Thalidomide (Synovir), noted for causing severe birth defects when taken by pregnant women, can effectively and safely heal serious mouth ulcers (oral aphthous ulcers) in people with

Interim results from a study supported by the National Institute of Allergy and Infectious Diseases (NIAID) show that ulcers healed in 14 of 23 people taking 200 mg/day oral thalidomide compared to only 1 of 22 people receiving placebo. The Food and Drug Administration's (FDA) Antiviral Drugs Advisory Committee met February 27 to March 1, 1996. At the meeting, the FDA granted full approval of ritonavir for the treatment of advanced AIDS. Ritonavir manufacturer, Abbott Laboratories, characterized the drug as generally well-tolerated, with the most common side effects being nausea, vomiting, and diarrhea. Controversy exists about the use of a new oral testosterone treatment, oxandralone (Oxandrin), manufactured by Bio-Technology General of Iselin, NJ. The drug was approved through the Food and Drug Administration's investigational new drugs (IND) procedure for promoting weight gain in
HIV
-positive individuals. Oxandralone, approved in the 1960s for disorders affecting growth in children, has been shown to have more anabolic activity and fewer masculinizing effects than other testosterone drugs, according to the company literature. However, experts in
wasting syndrome
are not prescribing the drug for their patients until larger efficacy studies are conducted. Some experts are prescribing the drug for patients identified with low levels of testosterone--research shows that about 20 percent of
HIV
-positive men have low testosterone levels. Kaposi's sarcoma (KS) may be an adverse effect of the testosterone supplement. Several case reports show that high doses of testosterone can suppress the biological mechanisms that help control KS lesions. Wasting syndrome

, is more often found in people of low socioeconomic status and women. The pathophysiology of wasting and its treatment are discussed. Perturbations of metabolism, malnutrition, androgen deficiency, treating underlying illnesses, nutritional considerations, and pharmacologic approaches are explored. The first step to treating HIV wasting syndrome is to provide appropriate antiretroviral therapy, including meticulous prophylaxis and treatment of opportunistic infections (OIs). Other treatments include using oral supplements or total parenteral feeding to maintain nutritional levels, approved appetite stimulants, such as Megestrol acetate and dronabinol, or the controversial use of anabolic steroids. Concerns about steroid use include potential for abuse, and the possibility of developing malignancies and severe gonadal dysfunction.

, in which chronic loss of muscle makes the patient feel progressively weaker and sicker. Exercise and eating can minimize muscle loss. Patients are advised to eat small frequent meals with high calorie and high protein foods. They should also eat at least four servings of starches, two of dairy products, and three fruits and vegetables. Exercise is important, and can reverse some of the muscle loss.
