Six cases of spigelian hernia are reported, including three unusual ones: (1) the presence of an acutely inflamed appendix in the hernial sac; (2) a patient with multiple associated hernias; and (3) a woman with a strangulated spigelian hernia occurring with endometrial carcinoma. The anatomy, etiology, clinical features and management of these hernias are discussed.

A case of hernia reduction en masse is reviewed. Discussion of the pathogenesis, diagnosis, and operative approach suggests that the use of taxis in the reduction of incarcerated hernia should be tempered by awareness of potential complications including mass reduction. Diagnosis is by history with the persistence of intestinal obstruction after reduction is a key feature. A preperitoneal approach is recommended for facility of reduction, resection, and anatomic hernia repair.

Herniation into the lesser omental sac through the foramen of Winslow is a rare cause of intestinal obstruction. A successfully managed case is presented. The clinical history and physical findings are reviewed, and radiographic findings are discussed. The proper interpretation of these findings should suggest the diagnosis. Treatment is prompt surgical correction, and various technical maneuvers that may be necessary are outlined.

125 cases of hernia, cholelithiasis, peptic and gastric ulcer, carcinoma of the stomach of stages III and IV were examined before operation and in the postoperative period. The peculiarity of the studied indices of energy metabolism was established in each group of patients. The authors believe, that the difinition of ATP level and of the erythrocytes energy charge can be used as one of criteria of patients' condition and of medicamental correction of metabolism.

A case of hernia through the foramen of Winslow diagnosed preoperatively is reported. Although the preoperative diagnosis is difficult to make because of the nonspecific symptoms presented and the rarity of the disease, careful evaluation of the plain abdominal X-ray and barium enema study gave the clue in the present case. Review of the literature including those from Japan revealed 115 cases up to 1976, of which diagnosis was established preoperatively in 11 cases.

We have previously reported the results of extended esophageal myotomy and Belsey hiatal hernia repair in 21 patients. Reflux was considered to be a late complication of this operation, and gastroplasty has subsequently been added. Thirty-four patients have now been surgically treated, 17 with myotomy and Belsey repair and 17 with myotomy, gastroplasty, and Belsey repair. Eight of the 17 with Belsey repair developed clinical and roentgenographic signs of reflux 6 to 27 months following operation without evidence of hernia recurrence; 5 of the 8 patients have required further operation, with the addition of gastroplasty for reflux control. Seventeen patients were treated primarily by extended myotomy, gastroplasty, and Belsey repair. None of the patients who underwent gastroplasty have reflux symptoms, and only 1 shows a trace of reflux radiologically.

Differential diagnosis respiratory insufficiency in children is great problem for pediatricians and a radiologists. The mortality rate for respiratory insufficiency especially in the newborn may be as high as 50 per cent. The etiology of respiratory insufficiency may be of different origin. Considering location one can distinguish between pulmonary and extrapulmonary causes, as congenital oesophageal stresia, diaphragmatic hernia, diaphragmatic relaxation, and tumours of the neck, mediastinum and thoracic wall. Roentgen examinations are important to diagnose of extrapulmonary causes for respiratory insufficiency.

We present a case of rupture of the right diaphragm, due to an automobile accident, discovered after a period of eight years and presenting a clinical picture of pleuropneumonia. The radiologic appearances of traumatic hernia of the diaphragm are more difficult to appreciate on the right side because of the presence of the liver and delayed symptoms. This may be dangerous or sometimes fatal.

The clinical impression of an allergist that early surgery increases the risk for developing asthma or hayfever was followed up by three studies. First, 115 children with pyloric stenosis were followed up and showed above-average prevalence figures. Second, 47 boys with hernia repairs were followed with similar results. Thirdly, 202 children reporting asthma or hay fever were studied for early hospitalizations. They reported more hospitalizations under the age of 2 than did a control group.

An unusual case of a perforated Richter's hernia in which is presented with abscess cavity contained a chicken bone. Richter's gernia is difficult to diagnose before strangulation occurs. The surgeon must be prepared to follow small bowel resection with a second operation for definitive repair of the hernia.
