Gilead Sciences has seen beneficial results from two trials of adefovir dipivoxil (Preveon) in the treatment of HIV. An additional trial of adefovir dipivoxil was a success, dramatically reducing hepatitis B viral load by 99.99 percent. GS 411 assigned treatment-naive patients to one of five regimens that included indinavir (Crixivan) and adefovir. The results suggest that adefovir, which is metabolized differently and has different side effects, may be an alternative for some nucleoside analogs. GS 408 added adefovir to the regimens of treatment-experienced patients and showed a marked decrease in viral load for those patients who used adefovir versus a placebo. Adefovir dipivoxil may also be used effectively against other viruses such as CMV, HHV6 and Epstein-Barr. Long-term effectiveness in controlling the HIV virus has yet to be determined.

Adefovir (Preveon) is a new compound for treating HIV infection and presents no risk for developing cross resistance. Adefovir, at 125 mg/day, has created a 0.5 log decrease in viral load in 50 percent of patients tested. At 250 mg/day, the decreases are similar. Increases in CD4+ cells are also recorded. Adverse reactions from adefovir include nausea, diarrhea, weakness, headache, and pain; slight elevations in liver enzymes were also recorded in some subjects. At the 12th week of the study, patients also experienced drops in carnitine levels by 42 percent at 125 mg/day, and 62 percent at 250 mg/day. Overall, adefovir is determined to be better tolerated at 125 mg/day with no greater benefits derived at higher doses.

The trial program for adefovir dipivoxil (preveon), an anti-HIV drug, has been expanded to include individuals whose current therapies have failed and who need new treatment alternatives. Individuals can be included regardless of their CD4 T-cell counts or viral load. Adefovir has been shown to lower the viral load of hepatitis B virus. Long-term management of HIV shows that combination therapy can achieve viral loads of under 50 copies, sustainable for 3 years or more. Highly affective antiretroviral therapy (HAART) is the most effective prevention against opportunistic infections due to its ability to stimulate the immune system. Some statistics show as much as an 82 percent decline in opportunistic infections in patients on HAART. Combination therapy does not work equally well for everyone, largely due to side effects and cross resistance, but newer medications and regimens appear more effective and have fewer side effects. Updated information is given for several new anti-HIV drugs, including amprenavir and adefovir (Preveon, 1592U89). Results of recent clinical trials with these drugs, including their effectiveness in restoring immune system strength, are reviewed. Genotypic and phenotypic resistance testing are also discussed.

The 38th Interscience Conference on Antimicrobial Agents and Chemotherapy featured information on new antiretroviral treatments, changes in dosing, and new vaccine information. Adefovir dipivoxil (Preveon) is a new nucleoside reverse transcriptase inhibitor that is administered once daily. A study of adefovir dipivoxil is described and the side effects are detailed. A clinical trial of the protease inhibitor Nelfinavir (Viracept) has shown that dosing twice a day may be as effective as the currently prescribed three times a day. Since lower dosing tends to increase a patient's adherence to treatment, it may have a long-term positive effect on treating HIV. Other drug treatments and possible changes in dosing are presented. A pneumococcal vaccine study is described, and it was found that the vaccine did not lead to an increase in viral load. Immunization against Pneumococcus is suggested for all adults who are HIV-infected.

Also addressed were the pharmacokinetics in patients with liver disease; the toxicities associated with Adefovir and Amprenavir (APV); and the uses of Hydroxyurea (HU).

Data on Gilead's anti-HIV drug, adefovir dipivoxil (Preveon), were presented at the 12th International Conference on Antiviral Research. A 4-week study compared 60 mg daily of adefovir against a placebo in 47 treatment-naive patients. Mean viral loads dropped in the study group compared to the placebo group. Another study looked at the drug in triple combination therapy at doses of 60 mg and 120 mg. Significant reductions in viral loads were also seen among these patients. Gilead evaluated the lower dose because of concern over nephrotoxicity (kidney toxicity) which occurred with the higher doses.

That cycle of research and development appears to have shifted, with only one drug expected to receive approval in the coming months. That drug, adefovir dipivoxil (Preveon) is Gilead Science's nucleotide analog. Other compounds are early in the development cycle. Those new compounds, which represent all three current anti-HIV drug classes, are summarized, and results presented at medical conferences during the past year are detailed. Several other drugs in development are aiming at novel targets. One new class of drugs is the fusion inhibitors.
