using a long-lasting local anesthetic agent. An ilioinguinal, iliohypogastric, and twelfth intercostal nerve block was carried out initially, followed by regional infiltration of the agent, using a technic first described by Ponka [8] with several modifications. This technic can be employed suffessfully in the majority of groin
hernia
repairs. It requires careful attention to detail in the administration of preoperative sedation and analgesia and the use of sharp dissection only and greater gentleness in the handling of tissue. We have observed a significant reduction in postoperative discomfort and the virtual elimination of urinary retention, urinary sepsis, atelectasis, and phlebitis in these cases. All patients are fully ambulatory, without assistance immediately after surgery and the majority are discharged the same day or the following morning. was achieved by the administration of 6% chloral hydrate followed by 5% thiopentone sodium. A postxiphoid approach and the use of a continuous lock stitch suture were preferred to repair the vent in the diaphragm. Pre- and post-operative use of hydrocortisones and fluids and sufficient tissue oxygenation by controlled positive pressure respiration are believed to be keys to the success of the treatment adopted.

This study was undertaken to evaluate our experience with umbilical colostomy. There were 101 cases available for review. Four patients had major complications that necessitated reoperation, an incidence of 3.9%. One patient was operated on for necrosis of the stoma, one for retraction, and two for periostomal evisceration of omentum and small bowel. Three patients had minor strictures requiring digital dilatation, and one needed minor revision under local

. No patient had a peristomal

or prolapse, making this a distinctly better colostomy than the conventional left-lower-quadrant colostomy. The ease and comfort in the care of this colostomy were evident during follow-up visits. We feel that this procedure has all the advantages of a conventional matured colostomy and has extra advantages of easy accessibility and absence of peristomal hernias and prolapse. For any elderly patient who needs a permanent colostomy, umbilical location of the stoma offers distinct advantages.

A recently described injection formulation for diazepam, consisting of an oil emulsion where the drug is dissolved in the oil phase, has been found to give a lessened degree of side reactions than commercially available preparations. This emulsion formulation was compared to Valium inj. as premedication in patients undergoing operation for

. The effects, measured as the degree of drowsiness according to an arbitrary scale, were found to be equal for the two formulations. Neither was there any difference in blood concentrations in the two application groups, respectively, between the preparations used. The propylene glycol content of Valium inj. is said to cause a delayed release of the drug from the injection site after i.m. application, and this is briefly discussed, as well as the possibility of administering the diazepam emulsion continuously, diluted with Intralipid, without the risk of precipitation.

Triorchidism, a rare anatomical phenomenon has been reported in 24 cases and in only 6 has spermatogenesis occurred. One of these 6 is a 45 year old man who had been bilaterly vasectomized without semen analysis following surgery. The patient's wife, complaining of 4 months amenorrhea, was thought to be pregnant. Examination of the patient revealed 2 lumps detected in the right hemi-scrotum which felt like small testes and did not permit transillumination. The left testis was normal and laboratory tests including excretory urography were normal. Semen analysis revealed normal spermatogenesis. At operation under local
anesthesia
, 2 vasa deferentia were detected in the right scrotum one of which had been partly resected. At no point did the 2 structures communicate. Resection of the supernumerary vas deferens was completed and biopsies of both right hemiscrotal testes proved normal. Moreover, the testis with intact vas deferens showed normal spermatogenesis. In the left scrotum a second resection was performed on the upper end because it had been only partly resected. Histological examination of the resected vasa deferentia confirmed triorchidism with normal spermatogenesis. This condition may be associated with other congenital abnormalities such as anomalous union between the testis and its epididymis, absence of a rete testis and

and hydrocele. Urogenital examinations before vasectomy and post vasectomy semen analyses should be done to prevent similar occurrences.
