We describe a patient with a history of gastric bypass and chronic alcoholism suffering from a rare Peterson's hernia resulting from her surgery. There are a few case reports in which afferent loop obstruction was misdiagnosed as pancreatic pseudocyst after Billroth II gastrectomy. Ours is the first in which Peterson's hernia was initially misdiagnosed as a pancreatic pseudocyst in a gastric bypass patient.

Ileoscopy with mucosal biopsy is fundamental in the management and surveillance of inflammatory bowel disease patients and intestinal transplant recipients. There is a paucity of data describing the risks of ileoscopy in the presence of a prolapsed stoma. Parastomal hernias are frequently associated with prolapsed stomas. We report the first case of perforation during ileoscopy in the setting of a prolapsed stoma and unrecognized parastomal hernia. Recognition of parastomal hernia associated with stoma prolapse is of paramount importance in patients undergoing ileoscopy as it may increase the risk of perforation.

We report a unique case of a 79-year-old woman with metastatic rectal cancer who developed bowel obstruction following endoscopic cryotherapy with liquid nitrogen for palliation of bleeding in the rectum. She developed abdominal distention and pain following the procedure. Computed tomography of the abdomen revealed a paraumbilical hernia containing a segment of transverse colon resulting in partial bowel obstruction. It appears that the recurrent freeze-thaw cycles with poor decompression of the colon despite active venting suction during cryotherapy may have resulted in bowel distention and collapse, causing conformational changes resulting in partial bowel obstruction due to a paraumbilical hernia.

A gastrostomy tube passes from a patient's stomach through the abdominal wall and allows for direct enteral access. An alternative to an open gastrostomy, a percutaneous endoscopic gastrostomy (PEG) is typically favored because it is a less invasive option. However, a thorough understanding of the anatomy involved is paramount to prevent complications, regardless of what technique is chosen for placement. We present a case of an incisional hernia, which developed after the removal of a PEG tube placed through the midline of an abdominal wall, which is both a rare and avoidable complication. Placement of a PEG tube lateral to midline and avoiding the linea alba, an inherent weak point, will decrease the incidence of incisional hernias.

A 75-year-old man presented to our facility with a 5-day history of hematemesis. He reported a left inguinoscrotal hernia that had been present since 1990. Physical examination demonstrated an incarcerated inguinoscrotal hernia. Abdominal computed tomography revealed the stomach, small, and large bowel in the hernia. Esophagogastroduodenoscopy revealed food and brownish liquid in the stomach. Neither the antrum nor the pylorus could be identified during the esophagogastroduodenoscopy, consistent with an incarcerated portion of the stomach. Blood was not seen in the examined portion of the gastrointestinal tract. He was emergently treated with surgical intervention.
