Epipodophyllotoxin (VM 26; 4'-demethyl-epipodophyllotoxin-beta-D-thenylidene glucoside) has been proved, in clinical screening, to be able to induce apparently complete remissions and pronounced though incomplete regressions in Hodgkin's disease, reticulosarcoma, and bladder cancer, as well as incomplete regressions in lymphosarcoma. Apparently complete regressions of malignant pleural effusions have been obtained after giving this drug systemically. It has a notable toxic action on the bone marrow.

The natural history of 292 consecutive cases of reticulum cell sarcoma and lymphosarcoma of Waldeyer's ring and the survival rate after radiotherapy are reported. In our institute since 1928 from 30 to 35% of pharyngeal neoplasms have been lymphomas, and of these 55% have been reticulum cell sarcomas, 21% lymphosarcomas, and 1% Hodgkin's disease. This high incidence may probably be ascribed to the fact that in all malignant lymphomas, irrespective of the clinical presentation, a systematic biopsy of the whole Waldeyer's ring was carried out. Pharyngeal lymphomas were confined to Waldeyer's ring in 19.6% of cases, with initial spread to contiguous cervical nodes in 43.8%, to distant nodes in 24.2%, and to extranodal tissues in 12.4%. Lymphography showed abnormal retroperitoneal lymph nodes in 38.3% of cases. There was gastrointestinal involvement either initially or later in 17.6% of cases. High-energy radiation therapy to both sides of the neck was the treatment of choice for local and regional disease. It achieved a five-year survival rate of 41.9% in the group of 97 patients treated during the past decade. The incidence of relapse (recurrence and new manifestations) was highest in the first year after treatment.

Epipodophyllotoxin VP 16213 (4-demethyl-epipodophyllotoxin-beta-D-ethylidene glucoside), given to 250 patients with various types of malignant disease, induced apparently complete remissions in four out of eight cases of acute monocytoid and acute myelomonocytoid leukaemia but only one complete regression and six incomplete remissions in 21 cases of reticulosarcoma. Incomplete regressions occurred in patients with Hodgkin's disease, lymphosarcoma, melanoma, and carcinoma of the breast, ovary, testis, bladder, kidney, and thyroid. Seemingly complete regressions of malignant pleural effusion occurred when the drug was given systemically. Toxic side effects interfered with treatment in 40 patients but stopped it in only nine. No signs of toxicity were seen in 114 patients and in 85 the side effects were negligible. VP 16213 represents an advance in the treatment of acute monocytoid leukaemia, which has been up till now insensitive to any drug.

One hundred and forty three patients with lymphoreticular blastoma were examined 97 of them with Hodgkin disease in various clinical stages, 26 patients with reticulosarcoma and 20--with lymphosarcoma. The following indices were determined in all patients; hemoglobin, serum iron, total ironbinding capacity and the degree of saturation of transferins. Low values of serum iron were found in the exacerbated stages of patients with Hodgkin, especially in III and IV clinical stage. They are decreased also in patients with reticulosarcoma but to a less degree, while in those with lymphosarcoma--they are within normal limits. The total iron binding capacity is elevated in the same stages, the saturation degree of transferins being decreased. Hemoglobin increases in the stage of clinical improvement in the patients with Hodgkin, as well as iron and saturation degree of transferins. The total iron binding capacity is decreasedmthe changes in the above indices, though not specific, were concluded to be able to serve as additional tests in the determination of the period and to a certain extent, of the stage in patients with Hodgkin as well as to make a differential diagnosis with the rest lymphoreticular blastomas.

