childhood HD was highest in the first 5 to 10 years after combined modality treatment, and aggressive forms of NHL were associated with excessive immunosuppression. Bone sarcomas predominated in solid SM in the first decade after HD treatment, whereas "adult-type" cancers, for example, breast and colon carcinomas, were more delayed. Our findings, supported by a literature review, point to a therapy-related enhanced risk of approximately age-appropriate solid SM. This possibility mandates careful surveillance of long-term survivors of childhood HD.

This article reports a patient with
Hodgkin's disease
in remission after combined modality therapy who developed metastatic pulmonary osteosarcoma, subsequently found to originate in soft tissue of the mediastinum, within a field irradiated 11 years previously. This patient developed a series of radiotherapy-induced complications in addition to osteosarcoma. Only 16 cases of extraskeletal osteosarcoma after radiation treatment have been reported, none originating in the mediastinum. To the authors' knowledge, this is the first reported case of extraskeletal osteosarcoma occurring in a patient previously treated for

and with the

originating within the irradiated field.

Hodgkin
Lymphomas (ML). 7 cases of SMT were leiomyosarcoma. The majority of the cases were found in the stomach. 8 cases presented as acute emergencies while the non acute cases had non specific complaints. Despite the use of barium studies, gastroscopy and CT scan, only 5 cases were diagnosed preoperatively. The majority underwent curative resection. ML cases had a combination of radiotherapy and chemotherapy. 2 patients with large and high grade leiomyosarcomas have died. 4 patients with ML have also died, 2 with locally advanced disease and 2 with distant metastases. We conclude that most cases are not diagnosed preoperatively due to their rarity and nonspecific clinical presentation. The important prognostic factors are size and mitotic index for SMT and stage of the disease for ML.

Fifty-seven patients with advanced malignant tumours were treated with ifosfamide (Holoxan) and mesna (Uromitexan) in our department from November 1979 to December 1984. This series comprised eight cases of
soft tissue sarcoma
, nine cases of ovarian carcinoma, five cases of non-seminomatous testicular tumour, 11 cases of bronchogenic carcinoma, three cases of renal carcinoma, seven cases of non-Hodgkin's lymphoma, two cases of skeletal fibrosarcoma, two cases of breast carcinoma, one case each of Ewing's tumour, prostatic carcinoma, seminoma, plasma cell tumour, multiple myeloma, malignant teratoma, nasopharyngeal carcinoma, Wilms's tumour, neuroblastoma and mycosis fungoides. Out of these 57 cases, 53 were evaluable. There were five complete remissions and 20 partial remissions, corresponding to a total response rate of 47%. The overall median survival time (MST) of the 53 evaluable patients was 7.5 months. The responders had a longer survival time (MST 10 months) than the non-responders (MST 4.75 months) (p greater than 0.05). Analysis of the results according to sex, age, dosage of ifosfamide and degree of histological differentiation of the tumour cells failed to show any influence of these factors on the therapeutic results. The response rate to ifosfamide found in this study might be related to the histological origin of the tumours and to whether the primary tumours had been resected. Hodgkin
's lymphomas and ovarian carcinomas showed a high response rate. The response rate was higher in the group in which the primary tumour had been resected (61%) than in the non-resected group (12%) (except the non-Hodgkin's lymphoma). The side-effects of this regimen were moderate. Dyspepsia, nausea, vomiting, myelodepression, dizziness, and alopecia were common. Cystitis could be prevented nearly completely by concomitant administration of mesna, when given correctly, for preventing side-effects of ifosfamide on the urinary system (haemorrhagic cystitis, etc.).

soft tissue sarcoma
(
STS
), non-Hodgkin's lymphoma (NHL), and acute myeloid leukemia (AML) in abattoir workers. The first published study involved 82 cases of
STS
(ICD 171) and found a relative risk of 2.8 (90% confidence interval 1.3-6.3). Interviews with an additional 51 cases reported here revealed a relative risk of 1.6 (90% confidence interval 0.9-3.0). Two further studies involved interviews with 100 cases of the category of NHL involving lymphosarcoma and reticulosarcoma (ICD 200) and 83 cases of other NHL (ICD 202). Relative risk estimates were 1.8 (90% confidence interval 1.1-2.9) and 1.7 (90% confidence interval 1.0-2.8), respectively. A study of 150 cases of AML (ICD 205.0) found a relative risk of 2.5 for abattoir workers (90% confidence interval 1.3-4.7). Finally, a United States cohort study found a standardized mortality ratio of 2.4 (90% confidence interval 0.8-5.4) for
Hodgkin's disease
(ICD 201) and 2.2 (90% confidence interval 0.8-4.5) for cancer of other lymphatic tissue (ICD 202, 203, 208) among abattoir workers. Abattoir workers are potentially exposed to oncogenic viruses, including bovine leukemia virus. Some workers may also be exposed to the animal carcinogen 2,4,6-trichlorophenol when treating pelts.

A population-based case-referent study in Kansas examined the relationship between exposure to insecticides and the development of soft-tissue
sarcoma
(STS) and
Hodgkin's disease
(HD). Data from telephone interviews for 133 STS cases, 121 HD cases, and 948 referents indicated that STS was associated with use of insecticides on animals, but not on crops. HD was not significantly associated with either use. STS risk was higher among the farmers who themselves mixed or applied insecticides to animals than among farmers who did not. Farmers who failed to use any protective equipment to reduce insecticide exposure were at a significantly elevated risk of STS. Risk rose with early calendar year of first use. The excess risk appeared to be primarily among fibrous and myomatous sarcomas with little association seen for lipomatous or other STS neoplasms. Myomatous sarcomas increased significantly with duration and time since first use of insecticides on animals. If the reported association between STS and insecticides is causal, the data suggest that exposure to the agent(s) responsible may have been reduced in the mid-1950s or the agent(s) have an average latency period for STS of at least 20 years.
