Controversy exists over whether numbers obtained from Quant. PCR & bDNA tests actually represent the
HIV
virus; however, a researcher is collecting evidence to determine whether both tests have the same prognostic value as the beta 2 microglobulin tests. There is enough evidence to show that beta 2 microglobulin levels correlate directly with high viral loads. The researcher speculates that Quant. PCR or bDNA measures the combined effects of both
HIV
and HHV-6A infection in destroying cells. To help resolve this and other issues, volunteers with Chronic Fatigue Immune Dysfunction Syndrome (CFIDS) and active HHV-6A infection are being sought. Volunteers will be asked to use
Norvir
after which its effectiveness against HHV-6A infection will be determined. CFIDS patients will show a viral load for
HIV
even though they do not have the virus; the research speculates that if the PCR test for
HIV
is non-detectable in HHV-6A related cases, it will prove that Quant PCR is not measuring HHV-6A titers in persons with AIDS. Other diagnostic tests are available to patients. Five of these tests are briefly summarized and responses to patient questions regarding viral load issues and therapy are included.

therapy research continues to show promising results and a research update is presented on the following antiviral topics: efavirenz (DMP 266, Sustiva), indinavir (Crixivan), indinavir plus ritonavir (
Norvir
), hard vs. soft gel saquinavir, nelfinavir (Viracept) cross-resistance, and T-20 (pentafuside). Tables present regimen results, including viral load drop and CD4 cell increases. Difficulties in preventing viral resistance are also discussed. Situations to avoid include using dosing schedules that do not sufficiently suppress the virus, waiting for viral load to return to pre-treatment levels before switching drugs, and not switching drugs to at least two new potent compounds when changing combination therapy. The challenge for patients who have failed previous drug therapies is to develop strategies that allow them to extend the effectiveness of their options long enough for at least two new potent drugs to become available.

drugs Viramune (Nevirapine) and Sustiva (efavirenz) can reduce levels of Methadone, sometimes causing withdrawal. Other drugs already known to reduce Methadone levels include

(Ritonavir) and Viracept (Nelfinavir), while Crixivan (Indinavir) and Fortovase (Saquinavir) may increase them. Another study has shown that Methadone may lower levels of ddI (Videx), suggesting a need to increase ddI dosages in those taking Methadone.
