The experience of 22 Hodgkin's disease (HD) patients with human immunodeficiency virus type I (HIV) antibodies, collected from a cooperative study of six hospitals during 1984-1989 is presented. Young men (average age, 27.6 years) with a high incidence of
intravenous drug abuse
(86%) were found. The status of the
HIV infection
at diagnosis of HD was: four patients, acquired immune deficiency syndrome (AIDS); eight patients, persistent generalized lymphadenopathy (PGL); and ten patients, asymptomatic. The natural history of HD was unusual, with a high incidence of B symptoms (81%), advanced Stages III to IV (90%), bone marrow invasion (50%), cytopenias before treatment (45%), opportunistic infections (68%), and aggressive histologies. A decreased response to chemotherapy with poor marrow tolerance and a significant decrease in survival, 18 months, was observed. The AIDS and cytopenias pretreatment were associated with a shorter statistically significant survival, which defines the importance of immunodeficiency in HD prognosis. Complete remission after treatment was a factor that contributed to a longer statistically significant survival. The PGL or asymptomatic patients survived longer but also had a poor course, and five of them had AIDS criteria during evolution. A high incidence of HD in relation to non-Hodgkin's lymphoma (NHL) in patients with
HIV infection
in the six cooperating hospitals was found. Criteria for considering HD as an AIDS-associated lymphoproliferative disease in our environment are discussed.

Since January 1985, 77 women have been found to be
HIV
-positive in the Department for Infectious Diseases, Hvidovre Hospital. Thirty-four out of these 77 women had histories of
intravenous drug abuse
, 11 had probably been infected with
HIV
by transfusion while 32 had probably been infected heterosexually. Out of the heterosexually infected women, a total of 9 were found to be living in a regular partnership with a
HIV
positive partner and eight of these couples were suitable for investigation. Seven of the eight women were probably infected by their regular male partner, four of whom belonged to known risk groups. Six of the eight men were
HIV
tested on account of
HIV
related symptoms while all of the women were tested because their regular partners were found to be
HIV
positive. The diagnosis of AIDS was established in ten out of 16 persons. Six of the men and three of the women were found to have AIDS less than one year after the first positive
HIV
test while the diagnosis was established in one man 22 months after the
HIV
test. The six men with
HIV
related symptoms had sought medical help on an average eight weeks before a
HIV
test was carried out and the diagnosis of AIDS was established in five immediately after referral to the special department. It is emphasized that every Danish doctor should be aware of the symptoms of
HIV
and these should be borne in mind even in the case of patients who do not belong to the known risk groups.

Infection with the human immunodeficiency virus (HIV) among reproductive-age women occurs disproportionately among inner-city minority populations. These women are at risk because of
intravenous drug abuse
and heterosexual transmission from partners infected through drug abuse. From July 1, 1988 to December 31, 1988, we conducted routine voluntary screening for HIV antibody among 923 women who requested induced first-trimester abortion at Grady Memorial Hospital. Eight (8.7 per 1000) women were seropositive on repeat enzyme-linked immunosorbent assay and Western blot testing. Two infected women had had heterosexual contact with a person at risk for
HIV infection
, two others reported "crack" cocaine use, and four acknowledged no risk factors. Thirteen percent of seronegative women reported risk factors for
HIV infection
. Nearly all women consented to HIV testing, and most completed the risk-behavior questionnaire. These data suggest that women seeking first-trimester abortion at our hospital are at risk for
HIV infection
.

A population of young children is appearing with
HIV infection
of perinatal origin whose personal and family problems are complex. Most of their mothers are involved with
intravenous drug abuse
. The infants are in reasonable health for the first half year and then present with increasing medical and developmental problems. Eventually, the majority require foster care. Some are integrated in intervention or preschool services, with confidentiality protected; others are in segregated educational programs. Family supports and specialized services are often inadequate.

