Pinealectomy leads to increased formation of fibrous tissue in the abdominal cavity, increased skin pigmentation and elevated cholesterol and alkaline phosphatase levels. It also leads to reduced formation and/or action of prostaglandin (PG) E1 and thromboxane (TX) A2. PGE1 plays an important role in enhancing function of T suppressor lymphocytes which control overactive antibody-producing B lymphocytes. In primary biliary cirrhosis there are increased skin pigmentation, hepatic fibrosis, elevated cholesterol and alkaline phosphatase levels, defective T lymphocytes and hyperactive B lymphocytes. Primary biliary cirrhosis may be a pineal deficiency disease. Serotonin is important in the pineal and the serotonin antagonist methysergide may cause retroperitoneal fibrosis by interfering with pineal function. There is a good deal of other evidence which suggests that melatonin PGE1 and TXA2 are important in the regulation of fibrosis in other situations such as "collagen" diseases, lithium-induced fibrosis and cardiomyopathies. This suggests that enhancement of formation of PGE1 and TXA2 may be of value in diseases associated with excess fibrosis and defective T suppressor cell function. PGE1 levels may be raised by zinc, penicillin, penicillamine and essential fatty acids. TXA2 levels may be raised by low dose colchicine. These new approaches to treatment may prove safer and more effective than existing ones. They may be of value in disorders such as cardiomyopathy, Hodgkin's disease and other lymphomas, multiple sclerosis, Crohn's disease, atopy and other diseases in which defective T cell function is suspected.

A case of Hodgkin's disease involving an area of Crohn's colitis in a patient with a 20-year history of inflammatory bowel disease is described. The coexistence of Crohn's disease and Hodgkin's lymphoma has not been reported previously. The possible relationship between gastrointestinal lymphoma and Crohn's disease also is discussed.

Ten cases of acute febrile neutrophilic dermatosis or Sweet's disease have been studied clinically and histologically. Seventy percent of the patients were females with mean age of 43.1 +/-9 years. All of them presented the typical skin lesions consisting of papules and painful erythematous--edematous plaques in face, neck and upper chest. Fever was observed in seven patients and painful joints in four cases. One case presented polyarthritis of the big joints and there was one other case of conjuntivitis. The analytical data revealed a constant increase in sedimentation rate observed in 90% of patients. Leukocytosis was observed in 30% of patients and neutrophilia in 40%. Histologically, the lesions showed a neutrophilic infiltration of the skin without signs of vasculitis. Eight patients received treatment with Prednisone per os, one of whom, because of his relapses, was later given Potassium Iodine. Another patient was treated with Indomethacin, and one patient did not received any treatment. The evolution was favorable in all cases with sustained remissions. Sweet's Syndrome has been described associated mainly with acute myeloid leukemia in 10-20% of patients and in isolated instances with other systemic and neoplasic diseases. The concomitant conditions in 50% of our cases were: Ulcerative colitis, nodular sclerotic Hodgkin disease, infiltrative ductal carcinoma of the breast, carcinoma of the uterus neck and Crohn's disease; these last two associations had not been previously described in the literature.

It has long been known that Crohn's disease may occur in association with certain forms of malignancy, specifically adenocarcinoma and lymphoma of the bowel and carcinoma of the vulva and perianal area. Here, the authors report the rare occurrence of extraintestinal Hodgkin's disease in a patient with documented regional enteritis of five years' duration. They propose explanations for this unusual combination of disease, reported in the literature only once previously, and review the pathophysiology of lymphoma and regional enteritis.

