The current histological classification of lymphogranulomatosis is presented. The possibility of the occurrence of the cellular phase of nodular sclerosis is discussed. In contrast to the current opinion on the origin of Berezovsky-Sternberg giant cells from reticular cells, the evidence in favour of their lymphocytic genesis is presented. Some problems of therapeutic pathomorphosis and thanatogenesis of patients with lymphogranulomatosis when current methods of treatment (radiation and polychemotherapy) are used are considered.

A literature review about the significance and incidence of lymphography in intrathoracic diseases (lymphogranulomatosis, reticulum cell sarcoma, lymphosarcoma, chronic lymphatic leukemia, giant follicular lymphoma, pulmonary sarcoidosis) is given. Intrathoracic manifestations of malignant lymphomas are absolute indications for lymphography. With the proof of retroperitoneal lymph node involvement alterations in staging of the disease and therapeutical consequences are emerging. In benign lymph node diseases, such as pulmonary sarcoidosis, lymphography will be also helpful for staging, but therapeutic consequences do not ensue, an absolute indication for lymphography is not validated. Lymphography is valuable for the diagnosis of diseases and injuries of the thoracic duct. The existing methods of lymphographic visualization of intrathoracic lymph nodes are not yet satisfying and need further research.

Studies on quantitative DNA content in lymphnode cells in patients with lymphogranulomatosis and reticulosis indicated that small prednisolon concentrations (0.1 mg/100 mg of cells) may be responsible for a reduced amount of DNA. The study of the quantitative content and turnover rate of total RNA has shown by the labeled uridine-H3 precursor incorporation a considerable decrease of RNA content and the reduce incorporation of the label in total RNA (approximately by 40% on an average).

The authors report the results of studying the parameters of the kinetics of induced chemiluminescence of blood serum in patients with lymphogranulomatosis prior to and after the therapy. The luminescence characteristics in patients were found to differ from those in donors. Following the treatment the kinetic parameters approach the initial values. The data obtained have demonstrated the possible aspects of using the method concerned in the complex diagnostics, and to control the efficacy of the therapy for the given pathology.

Under study were T- and B-systems of immunity in patients with lymphogranulomatosis, and these were correlated with clinical findings (the stage, intoxication, the effect of chemotherapy). Blastransformation reaction (RBT) with phytohemagglutinin (PHA), direct blasttransformation with sheep erythrocytes, immunofluorescence were estimated. The absolute amount of lymphocytes in 1 ml and also T- and B-cells were counted. Already in the stage I--II there was noted a reduction in the amount of T-lymphocytes and RBT to PHA. These changes in cell immunity were aggravated in the stage III B and stage IV B in particular. After the chemotherapy conducted according to the MOPP programme in most patients the immunological background would be more unfavourable. There was noted a relationship between the immune indice stability during chemotherapy for lymphogranulomatosis and the duration of remission.

The results of natulan therapy in III patients with spread lymphogranulomatosis are reported. An analysis of the dependence of the remission frequency and duration on different morphological variants of lymphogranulomatosis has shown that the incidence of remissions was high and practically identical in patients with nodular sclerosis, the lymphocytic and mixed cell type. The effect of natulan was found to be limited in the variant "Lymphocytic depletion".

The diagnostic value of Ga-67 citrate scintigraphy was estimated based on the comparison of clinical and roentgenological findings, including lymphography, in 46 patients with lymphogranulomatosis and other malignant lymphomas. The results obtained indicated that scintigraphy with Ga-67 citrate is a valuable adjunct for diagnosing malignant lymphomas and lymphogranulomatosis in previously untreated patients and in the recurrences.

The phenomenon of antigen-induced aggregation of human buffy coat leucocytes correlates well with skin testing using purified protein derivative of tuberculin as the antigen and in this system appears to be a simple, sensitive test of cell-mediated immunity. The same phenomenon may, however, be used as an indicator of humoral immunity. In addition antigen-induced buffy coat leucocyte aggregation is suppressed by serum from patients with Hodgkin's disease and sarcoidosis. The possible uses and also the limitations of this test are discussed.

Bilateral accumulation of 67Ga-citrate by the parotid glands was observed in 9 of 27 patients (33%) with sarcoidosis and in a subgroup of 12 with systemic symptoms such as fever or clinically acute, progressive disease (75%). Asymptomatic patients or those with probable sarcoidosis associated with hilar and/or paratracheal adenopathy will not have parotid uptake. Similarly, uptake seldom occurs in normal patients or in those with untreated lymphoma or Hodgkin disease. If other inflammatory or granulomatous processes such as tuberculosis can be excluded, and if there is no history of head and neck irradiation, then this finding in acutely ill patients suggests sarcoidosis.