One hundred and ninety-nine patients with a history of
intravenous drug abuse
, and enrolled on the St Luke's-Roosevelt Hospital Center Methadone Program, had baseline evaluations performed from September 1984 to April 1987. The study was designed to examine immunologic parameters associated with
HIV
seropositivity and those predictive of progression to AIDS-related complex (ARC) and AIDS. Sixty-four patients (32%) had antibodies to
HIV
by enzyme-linked immunosorbent assay (ELISA), with confirmation by Western blot and none of these patients had ARC or AIDS at the time of initial evaluation. The mean values for white blood-cell count, absolute lymphocyte count, proportion and absolute CD4, and CD4/CD8 ratio were decreased significantly in the
HIV
-seropositive group compared with the
HIV
-seronegative group. On the other hand, levels of circulating beta 2-microglobulin, SCD8, SIL-2R, and
HIV
p24 antigen were significantly elevated in the
HIV
-seropositive group compared with the
HIV
-seronegative group.

A high percentage of intravenous drug addicts was found among
HIV
-infected individuals who had been detected by anonymous testing from May, 1985, to October, 1988, at the AIDS Counselling Service of the Municipal Health Department in Stuttgart. Among the subjects being seropositive for
HIV
-1 antibodies, 36.6% were drug abusers, while until October, 1988, only 10.3% of the reported AIDS cases in the Federal Republic of Germany were members of this risk group. On the other hand, only 1.9% of 1.001 individuals who came to the AIDS Counselling Service of the Municipal Health Department in Stuttgart for
HIV
antibody testing were drug addicts. Our observations show that special information campaigns for young people about the risk of
HIV infection
by
intravenous drug abuse
should be intensified.

The number of AIDS cases in individuals 50 years of age or older in the United States is reported to exceed 9000. Contaminated blood and blood transfusions are major contributors to
HIV
-1 infection in this age group. Sexual transmission and infection through
intravenous drug abuse
are also potential avenues of
HIV
-1 transmission in the older population. AIDS presents with a variety of clinical manifestations, including dementia, frequently seen in non-
HIV
-1-infected older people. Neurological deficiencies associated with AIDS are very common and may lead to misdiagnosis in the elderly. The observed incubation period of
HIV
-1 infection is longer than previously estimated, increasing the risk of older individuals exposed to
HIV
-1 in the past to develop AIDS. Oral manifestations may present as one of the early clinical signs of AIDS. Little is known concerning AIDS in the geriatric population. It demands consideration by dental professionals treating older individuals belonging to one of the exposure categories of the disease.

In a series of 29
HIV
-seropositive women (mean age 27.4 years; 75.9 per cent acquired
HIV
by
intravenous drug abuse
) observed over a 2 years' period, we were able to determine the main epidemiological characteristics of cervico-vaginal and vulvar lesions caused by human papillomavirus (HPV). More than 58 per cent of cervico-vaginal smears showed cytological evidence of HPV infection (koilocytosis), and 6 cervical biopsies showed histological abnormalities (from CIN I to invasive carcinoma). Condyloma acuminata was found in 38 per cent of the cases, always associated with cervical HPV lesions. Systematic colposcopy of the whole female genital tract frequently detected multifocal HPV infection.
HIV
-seropositive women constitute a high-risk group for cervico-vaginal and vulvar HPV infection. They clearly need close supervision with frequent cervical smears and, preferably, colposcopy and biopsy for early detection and eradication of genital dysplasias and viral lesions.

A group general practice in Dublin's inner city has had extensive experience of intravenous drug users since the late 1970s. Since 1985 a total of 54 human immunodeficiency virus (HIV) seropositive patients have attended the practice, of whom 48 are intravenous drug users, four are the children of drug users and two have been infected through sexual contacts. Three patients have developed the acquired immune deficiency syndrome and at least eight have symptomatic
HIV disease
. Sixty per cent of Ireland's seropositive population have been infected through
intravenous drug abuse
but nationally only 16% of all intravenous drug users tested are seropositive; in the study practice, however, at least 35% (48/137) of known intravenous drug users are seropositive.

The current epidemic of
HIV infection
/AIDS initially limited to homosexual males appears to be spreading through heterosexual transmission and
intravenous drug abuse
. Infants, children, and adolescents are affected. We examine the modes of transmission of
HIV infection
in this population and the impact the spreading of this infection in the pediatric age group will have on Texas.
