The authors report 4 recent cases of abdominal intercostal hernia and in the light of other cases in the literature, discuss the clinical presentation of this curious and rare disease. Presenting all the characteritics of an uncomplicated hernia, abdominal intercostal hernia is an easy diagnosis on a single clinical examination. Further respiratory and digestive investigations are however essential before undertaking a surgical cure of this hernia at the limit of the thorax and abdomen. Treatment is purely surgical. Instead of the classical treatment by the direct approach, where the risk of relapse is important, one should prefer the transperitoneal approach with use of an inert supple prosthesis. This method gave excellent results with a follow-up of from 1 to 3 years.

The authors report the cure of six recent cases of strangulated or impacted diaphragmatic hernia: 3 post-traumatic, 2 hiatal, one congenital. Strangulation or simple impaction are responsible for ischemic phenomena with necrosis and intrathoracic visceral perforation. Intensive care for respiratory, cardio-vascular and metabolic complications, including gastric aspiration, has transformed the prognosis which was recently so poor in this condition. The thoracic approach is advised for the usual reasons and owing to frequent intrathoracic adhesions. It is also advised for ease of treatment of the strangulated viscus and local septic complications.

The discal hernia are unfrequent in dorsal localization and neurological appearances are deceptive. We report a case with amyotrophic and fasciculations developing a progressive spinal cord amyotrophy aspect. The complementary investigations (gaz myelography and spinal angiography) show the discal hernia in T11-T12 which was operated successfully. The vascular factor role is discussed about semiologic and pathogenic view.

This is a case report of a successful repair of congenital diaphragmatic hernia in a 48 years old male that contained the transverse colon which was strangulated and ruptured in the left thoracic cavity. It was approached in two stages. First through a laparotomy the proximal part of the transverse colon was divided. The side going to the hernial sac was sutured and the proximal stump was anastomosed to the descending colon. In a second stage, two days later, through a felt thoracotomy the strangulated and ruptured colon was resected and the distal stump of the transverse colon was sutured and the hernia repaired.

A normal diaphragm at birth cannot eliminate a right-sided diaphragmatic hernia, as the liver comes between thorax and abdomen. Diagnostic problems, pathogenesis and treatment are seen about two cases.

The authors report the documented observation of a 59 year old woman admitted to hospital for recent pains in the left flank caused by acute hydronephrosis. Surgical exploration revealed a surprising loss of continuity in the ureter resulting from an ischemic occlusion of the upper ureter. The occlusion was in turn due to compression from a hernia of the renal hilum. The authors conclude with a brief physiopathologic study.
