Dietitians have the opportunity to be leaders in providing medical nutrition therapy and home-delivered meals for people with human immunodeficiency virus/acquired immunodeficiency syndrome (
HIV
/AIDS) in the community setting. Four community agencies developed the Visiting Nurse Service
HIV
/Home Delivered Meals Program. Fifty clients with the diagnosis of
HIV
and/or AIDS were served for a 6-month period. The program provided convenient, energy-enhanced nutritious meals. Participants received daily hot and
cold
, energy- and protein-enhanced home-delivered meals; weekly high-energy, high-protein, shelf-stable Snack Packs; a Medical Nutritional Supplement Sampler Pack; and two home visits from dietitians. Different diet options were available. Dietitians completed 47 initial nutrition assessment and food safety education visits 1 to 2 weeks after meal delivery began, and 35 participants received follow-up nutrition counseling visits 4 to 8 weeks later. Participant data (including weights) were self-reported. Dietitians reported that 13 of 35 participants gained weight (mean = 6 lb), 11 of 35 remained the same weight, and 11 of 35 lost weight (mean = 5 lb). Initially, 14 of 35 participants were considered to be "doing well"; this improved to 19 of 35 participants 1 to 2 months later. Snack Packs were effective means to help participants meet some of their increased nutrient needs. The Medical Nutritional Supplement Sampler Pack was effective in familiarizing each participant with available products. Medical nutrition therapy by registered dietitians helped most participants improve their food consumption.

A national survey was conducted to investigate current procedures in New Zealand dental practices for control of cross-infection, and to gauge dentists' perceptions of their current procedures. A questionnaire was sent to all dental practices in New Zealand, and non-returns were followed up by two further mailings. From three mailings 767 useable questionnaires were returned (71.3 per cent of those sent out, 79.0 per cent of those potentially valid). Responses indicated that formal guidelines were generally well received, but varying proportions thought that some recommendations were unrealistic or irrelevant. Asked whether the dental practice offered treatment to 'high risk' (
HIV
-positive or HBAg-positive) patients, 25.3 per cent responded that none sought treatment. Almost all practices (92.0 per cent) had an autoclave, but only 42.8 per cent of practices autoclaved dental handpieces, and 10.9 per cent of practices wiped,
cold
disinfected or boiled extraction forceps. The large majority of responses complied with guidelines, but specific difficulties were identified with regard to sterilisation of handpieces.

-infected patients with diarrhoea. Laboratory diagnosis of cryptosporidiosis was performed of faecal samples concentrated by a formalin-ether sedimentation technique and stained by a modified

Ziehl-Neelsen method. Results demonstrated that 17 (33.3%) of these patients were infected with Cryptosporidium. Moreover, Cryptosporidium infection was the first clinical marker of AIDS in 7 cases. Our data show that the prevalence of this parasitosis in

-infected people seems to be higher in our region (Apulia, South Italy), compared to other areas of the world.

On the basis of numerous animal experiments, a pilot study was undertaken to evaluate the effect of undenatured, biologically active, dietary whey protein in 3

-seropositive individuals over a period of 3 months. Whey protein concentrate was prepared so that the most thermosensitive proteins, such as serum albumin which contains 6 glutamylcysteine groups, would be in undenatured form. Whey protein powder dissolved in a drink of the patient's choice was drunk
cold
in quantities that were increased progressively from 8.4 to 39.2 g per day. Patients took whey proteins without adverse side effects. In the 3 patients whose body weight had been stable in the preceding 2 months, weight gain increased progressively between 2 and 7 kg, with 2 of the patients reaching ideal body weight. Serum proteins, including albumin, remained unchanged and within normal range, indicating that protein replenishment per se was not likely the cause of increased body weight. The glutathione content of the blood mononuclear cells was, as expected, below normal values in all patients at the beginning of the study. Over the 3-month period, glutathione levels increased in all 3 cases. In conclusion, these preliminary data indicate that, in patients who maintain an adequate total caloric intake, the addition of "bioactive" whey protein concentrate as a significant portion of total protein intake increases body weight and shows elevation of glutathione (GSH) content of mononuclear cells toward normal levels. This pilot study will serve as a basis for a much larger clinical trial.

We report the analysis of 4892 parasitological stool samples coming from outpatients and patients of a nutritional recuperation center of north Santiago. Cryptosporidium parvum was detected in 21 samples (0.4%). The protozoan was detected in 6 of 1203 samples from children of less than 2 years old, 3 of 1.727 samples from children between 6 and 15 years, none of 776 samples from healthy adults and 2 of 13 samples from
HIV
infected patients. Nine of 97 children of less than 2 years old, hospitalized in the nutritional recuperation center, were infected with Cryptosporidium; this frequency was significantly higher than that of outpatients of the same age. Most infections were detected from May to August, a rainy and mildly
cold
period. It is concluded that Cryptosporidium infections are infrequent in healthy outpatients and that its prevalence increased in hospitalized children and
HIV
infected adults.

This study investigated gay men's use of perceptible characteristics to infer antibody status. Participants (n = 66) read brief descriptions of men they did not know and estimated the likelihood that they were
HIV
-infected. Each description highlighted one of 6 characteristics: physical attractiveness, intelligence/education level, healthy appearance and lifestyle, personality, a combination of the preceding, and wealth. Three versions of each sketch were used; they depicted the man in positive, neutral, and negative terms respectively. There were significant differences in the ratings for the 3 versions in the case of every characteristic except wealth. In general, the negative version elicited higher ratings (corresponding to a greater likelihood that the man was
HIV
-positive) than either the positive or neutral versions; in the case of physical attractiveness, the positive version elicited higher ratings than the neutral version. Results are discussed in relation to earlier findings regarding gay men's inferences during sexual encounters, of antibody status from perceptible characteristic; to possible differences between AIDS-related thinking during sexual encounters and in the
cold
light of day; and to educational techniques that might be used to counter inferences of this type.
