Total hemolytic complement (CH50) and eight antigenic fractions of the complement system were determined in 30 patients with hematological neoplasias, distributed into the following groups: six cases of non-Hodgkin's lymphomas (NHL), seven cases of chronic lymphocytic leukemia (CLL), five cases of Hodgkin's disease (HD), seven cases of acute leukemia (AL), three cases of chronic myeloid leukemia (CML) and two cases of multiple myeloma (MM). CH50 was titred accordingly to a modification of the Kabat and Mayer method, C1q, C1s, C3, C4, C5, INHC1, C3A and properdin were determined with specific antisera by Manani and Laurell's techniques. The results obtained showed significant increase in CH50 above normal values in patients with HD, AL, and CML, especially the former, even in early stages. C1s was found to be increased in CML and AL, as well as C3 in CML. C4 is increased in CML and HD. C5 follows a course similar to C4, being also increased CLL and MM. C9 is increased in all groups, except NHL. A significant increase in C3A was found in NHL, HD and AL. There were no significant variations in C1s, INHC1 and properdin in any of the former groups. No correlation was found between clinical course and complement increase. The role of complement in neoplastic disease is discussed.

32P is effective therapy for polycythemia and primary thrombocytosis. The Polycythemia Vera Study Group is comparing radioactive phosphorus with alkylating agents to determine relative efficacy. Less well investigated is the effectiveness of 32P vs. busulfan in chronic granulocytic leukemia. Endolymphatic administration of radiopharmaceuticals may play a role in the therapy of infradiaphragmatic lymphoma. Among the radionuclides that have at times been used in hematology are 32P, 198Au 24Na, 76As, 89Sr, 52Mb, 54Mn, 91Y, 95Zr, 95Cb, 111Ag, 109Pd, 131I, 185W, and 192Ir. As stated, 32P has proven single most efficacious agent. The hematologic diseases that have been treated include both malignant and benign conditions. Among the malignant conditions are polycythemia vera, agnogenic myeloid metaplasia, thrombocythemia, leukemia, Hodgkin's disease, and multiple myeloma. Hemophilia, and Osler--Weber--Rendu disease are among the benign entities in which the agents have been tried. Polycythemia and thrombocythemia remain those in which the greatest success has been achieved.

Based on data from the cancer register of the German Democratic Republic established in 1952 and on the official mortality statistics, incidence of and mortality from malignant lymphomas (ICD 200-203) in the GDR are analysed. Age-specific incidence and mortality of Hodgkin's disease show a peak in the age group of 25-30 years and rise steadily from 45 years on up to the highest age. Lymphosarcoma and reticulosarcoma increase slowly from infancy to old age, whereas multiple myeloma is a disease of the elderly and extremely rare before the age of 40. The apparent increase of malignant lymphoma may be due to underregistration at the beginning of the cancer register. In the past years mortality from Hodgkin's disease is slowly decreasing, thus reflecting progress in methods of treatment and results.

Tasmania, an island state of the Australian Commonwealth with a population of 400,000 of predominantly Anglo-Saxon heritage, has relatively centralized oncology services. A study was undertaken of all patients known in December 1971 and of all new cases diagnosed since January 1972 with all forms of leukemia, Hodgkin's disease, non-Hodgkin's lymphoma, myeloma, and other myeloproliferative and lymphoproliferative disorders. Data were obtained with respect to lifetime residential and occupational history, schools attended, and known familial cases of any of the myeloproliferative and lymphoproliferative disorders.

The occurrence of angioimmunoblastic lymphadenopathy, Hodgkin's disease or IgG-lambda multiple myeloma is described in four patients receiving chronic diphenylhydantoin (DILANTIN) therapy. Although the association between diphenylhydantoin therapy and the development of immunosuppression and lymphoma is well documented, the role of the drug in the etiology of these disorders is still controversial. It is suggested that periodic examination of patients receiving diphenylhydantoin for lymphadenopathy and repeated serum electrophoresis may be useful in detecting early aberrations of the immune system in these individuals.