The majority of patients with sarcoidosis in this large series have had a number of biochemical investigations performed. Abnormal calcium metabolism was demonstrated in 40% of the patients but permanent renal damage due to nephrocalcinosis as a result of persistent derangement of calcium metabolism was rare. Raised immunoglobulin levels were seen. Half the white and two-thirds of the West Indian patients had elevated IgG levels. Abnormal immunoglobulin levels carried no obvious diagnostic or prognostic significance. Raised alkaline phosphatase levels reflected space-occupying hepatic granulomas and occurred in 23% of patients. Serum angiotensin converting enzyme (SACE) was elevated in half the patients. The highest SACE activity was found in patients with severe parenchymal lung infiltration due to sarcoidosis, and the lowest levels in those with inactive disease or after successful management with steroid drugs. SACE levels were not significantly elevated in four other granulomatous conditions: Crohn's disease, primary biliary cirrhosis, Hodgkin's disease, and active tuberculosis.

Hypogonadism, infertility, and sexual dysfunction occur in some men with coeliac disease. We have measured plasma testosterone, dihydrotestosterone, sex-hormone binding globulin, oestradiol, and serum luteinising hormone in 41 men with coeliac disease and have related these findings to jejunal morphology, fertility, semen quality, and sexual function. To determine the specificity of these observations in coeliacs we also studied 19 nutritionally-matched men with Crohn's disease, and men with chronic ill-health due to rheumatoid arthritis and Hodgkin's disease. The most striking endocrine findings in untreated coeliacs were increased plasma testosterone and free testosterone index, reduced dihydrotestosterone (testosterone's potent peripheral metabolite), and raised serum luteinising hormone, a pattern of abnormalities indicative of androgen resistance. As jejunal morphology improved hormone levels appeared to return to normal. This specific combination of abnormalities was not present in any of the disease control groups and, to our knowledge, androgen resistance has not been described previously in any other non-endocrine disorder. Plasma oestradiol concentration was modestly raised in 10% of coeliacs and 11% of patients with Crohn's disease. Unlike plasma androgens and serum luteinising hormone in coeliacs, plasma oestradiol was not clearly related to jejunal morphology. Androgen resistance and associated hypothalamic-pituitary dysfunction appear to be relatively specific to coeliac disease and cannot be explained merely in terms of malnutrition or chronic ill-health. In addition, our findings suggest that this endocrine disturbance may be related to sexual dysfunction in coeliac disease but its relationship to disordered spermatogenesis in this condition has not been clearly established.

The authors analysed the CT findings in 60 normal subjects and 14 patients with small bowel disease to assess the ability of CT to detect wall thickening. The patients all had a small bowel wall thickness larger than 0.5 cm. Benignity could not be distinguished from malignancy solely on the pattern of small bowel thickening. Thus, Crohn's disease could not be differentiated from intestinal Hodgkin's disease or post radiation therapy ileitis. Intra- and extramural masses are also a common but non-specific pattern; they can be associated with small bowel wall thickening and have been encountered in different situations such as duodenal duplication, jejunal haematoma or adenocarcinoma. Knowledge of normal anatomy, optimal endoluminal opacification and careful attention to the appearance of the small bowel are recommended in every CT survey of the abdomen.

A radiometric ligand binding assay for human C-reactive protein (CRP) was established using pneumococcal C polysaccharide (CPS) coupled to magnetizable cellulose particles as the solid phase ligand. Competition for binding to the solid phase between 125I-CRP and unlabelled CRP permitted detection of 30 micrograms/l of CRP and the precise assay of concentrations up to 3000 micrograms/l. Identical results were obtained when the assay was used to quantitate isolated pure CRP and pure CRP added to normal human serum. However in vitro addition of known ligands for CRP to acute phase serum resulted in lowering of the apparent CRP concentration in this assay and addition of as little as 1 microgram/l of free CPS or 1 mg/l of lecithin was demonstrable in this way. A combination of the ligand binding assay and the standard electroimmunoassay for CRP was therefore used to test acute phase sera for the presence of CRP complexed in vitro. No evidence of complexed CRP was detected among sera containing between 1-319 mg/l of CRP from patients with Hodgkin's disease (10), rheumatoid arthritis (10), Crohn's disease (19) and various microbial infections (11), including six with subacute bacterial endocarditis. Since it is likely that CRP does form complexes with its ligands in the plasma these results suggest that complexed CRP is rapidly cleared from the circulation.
