We experienced a surgical case of large
Hodgkin's lymphoma
of the
thymus
. An 18 year-old male who had been complaining of a persistent cough was admitted to our hospital. Chest X-ray film showed an anterosuperior mediastinal tumor. But there was no superficial lymphadenopathy and hepatosplenomegaly. He received a surgical extirpation of the tumor approached by median sternotomy. The tumor expanded to the whole antero superior mediastinal region, which was 18.5 X 15 X 5.5 cm in size, surrounding the trachea and main branch of aortic arch and veins. The tumor directly invaded the bilateral pleura and left innominate vein, so these regions were resected with the tumor. The left innominate vein was reconstructed with a PTFE graft. The pathological diagnosis was
Hodgkin's lymphoma
nodular sclerosis type by LSG classification. A post operative course was uneventful.

Fifteen cases of non-Hodgkin's lymphoma of the anterior mediastinum are reported. In the first group of four, fresh tissue was available and immunohistochemical studies demonstrated their B-cell origin, with monotypic immunoglobulin production in two. Only fixed tissue was available in the second group of 11 patients. All stained with antibody to leucocyte common antigen (PD7/26) and three showed monotypic immunoglobulin production. If the two groups are combined seven of the 15 tumours were clearly of B-cell origin. Classification on morphological grounds was difficult, with most tumours showing mixtures of centroblasts and large centrocytes, and the original diagnoses had included
Hodgkin's disease
(three), thymoma (one) and undifferentiated carcinoma (two). None of the patients had evidence of extra-thoracic disease at presentation and when this developed the organs involved were liver (one), kidney (two) and thyroid (one). Direct extension within the chest led to infiltration of chest wall, sternum, lung, superior vena cava and other structures. The site of origin, lack of nodal involvement and, in one case, presence of residual
thymus
around the tumour indicate an origin in thymic B-cells.

Hodgkin's disease
, poorly differentiated clear cell carcinoma and seminoma, thus leading to inappropriate therapeutic approaches. We diagnosed 6 cases of MLCLS in 1986 and 1987, in 4 males and 2 females, aged 16 to 44 (mean 27). Five of the 6 patients presented with a compressive anterior mediastinal mass, dyspnea, dysphonia, superior vena cava syndrome and parietal protrusion (3 cases). The CT scan confirmed the invasive and compressive characters of the tumor. Two patients initially had disseminated disease. Microscopically, the proliferation was composed of large clear cells, and segmented by bands of sclerosis. Immunologic studies demonstrated the B origin of the tumor cells. All 6 patients received combination therapy with an intensive CHOP protocol (L N H 84 Lyon). The first patient was initially operated, after a false frozen section diagnostic of thymoma. Resistance to chemotherapy (and rapid death within 2 months) was seen in 2 cases. On patient had a partial remission and 3 entered complete remission, of 5+, 10+ and 11 months duration, respectively, with central nervous system relapse in the latter case. A synthetic
thymus
-independent (TI) antigen, dinitrophenyl-'Ficoll', was used to immunise controls, untreated patients with
Hodgkin's disease
(HD), and patients who had had splenectomy (most for staging in HD). The antibody response to DNP-ficoll was much lower in splenectomised patients than in the other groups and the antibody titres 7, 14, and 21 days after immunisation of splenectomised patients were approximately a tenth of the titres in the other groups. In contrast patients who had been primed with DNP-ficoll before splenectomy responded normally to re-immunisation 6-28 months after splenectomy. These results suggest that the antibody response upon first exposure to certain TI antigens occurs mainly in the spleen. Antibody responses are not generally impaired in splenectomised patients immunised with pneumococcal or other bacterial polysaccharides, probably because natural exposure and priming by these TI antigens before splenectomy are widespread. However, such patients must be vulnerable to overwhelming infection by encapsulated bacteria not met before splenectomy.

Some B-cell neoplasms with cellular origin different from that of centroblastic-centrocytic lymphoma, such as chronic lymphocytic leukemia and centrocytic lymphoma, had low levels of both enzymes. The role of investigation with specific antibodies against these two enzymatic activities in the physiology of lymphoma cell membrane is discussed.

were surgically treated during the 15-year period from 1969 to 1984. Twenty-six patients had far advanced disease when treatment was began. is alive with tumor bearing. It is important to distinguish malignant lymphoma from other types of thymic tumors, because chemo-radiotherapy is superior to surgical therapy. Of nine patients with germ cell tumor, two patients with seminoma have been well for 11 and 5 years. CDDP and radiotherapy were effective on their long survival. Three patients with embryonal carcinoma responded well to combined surgical and chemo-radiotherapy. But only one of them is alive with tumor free one year after operation. Prosthetic grafts were employed in 7 cases for replacement of the innominate and superior caval vein. Angiogram taken one month after operation disclosed the overall patency rate with 92% and the longest patent period confirmed was 4 years. Extended operation by using the prosthetic graft should be performed in the advanced cases to achieve a complete removal and to aim better prognosis for this kind of disease. In conclusion, aggressive surgical removal followed by radio-chemotherapy offers best cure of the malignant thymic tumor.

To determine the significance of 67Ga localization in the

of children, 142 67Ga photoscans from 45 children with various tumors were studied. Sixty-nine photoscans were taken for 17 cases of lymphoma, 73 photoscans were made for 28 cases of nonlymphoid tumors. Thymic localization of 67Ga was positive in 16 (36%) of the 45 patients and in 30 (21%) of the 142 photoscans. Positive

scans were seen in five (29%) of the 17 cases of lymphoma and 11 (39%) of the 28 solid tumors. The positive incidence was highest (90%) in ages 1-2 yr old. Of the eight grade 2 (strong positive) patients, the

in one case of
Hodgkin's disease
was diagnosed as malignant and the other seven solid tumor cases were nonmalignant. Most of the latter seven cases became positive after beginning of treatment (surgery and/or chemotherapy). Although the precise mechanism is not well understood, thymic localization of 67Ga may represent immunologic response to tumors, especially in infants with nonlymphoid neoplasms.