Ferritins are a group of isometric proteins having an important function in iron storage and metabolism and are found in high concentration in the liver, spleen and bone marrow. Acidic isoferritins are found in human fetal liver, primary mammary, gastric and pancreatic carcinomas, and are termed carcinofetal ferritins. Elevated levels of serum ferritin were found in patients with various malignant diseases such as Hodgkin's disease, chronic myeloblastic, granulocytic and lymphatic leukemias and myeloblastosis, in patients with breast cancer, multiple myeloma, malignant lymphoma, carcinoma of the gastro intestinal tract and germinal cell tumors of the testis. Recently a subpopulation of circulating T lymphocytes bearing surface ferritin was found in patients with breast cancer and untreated Hodgkin's disease. No such lymphocytes were demonstrated in normals or in patients with benign breast disease. The appearance of such subpopulation in the circulation is an early manifestation of the neoplastic disease, and its identification may provide a tool of potential diagnostic and prognostic importance in the management of Hodgkin's disease and breast cancer.

A retrospective review was undertaken to ascertain whether there are distinctive histopathologic features of the lymphoid neoplasms that occur in patients with previous connective tissue disease. Of 29 patients studied, 12 had malignant lymphoma with diffuse large-cell cytomorphology. Only 1 of these 12 had an immunoblastic cell type. The remaining 17 patients had neoplasia of a widely diverse nature. Six had lymphocytic lymphoma (one nodular poorly differentiated, three diffuse poorly differentiated, and three diffuse well differentiated), two had Hodgkin's disease, three had plasma cell myeloma, and six had chronic lymphocytic leukemia. Data fail to confirm a relationship between lymphoid proliferations with immunoblastic morphology and connective tissue diseases.

Immunological factors are involved in all aspects of the lymphomas and leukaemias. The aetiology of these diseases is related at least in some cases to immunodeficiency, immunostimulation, autoimmunity and a dysregulation of the immune system. The majority of lymphomas and leukaemias are monoclonal proliferations of the B-lymphocyte series at different stages of maturation while some are derived from T lymphocytes and others have no recognisable B or T-cell markers. Each of the lymphoid malignancies has a characteristic and unique pattern of immunological deficiency, suggesting a unique aetiology. Hodgkin's disease and histiocytic lymphoma, the acute leukaemias and chronic myelogenous leukaemia have predominantly cell-mediated immune deficiencies, while lymphocytic lymphoma, chronic lymphocytic leukaemia, multiple myeloma, and the plasma cell dyscrasias have predominantly humoral immune deficiencies. There is a relationship between immunocompetence and prognosis and between immunocompetence and extent of disease in the lymphomas and leukaemias. Immunocompetent patients have a better prognosis and more limited disease than immunoincompetent patients. Therapy for these diseases profoundly suppresses host defence mechanisms, particularly those which are cell-mediated. Ability to resist or recover from this immunosuppression is also associated with an improved prognosis. Lymphoma and leukaemia also induce a tumour-specific immune response in the tumour-bearing host and this also correlates with prognosis. These factors form a rational basis for immunotherapy and indeed lymphomas and leukaemias respond to active nonspecific immunotherapy with BCG and active specific immunotherapy with tumor cells resulting in prolongation of remission duration and survival.

Thirty-four patients with an IgM M-component were evaluated for clinical presentation and course, laboratory data, and histologic findings. An attempt was made to ignore the presence of the IgM M-component and to assign each patient to one of the following categories: Waldenstrom's macroglobulinemia, IgM myeloma, Hodgkin's disease, lymphoma, chronic lymphocytic leukemia, chronic lymphosarcoma cell leukemia, and IgM M-components not associated with an identifiable lymphoproliferative disorder ("benign" M-component). Although transitional forms occasionally occurred, most patients could be readily categorized. The patients with lymphoma, Hodgkin's disease, chronic lymphocytic leukemia, and chronic lymphosarcoma cell leukemia did not appear to behave differently from patients with these disorders who did not have serum IgM M-component. Both for descriptive convenience and for clinical management, continued attempts should be made to separate patients with IgM M-components according to their underlying conditions.

A patient in whom lymphocyte-depleted Hodgkin's disease and multiple myeloma were diagnosed simultaneously is reported. There was no prior history of irradiation or chemotherapy, the significance of which is discussed in relation to previously documented patients with Hodgkin's disease and myeloma.
