Two hundred patients treated with curative intent for
Hodgkin's disease
between October 1964 and April 1984 at a single institution were studied retrospectively for development of second malignancies. The minimum follow-up was 2 years (median, 11 years). The staging distribution was IA-B, 61; IIA, 54; IIB, 20; IIIA, 46; and IIIB, 19. Sixty-one percent of the patients had laparotomy. Initial management was irradiation alone (RA) in 143 patients and a combination of chemotherapy and irradiation (CB) in 57 patients. Actuarial 10-year survival rates were 82%, IA-B; 78%, IIA; 66%, IIB; 66%, IIIA; and 24%, IIIB. Cause-specific deaths due to
Hodgkin's disease
or complications of initial or salvage therapy occurred in 3% of IA-B patients, 18% of IIA-B patients, and 35% of IIIA-B patients. One patient had a prior T3N1
squamous cell carcinoma
of the retromolar trigone, and a second was diagnosed with concurrent
Hodgkin's disease
and granulocytic sarcoma. Subsequent solid tumors have occurred in six patients from 5 to 21 years after treatment, including papillary carcinoma of the thyroid, renal cell carcinoma, unilateral breast carcinoma, cervix carcinoma in situ, and lung carcinoma after RA, and bilateral breast carcinoma after CB. Seven fatal hematopoietic disorders (HPDs) were observed, including four acute leukemias, one dysmyeloproliferative syndrome (DMPS), one autoimmune hemolytic anemia, and one aplastic anemia. Two occurred in patients initially managed with RA who subsequently required chemotherapy for relapse. Five HPDs occurred in patients initially managed with CB who never relapsed. All HPDs were observed between 2 and 7.5 years after administration of chemotherapy. Statistical analysis of the data using a rerandomization test on Gehan ranks of treatment and clinical variables showed significant correlations between development of a secondary HPD and (1) initial management with CB; (2) higher doses of chemotherapy; and (3) more advanced disease, particularly IIIB. When only the five events generally associated with treatment (i.e. the four leukemias and one DMPS) were considered, there was a significant correlation with exposure to chemotherapy and presentation with advanced disease. The patient population was small so that interdependence between treatment factors and initial extent of disease in affecting the risk of a secondary HPD cannot be discounted but should be further investigated with larger patient populations.

No false-positive results occurred in this series, but five false-negative results were seen in the 10 patients with hyperplasia on fine-needle aspiration biopsy specimens who subsequently had open surgical biopsy. From our experience, we believe fine-needle aspiration biopsy is a useful, cost-effective initial method to evaluate lymphadenopathy in patients seen at an AIDS outpatient clinic.

The summation and classification of patients registered for planned radiation therapy in the 24-year period from 1962 to 1986, in the Department of Radiation Therapy, National Cancer Center Hospital, are reported. Patients numbered 16,471, and total sessions of treatment were 26,175. The ratio of the two figures, 1.58, is the average number of treatment sessions per patient. Peak age was 61-65 years' old and 56-60 years old, respectively for males and females. The most frequent primary site of disease for radiation therapy was the head and neck, followed by trachea, bronchus and lung, crevix uteri, breast, and esophagus. Frequency of
squamous cell carcinoma
was 38.0%; that of adenocarcinoma was 20.9%. Radical treatment was performed in 32.0% of patients in the first session, but this figure decreased to 24.7% for all sessions. There were many cases of secondary and primary palliative treatment, i.e., 31.8%. Radiotherapy was done in 70% of patients by megavoltage X-ray, with 9.4% treated by electron beam. and 1 adenocarcinoma-before and after irradiation. Individual cells were analysed by micro-electrophoresis, a new technique which estimates radiation-induced DNA strand breaks. The cells were embedded in agarose gel; after lysis of the cells in a neutral detergent solution, an electric field (5 V/cm) was applied for five minutes. DNA showed a tendency to migrate, some cell diameters, and was more pronounced in irradiated than in control cells. The DNA migration was evaluated by a microscope photometer which estimated the fluorescence in cells stained with acridine orange. This technique was found to be suitable for human material in vivo as only a few cells are needed and no radioactive prelabelling is necessary.

Culling data from this report and from the literature we reviewed the different steps concerning the diagnosis and the treatment of this complication of radiation therapy. In order to study the connection between occupation and sinonasal cancer we have reviewed the files and histological sections of 70 patients (24 females and 46 males) treated at the Department of Oto-rhino-laryngology, National Hospital of Norway. Detailed information concerning previous occupations was obtained by telephone interviews according to a standardized questionnaire. This pilot study revealed a strong association between wood dust exposure and sinonasal cancer. Of 12 wood dust exposed men, 11 had been exposed exclusively to softwood. While

(HD). Both tumors arose in previously irradiated fields. Sixteen reported cases of

Pulmonary toxicity was less frequent in pepleomycin treatment of
squamous cell carcinoma
than in bleomycin treatment. The tumor-regressing effect appeared at about day 10 of pepleomycin treatment, and at about day 21 of bleomycin treatment. The cumulative dose of pepleomycin to complete remission was smaller than that of bleomycin. Pepleomycin is effective against bleomycin-sensitive malignancies:
