In order to study the localization of Lentil lectin (
LCH
)-binding glycoresidues in glomeruli from patients with a variety of glomerulopathies, and to elucidate the relationship between
LCH
-binding sugars and the components of the extracellular matrix, laminin and type IV collagen, investigations of formalin-fixed, paraffin-embedded kidney tissues digested with trypsin were carried out by the direct and indirect immunofluorescence microscopy techniques. The glomerular basement membrane (GBM) and the mesangium reacted well with
LCH
, whereas areas with sclerotic lesions exhibited a decreased reactivity. The pattern of
LCH
binding to the GBM in various glomerulopathies was similar to that of laminin but different from that of type IV collagen. The pattern of localization of
LCH
-reacting sites and of laminin in the GBM included the double linear lines in diabetic nephropathy, inner linear line with outer projections (spikes) in membranous nephropathy, and reduplicated basement membrane in membranoproliferative
glomerulonephritis
. The results obtained by enzyme-linked immunoadsorbent assay showed that
LCH
had a stronger reactivity for laminin than for type IV collagen or fibronectin. These findings suggest that
LCH
is more reactive with laminin than with other components of the glomerular extracellular matrix.

Although electron microscopy no longer enjoys the important role in the diagnosis of interstitial lung diseases that it had in the 1960s and 1970s, it remains an important adjunct in the differential diagnosis of certain pulmonary diseases. Examples include various manifestations of systemic lupus erythematous pneumonitis, in which the presence of tubuloreticular structures and electron-dense deposits are useful for diagnosis; immotile cilia disorders, in which qualitative and now quantitative studies of the cilia of respiratory epithelial cells can help to establish the diagnosis; infections by viruses and other subcellular microorganisms as shown by the role played by electron microscopy in the initial diagnosis of the Hantavirus pulmonary syndrome; pneumoconioses, in which, in conjunction with elemental analysis probes, scanning electron microscopy is of critical importance in establishing the presence of offending foreign compounds in lung tissue or fluids; pulmonary fibrilloses, such as amyloidosis, light chain disease, and fibrillary
glomerulonephritis
, affecting the lung; and cases of alveolar proteinosis or
Langerhans cell granulomatosis
diagnosed from fluids such as bronchoalveolar lavages or small tissue samples. As important, electron microscopy remains of enormous usefulness in the study of early structural events leading to the pathogenesis of diseases. For example, recent uses of the technique have focused on the alveolar-capillary wall damage induced by alveolitis in hypersensitivity pneumonitis and sarcoidosis. In summary, electron microscopy remains a useful method in the study and diagnosis of some interstitial lung diseases, but because of its expense it is incumbent on the clinician to use good judgment in the selection of cases and diseases for study by this method.

Langerhans' cell histiocytosis

LCH
) is a clonal disorder of proliferating histiocytes, which can affect various organs. The clinical picture ranges from localized to disseminated disease with multiple organ involvement and severe organ dysfunction. Renal involvement is uncommon and the association with
glomerulonephritis
is unusual. We report here a case of nephrotic syndrome caused by membranoproliferative
glomerulonephritis
(MPGN) in a 20-year-old woman, which revealed a disseminated form of
LCH
. Surgical removal of the
LCH
process and high doses of corticosteroids led to the remission of the basic disease and the renal involvement. The occurence of membranoproliferative
glomerulonephritis
in the course of
LCH
might be mediated by circulating immune complexes, composed at least in part of antigens or neoantigenes which may cause the abnormal proliferation of the Langerhans' cells.
