Involvement of the liver with the same opportunistic organisms and neoplasms affecting other organs has been recognized since the beginning of the AIDS epidemic. In this overview of hepatic histopathologic features in AIDS, we review the range of opportunistic infections and neoplasms accompanying HIV infection. Hepatic disease may result from viral, bacterial, protozoal, or fungal infection, or secondary to drugs and neoplasms. Liver involvement in AIDS usually reflects disseminated rather than primary disease. CMV and mycobacteria are the most common organisms in liver identified in biopsy and autopsy studies. A variety of nonspecific features, including steatosis, granulomas, and sinusoidal abnormalities may also be seen. HIV-1 itself was recently identified in the liver. Speculation regarding the significance of this finding has been discussed in this review. Hepatitis B, C, and D may also complicate the course of disease in patients with AIDS. Hepatitis B behaves differently in the population with AIDS than in immunocompetent patients. We concluded our review with a discussion of the present recommendations regarding the use of liver biopsies in these patients. This topic continues to be widely debated in the literature.

In a prospective clinical trial the risk of infection after application of virus inactivated antithrombin III concentrate ANTITHROMBIN III IMMUNO (AT III) was investigated in patients undergoing cardiovascular surgery. The study was conducted according to the recommendations of the International Committee on Thrombosis and Hemostasis (ICTH), with the exception that most patients required additional blood products as well as AT III. Twenty-seven patients were eligible to test for the risk of acquiring hepatitis B. Twenty-six patients could be evaluated in terms of hepatitis NANB transmission considering ALT-levels whereas 20 patients could be tested for anti-HCV one year after surgery. Samples from 78 patients could be monitored for anti-HIV-1. None of these patients showed any signs of infection. AT III IMMUNO seems to be an antithrombin III concentrate with low or absent infectivity.

We assessed the prevalence and clinical significance of antibodies to hepatitis C virus among a cohort of 148 patients with chronic hepatitis B virus infection. Sixteen patients (11%) had anti-hepatitis C virus detectable by enzyme-linked immunoassay. The results from eight of these patients were positive by recombinant immunoblot assay. The results of recombinant immunoblot assay testing were not consistent; therefore the analysis of the patients' data was based on anti-hepatitis C virus enzyme-linked immunoassay results. Patients with chronic hepatitis B with anti-hepatitis C virus were more likely to be cirrhotic (44% vs. 21%) and to have decompensated liver disease (24% vs. 6%). Hepatitis B virus replication appeared to be suppressed in patients with both infections as measured by hepatitis B virus-associated DNA polymerase activity (mean = 2,055 vs. 2,555 cpm). Human immunodeficiency virus infection was more common in the anti-hepatitis C virus positive group (36% vs. 11%). Thus hepatitis C virus appears to suppress hepatitis B virus replication and to cause more severe liver disease in patients with chronic hepatitis B infection.

In order to obtain more information on sexual transmission of hepatitis C (HCV) we compared different high-risk groups for HIV and hepatitis B to see if they were seropositive for HCV. A high seroprevalence (38/81) of hepatitis C (HCV) was found among intravenous drug users. Nursing staff (n = 35) and patients of a dialysis unit (n = 57) had a low prevalence of anti-HCV antibodies (0% and 5%, respectively). Serology laboratory technicians also had a very low prevalence (0% out of 29). Among prostitutes (n = 114), healthy homosexual men (n = 132) and HIV-infected homosexual men (n = 31), we found a remarkably low seroprevalence of HCV (3.5%, 0.8% and 0.0% respectively). These data support the view that parenteral exposure to the virus is the most important way of acquiring the infection and that neither heterosexual nor homosexual promiscuity are associated with a high risk of transmission of hepatitis C.

The fact that viruses can cause cancer in animals has been appreciated since the turn of the century. The widely held belief that viruses had little to do with cancer in humans has only recently been dispelled. Two classes of human retrovirus (HTLV and HIV) have been discovered in the last decade and the malignant potential of hepatitis B virus, Epstein Barr Virus and the human papilloma virus have been documented not only by confirming their association with disease by large scale epidemological studies but also at the molecular level. Indeed detailed investigation of the way viruses can cause cancer can reveal new insights into 'final common pathways' and hopefully provide new approaches for treatment over and above the real possibility that virus associated cancers can potentially be vaccinated against.

The results of antibody assays for viruses of the herpes group (HSV, EBV, VZV and CMV) and for hepatitis B virus (HBV) were retrospectively evaluated in 439 HIV-seropositive patients classified into different stages of HIV infection. The prevalence of specific IgG, IgM and IgA antibodies in these groups was compared with that of a control group of HIV-negative unselected hospital patients. Antibodies to herpes viruses and HBV were more prevalent amongst HIV-seropositives, especially LAS and AIDS patients than in controls. However, marked differences were found only with CMV-IgG and anti-HBc-IgG, both with a comparatively low prevalence in HIV-negative persons (64.5% and 23.2%). Significantly more seropositives were found among asymptomatic HIV carriers (83.3% and 50%) and still more in patients with full-blown AIDS (95.4% and 82.5%). The increased frequency of CMV and HBV antibodies, already seen in asymptomatic HIV patients reflects their higher risk for sexually transmitted infections. Moreover, IgA antibodies to CMV were detected in 25.4% of LAS and 37.3% of AIDS patients, respectively, but only in 7.6% of the controls. Elevated CMV-IgA titres were found exclusively in HIV-infected persons. The differences in the antibody patterns found in this cross-sectional study may reflect the progression of the HIV disease. However, prospective follow-up studies are required to assess the value of these markers as indicators of prognosis in HIV-infected subjects.

