Although Streptococcus equinus has been isolated from the human bowel in an appreciable percentage of the adult general population, it has only rarely been described as a human pathogen. Our report describes the occurrence of S. equinus endocarditis in a patient who had no history of pre-existing heart disease, but who showed evidence of a late-stage pulmonary histiocytosis X. Endocarditis resolved promptly after antibiotic treatment, but required aortic valve substitution. Abnormalities of the immune system that have been demonstrated in patients with histiocytosis X could explain the occurrence of endocarditis in this patient.

Bacteraemia and endocarditis are the most frequently reported clinical infections due to Abiotrophia defectiva species. This species has been rarely implicated in infections in neutropenic patients. We report a rare case of long-term venous catheter-related infection caused by A. defectiva that occurred in a febrile child who had neutropenia and Langerhans' cell histiocytosis.

We report the case of a 2-year-old boy with severe Langerhans cell histiocytosis who had tricuspid endocarditis caused by Staphylococcus lugdunensis and required surgery despite appropriate antimicrobial therapy. Through this case and literature review of endocarditis caused by S. lugdunensis in children, we highlight pitfalls and mistakes to be avoided in the management of this rare but serious infection.