A clinico-pathological study of malignant lymphoma in Jamaica was undertaken to examine the disease pattern in a predominantly negro population of West African origin. During a 9-year period (1958-66) 260 histologically verified cases of malignant lymphoma were encountered. The distribution of the different histological types was as follows: Hodgkin's disease 50.9%, lymphosarcoma 33%, reticulum cell sarcoma 14.2%, giant follicular lymphoma 1.9%. No cases of Burkitt's tumour were encountered.This study indicates that malignant lymphoma is not uncommon in Jamaica, and that its distribution pattern is similar to that observed in Europe and North America, except for the paucity of giant follicular lymphoma, and is different from the pattern observed in parts of Africa populated by Negroes, where Burkitt's tumour is the most common type, and where Hodgkin's disease is relatively uncommon. The age and sex incidence was in general similar to other reported series, but the duration of symptoms was short. The majority of patients presented with generalised peripheral lymphadenopathy. Hepatosplenomegaly and anaemia were common on admission. The prognosis was generally poor in comparison with European and North American series due to advanced stage of disease on presentation.

Intestinal lymphoma is frequently associated with sprue-like bowel patterns. A standardized approach to the problem is suggested.Intestinal lymphoma has been subdivided into four groups, depending on the extent of involvement of the anatomical region. Only cases with specific lesions are accepted as primary lymphoma. The villous pattern in 179 cases at necropsy with suitably preserved mucosa was determined to form a normal baseline for comparison with the uninvolved portions of the lymphomatous small bowel. Only 15% of all small bowel of this random material of children more than 1 year old and adults showed a flattened mucosa. The non-lymphomatous mucosa of 20 cases with definitely primary intestinal reticulum or lymphosarcoma showed severe sprue-like atrophy in 18 (90%). Two cases of intestinal Hodgkin's disease and four cases of gastric lymphoma were associated with regular mucosal patterns. It is concluded that sprue-like villous atrophy of the small bowel is definitely a triggering factor for the development of primary intestinal reticulum cell or lymphosarcoma. A hypothesis for the possible aetiological relationship of these two conditions is discussed.

Immunoelectrophoretic study of serum proteins in patients with various types of malignant proliferations of haemopoietic tissue was performed. It was established that myeloproliferative diseases (chronic myeloleukemia, acute leukemia) are notable for changes in alpha 2-globulins (frequent increase in the content of alpha 2-macroglobulin, haptoglobin) increase in the of beta 1 A-globulin as well as for reactive increase in the content of one or several immunoglobulins. During malignant proliferations of lymphoreticular tissue (chronic lympholeukemia, lymphosarcoma, myeloma, lymphogranulomatosis), major disturbances occur in the group of immunoglobulins, changes in which during this pathology can be of three types: reactive increase, decrease with a syndrome of antibody deficiency, paraproteinemia. The type of changes of immunoglobulins is determined by morphogenetic species of malignant cells.

The graft versus host reaction which occurs in F(1) hybrid mice injected with parental spleen cells was used to examine several immunological theories of neoplasia. Long-term survivors of this reaction developed lymphoid neoplasms which resembled Hodgkin's disease and lymphosarcoma. Mice with these tumors were chimeras, but the parental cells present within their spleens had specific immunological tolerance toward host antigens. This, together with the finding that the tumors were transplantable only to isogenic recipients, indicates that the tumors were of host rather than donor origin.

Among 8,758 necropsies there are 93 cases of systemic amyloidosis. Of these, 14 are associated with malignancy: seven with myelomatosis or malignant lymphoma, and seven with carcinoma. The incidence of amyloidosis in myelomatosis is at least 10%. Attention is drawn to the presence of amyloid in the tubular casts of ;myeloma kidney'. In Hodgkin's disease the incidence is about 4% but it may be higher in patients receiving chemotherapy. In lymphosarcoma it is of the order of a fraction of 1% but in macroglobulinaemia, essential or associated with malignant lymphoma, the incidence is considerably higher. Systemic amyloid is found in one in 375 of patients with carcinoma and in only a single patient among 1,500 ;control cases'. Renal carcinoma accounts for one-quarter of all carcinomas associated with systemic amyloid. The other carcinomas originate in a variety of organs. In myelomatosis, amyloid may be found in the tumour deposits. In Hodgkin's disease and in lymphosarcoma there appears to be greater amyloid deposition in neoplastic tissue than hitherto realized. The carcinomas provide a striking example of topographical association of amyloid and tumour, the two being closely related in six of seven cases.