The prevalence of antibodies to hepatitis C virus (anti-HCV) was investigated among different populations in Taiwan, where anti-HCV was detected in 0.8% (24/2,994) of adult volunteer blood donors, 0.1% (1/1,305) of youngsters and children, 12.5% (8/64) of adult volunteer blood donors with elevated alanine aminotransferase (ALT), 36.5% (23/63) of hemodialysis patients, 4.1% (13/318) of male homosexuals, 25.4% (16/63) of cases positive for antibodies to human immunodeficiency virus (anti-HIV), 82.2% (578/703) of intravenous drug users (IVDUs), and 10.3% (23/223) of female prostitutes (FPs). Among patients with chronic liver diseases including chronic hepatitis, cirrhosis and hepatocellular carcinoma (HCC), the overall prevalence rate for anti-HCV was 34.1% (42/123), and a higher prevalence was noted in hepatitis B surface antigen (HBsAg)-negative cases than in HBsAg-positive cases. The prevalence of anti-HCV in volunteer blood donors and high prevalence found in IVDUs, hemodialysis patients, anti-HIV positive cases, and FPs are consistent with those results from other countries. These findings suggest that hepatitis C virus (HCV) infection is transmitted by both blood-borne and sexual contact routes. Among flavivirus infections, anti-HCV was detected in 0.3% (1/289) and 1.3% (4/310) of Japanese encephalitis and dengue fever patients, respectively. In conclusion, in Taiwan, an area with high endemicity of hepatitis B virus (HBV) infection, the epidemiological status of HCV infection is similar to that observed in other countries, and no serum cross-reactivity was noticed between HCV and flavivirus infections.

A seroprevalence study was carried out on 1757 outpatients consecutively seen in a sexually transmitted disease (STD) clinic in order to evaluate the sexual transmission of hepatitis C virus (HCV). A total of 1442 consenting patients were tested for hepatitis C, hepatitis B and human immunodeficiency virus type 1 (HCV, HBV, HIV-1) antibodies. The relations between anti-HCV, anti-HBc and anti-HIV-1 were studied. Of 73 anti-HCV positive reactions, 45 (61.6%) were confirmed by the recombinant immunoblot assay (RIBA). The proportion of individuals with anti-HCV was higher in outpatients with a history of sexually transmitted disease than without. It was 2.8% in non drug user heterosexuals and 2.9% in non drug user homosexuals. Intravenous drug users (IDU) had higher anti-HCV prevalence when a history of STD was taken into account (42.3% in subjects with STD versus 36.7% in subjects without STD). Among non drug user heterosexuals an association was found between anti-HCV and anti-HBc. These data suggest that sexual transmission of HCV occurs, although it seems to be less efficient than other parenteral modes of transmission. When a more sensitive and specific marker of HCV infection become available, a more accurate estimate of the frequency and efficiency of the sexual transmission will be possible.

The present study compares two new factor VIII preparations currently used in the treatment of haemophilia. A Factor VIII was partially purified from plasma obtained from unpaid voluntary Belgian donors in the blood transfusion centers of Lille and Amsterdam and virus-inactivated by exposure to solvent-detergent (FVIII-SD) or by pasteurization (FVIII-P) respectively. The factor VIII content and the purity of both preparations were assessed in vitro, whereas in vivo we studied the recovery and the plasma half-life of both concentrates. The higher purity of FVIII-SD was confirmed. Factor VIII-P preparations contained more factor VIII than mentioned on the label. Both preparations gave good in vivo recoveries and half-lives. Patients who did not have antibodies to hepatitis B, hepatitis C or HIV at the initial screening, remained negative after six months treatment with the new concentrates. No patients developed neutralizing factor VIII antibodies. Furthermore patients appreciated the ease of administration of both preparations. In conclusion both FVIII concentrates are suited for the treatment of haemophilia A patients.

The discussions on the pros and cons of obstetric screening for connatal infections have been going on for years. We, therefore, conducted a prevalence study of the most common connatal infections. HIV infection, rubella and syphilis were not subjects of this study. We analysed the relevance of these infections in 512 pregnant women and their newborn infants at the moment of delivery. Further serological tests were run three months post partum, if necessary even for a longer period. Cytomegaly IgG antibodies were found in 46% of the examined women, IgM antibodies in 1.3%. Women under the age of twenty and women of low social standing showed the highest rate of prevalence of infection with CMV. The prevalence of IgG antibodies against parvovirus B 19 was 29%. In 10 mothers, positive IgM titers were found at the time of delivery. In all these women, pregnancies had been uneventful. However, 9 mothers exhibited a significantly raised abortion rate within the last 20 months before delivery. 7 of 512 women turned out to be HBs antigen carriers, 3 women and their babies were anti-HCV positive. The prevalence of toxoplasmosis IgG antibodies was 36%, of IgM antibodies 5.3%. By further investigation (Toxo ISAGA, Toxo IgA) we were able to detect one child with connatal toxoplasmosis. We conclude, that screening for parvovirus B 19 and hepatitis C is required only, if there are contact or clinical hints that the patients might have acquired either one of these infections. But we postulate, that a routine screening programme for hepatitis B and toxoplasmosis should be carried out in all pregnant women.(ABSTRACT TRUNCATED AT 250 WORDS)
