During the 12 year period, 1978-1989, 555 operations were performed at the Department of Clinica Chirurgica III of the University of Bologna, on 530 patients greater than 80 years old at the time of surgery. These were the indications for surgical procedure in these patients: malignant neoplasm of digestive system (243), other diseases of the digestive system (111), abdominal wall hernia (74), biliary disease (77), miscellaneous (50). The purpose of this paper is to define the role of surgery in patients over 80 years old. An acute complication required an emergency operation in 295 cases (53%). In all the other cases (260, 47%) an elective operation was performed. There were 63 deaths with an operative mortality rate of 11%. Among the patients who underwent elective surgery there were 11 deaths (4%); in the emergency group there were 52 deaths (17.6%). These rates were found to be statistically significant. The overall morbidity rate was 37.6% in the elective group and 49% in the emergency group. Also these rates were found to have a statistic significance. Then we have compared morbidity and operative mortality of over-eighty years old patients with the ones of two younger groups of patients: I group (age greater than 80 years), II group (age 65-80 years), III group (age less than 65 years). Patients of the three groups had undergone identical surgical operations for the same pathology. The operative mortality was 11% in the I group, 5.2% in the II group and 1.4% in the III group. The morbidity was 46%, 30% and 17% respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Pyelocystography on a 61-year-old man with a ureteral stone incidentally revealed the inguinal bladder hernia. He had no complaint of urinary disturbances. During the operation for the hernia, we could not find the bladder without transurethral saline instillation; only the intestine covered with peritoneum was in sight. Utility of preoperative cystography on the cases of inguinal hernia is discussed.

Forty-two patients of typhoid perforation of the gut have been studied. All the patients were subjected to surgery after resuscitation. Simple closure (with or without serosal patch) and temporary ileostomy were the commonly performed surgical procedures. The overall mortality was 14.2%. Postoperative morbidity included wound dehiscence, fistula, encephalopathy, septicaemia, intra-abdominal abscesses and incisional hernia. Faecal fistula formed in 5 out of 42 cases. Ileostomy was found to be the best procedure with regard to morbidity and mortality.

Presented are 3 cases of tumors affecting the papilla of the optic nerve. On the basis of the ophthalmoscopic picture, of fluorescein angiography and of clinical symptoms the authors identified a capillary haemangioma, a pigmentary naevus and a meningeal hernia. In all the cases observation did not detect any increase in the changes.

In 1948-1990 the authors operated 112 patients with paraoesophageal and mixed hiatus hernias. 100 patients operated in 1988 were followed up on a long-term basis. Gastroesophageal reflux was recorded before operation in 52%, haemorrhage in 18%, an ulcer in the neck in 7% an incarceration and obstruction in 9%. Before the onset of the seventies hiatorrhaphy and gastropexy were performed. During check-up examinations in these patients relapses were recorded in 47%, reflux in 43%, subjective complaints in 26%. This made the authors change the surgical procedure and in subsequent years they performed hiatorrhaphy, fundoplication, fundophrenopexy and anterior gastropexy. The incidence of relapses of hernia during check-up examinations declined to 21%, of gastroesophageal reflux to 9% and subjective complaints to 10%. The follow-up period varied between 1 and 19 years. The surgical lethality was 2%.

Spigelian and epigastric hernias can cause diagnostic problems as they often present in elderly, obese patients with few specific symptoms or signs. Early diagnosis is important as both groups of hernia have a high rate of bowel strangulation. The combined value of ultrasound and tangential radiographs in the diagnosis of these hernias is presented along with a review of the literature.

A case of right-sided congenital diaphragmatic hernia was detected at 33 weeks of gestation. Fetal echocardiography revealed the presence of an intrapericardial mass (3.5 x 3 cm) localized at the right of the heart and surrounded by a massive pericardial effusion. This mass had the same echogenicity as the liver, with which it shared vascular channels. The diagnosis of right diaphragmatic hernia with protrusion of hepatic tissue into the pericardial sac and secondary pericardial effusion was made and confirmed after birth. In utero diagnosis of this anomaly enabled correct assessment of perinatal risk, and optimal fetal and infant management.

A 68-year-old man was discovered to have a large peritoneopericardial hernia when operated on for coronary artery bypass. Such hernias are very unusual. The pathogenesis, clinical presentation, and management of this problem are discussed.

Initial survival rates for the first 45 North Carolina ECMO neonates in the study group are somewhat better than national figures. Developmental functioning as assessed at one year of age is comparable to that reported in other studies. A congenital diaphragmatic hernia diagnosis was significantly related to poorer outcome. Measures of family functioning approximate those reported for families of non-ECMO neonates treated in intensive care. Support provided by ECMO staff was rated at a notably high level. With nearly three thousand survivors and multiple numbers of ECMO centers now operating in this country, ECMO has made the transition from an experimental treatment to an established procedure for treatment of neonates in pulmonary failure. A few beginning attempts to utilize ECMO with older patients are being made. Neonatologists are now directing efforts toward refining techniques and selection criteria. Further evaluation of longer-term effects of this type of treatment is essential. Effective protocols for follow-up care need to be designed, particularly for those at greatest risk for developmental disability.

Over a 16-year period, 33 newborn infants developed necrotizing enterocolitis (NEC) following some form of operation (accounting for 19 per cent of all neonates with NEC), including repair of myelomeningocele, intestinal atresia, gastroschisis and congenital diaphragmatic hernia. The neonates with postoperative NEC had a median birth-weight of 3.05 (range 1.0-4.4) kg, and median gestational age of 40 (range 27-41) weeks. Twenty patients were treated non-operatively. The remaining 13 required operation for complications of NEC. Twenty-three patients are alive and have been followed for a median of 66 (range 3-168) months. If a neonate deteriorates following any operation NEC must be included in the differential diagnosis.
