Five cases of sarcoidosis associated with malignant lymphoma are reported. In one patient, sarcoidosis was diagnosed several years after the onset of Hodgkin's disease. Of the remaining 4 patients, three developed nodular mixed lymphoma and one immunoblastic lymphoma (complicating angioimmunoblastic lymphadenopathy with dysproteinaemia) 3 months to 12 years after sarcoidosis were diagnosed. Such cases are extremely rare. They may be due to coincidence, but Danish epidemiological data suggest a relationship between the two diseases. Overstepping of humoral immunity in sarcoidosis may induce the emergence of a clone of formerly quiescent neoplastic B-cells.

Thirteen patients with lymphoma of the breast are presented. In addition, 163 previously reported cases of lymphoma of the breast are reviewed. Complete staging was performed on all patients. No patient had a diagnosis of lymphoma prior to breast biopsy. The histologic findings were diffuse histiocytic lymphoma (DHL) in eight patients, nodular lymphocytic poorly differentiated lymphoma (NLPD) in two patients, nodular mixed lymphoma (NM) in two patients and nodular sclerosing Hodgkin's disease (NSHD) in one patient. Five patients had Stage IV disease, two had Stage III disease, four had stage II disease and two Stage I disease. Nine patients each underwent an excisional biopsy and four patients each had a modified radical mastectomy as initial therapy. Two patients each underwent a staging laparotomy. In advanced disease, chemotherapy achieved complete remissions in approximately 50% of patients. Unfavorable histologic findings are most common in lymphoma of the breast and thorough staging is necessary to select the best form of therapy. The absolute survival rate (61%) and the disease free survival rate (46%) are similar to nodal lymphoma of corresponding histologic factors and stage.

There were 51 cases of non-Hodgkin's malignant lymphoma (NHL) and 9 cases of reactive follicular hyperplasia among 60 cases of lymphoproliferative conditions. According to the Kiel classification, lymphomas of a low grade of malignancy (17 cases) and lymphomas of a high grade of malignancy (34 cases) were distinguished among NML. Significantly higher activity of the nucleolar organizers (NO) was observed in lymphomas of a high malignancy grade, particularly in lymphomas formed of cells of early differentiation stages, and enhancement of NO activity with development of lymphoma aggression. "Anomalies" of NO activity expression reflecting clinical lymphoma presentation were noted among NML of various malignancy grade. NML with a high NO activity are described as having less favourable prognosis compared to lymphomas with low NO activity. No significant difference in NO activity was found between reactive follicular hyperplasia and follicular centroblast-centrocytic lymphoma. Thus, NO activity can be considered as an additional diagnostic marker of NML grade of malignancy and prognosis.

Involvement of the temporal arteries, considered to be the hallmark of giant cell arteritis, is rather rare in other pathologic processes. We describe a patient with lymphoma involving temporal artery. A 68-year-old man has been followed closely without therapy since 1989 for a low-grade non-Hodgkin lymphoma. He presented in 1995 with asymptomatic nodules on the temporal artery with preservation of the pulse. Temporal artery biopsy showed periarterial infiltration of mononuclear cells in keeping with follicular mixed cell lymphoma. The differential diagnosis of temporal arteritis must therefore, include other vasculitides, light chain amyloidosis but also lymphoma and emphasize the need for a temporal artery biopsy.
