The authors describe the anaesthetic procedure for a
strangulated hernia
repair needing resection and anastomosis of the small bowel in an adult patient. This procedure was performed with an ilio-inguinal/ilio-hypogastric nerve block according to a paediatrical simplified technique with a single puncture. For this patient who had relative contraindications for central blocks, this regional technique allowed to avoid general anaesthesia with its gastric aspiration and predictible difficult intubation risks. This block associated with a very light sedation was sufficient for all the surgical procedure, and postoperative analgesia was efficient over 3 hours. This simplified nerve block, better than the conventional approach for the clinical practice, represents a recommended alternative for
hernia
repair in emergency for high risk patients who could have a general anaesthesia or a central block.

The aim of this study was to determine the outcome for patients treated for strangulated inguinal hernia with intestinal resection for gangrenous. Between May 1997 and November 1998, 124 patients were admitted to our hospital for acute intestinal obstruction; 34 of them were treated for strangulated inguinal hernia and 17 underwent an intestinal resection. The outcome for the 34 patients with strangulated hernias were analysed retrospectively. Strangulated inguinal hernia occurred in young patients (80 per cent of our patients were aged under 45 years) and strangulation had evolved over an average of 2.5 days. The clinical picture was simple
strangulated hernia
in 10 cases, with intestinal obstruction syndrome in 15 cases, peritonitis in 3 cases, phlegmonous
hernia
in 4 cases, and fistulae in one case. Fifty per cent of patients had intestinal resection with poor means of resuscitation. This resection concerned 80 per cent of patients with strangulation lasting over 72 hours. Surgical repair of
hernia
was performed in 24 cases at the same time. There were numerous complications, notably: wound sepsis (16 cases), post-operative peritonitis (3 cases) and multiple system failure (12 cases). The mortality rate was 40 per cent and concerned 86 per cent of patients with small bowel necrosis and 89 per cent of those admitted after 96 hours of strangulation.

Several tension-free femoral

repair techniques are currently available with different approaches and prosthetic devices, all of which, however, largely yielding excellent results. The aim of this report was to describe the technical aspects of femoral
hernia
repair via an infrainguinal approach using the Bard Mesh Dart Plug, and to evaluate the short- and medium-term results in our experience. From May 2000 to December 2003, 25 patients (F/M ratio 2.1:1; mean age 66.4 years) underwent femoral
hernia
repair with the Bard Mesh Dart Plug. Fourteen patients (56%) were operated on as emergency cases for irreducible hernias and in 3 cases a bowel resection under general anaesthesia was necessary. The other 11 patients underwent surgery on an outpatient day surgery basis under local or spinal anaesthesia. Topical antibiotic prophylaxis was administered in all cases, while short-term prophylaxis with ceftazidime was limited to emergency patients. The main phases of the operation are described. The mean operative time was 40 minutes and the patients were mostly discharged within 2 hours of surgical treatment. No general or local intraoperative complications were registered. Postoperative mortality occurred in a single case (a 98-year-old woman, ASA IV, affected by
strangulated hernia
). Postoperative morbidity included 4 seromas and 1 haematoma. No recurrences were observed over a mean follow-up period of 24 months (range: 5-46 months). The femoral

repair with the Bard Mesh Dart Plug is a simple, safe, fast and effective procedure which can often be carried out in the day surgery setting. It can therefore be regarded as a valid alternative to other tension-free techniques.

We report a case of 28-year-old woman presenting with small bowel obstruction. She had neither prior surgery nor delivery. An upright abdominal radiograph revealed several air-fluid levels in the small bowel in the midabdomen and the pelvic cavity. Computed tomography demonstrated a dilated small bowel loop in the Douglas's fossa,but no definite diagnosis could be made. Supportive therapy with draining the intestinal fluid by a long intestinal tube did not result in improvement,which suggested the possibility of a
strangulated hernia
. Exploratory laparotomy revealed mobile cecum and a 20-cm length of the ileum herniated into a defect of the right broad ligament. As a gangrenous change was recognized in the incarcerated bowel, its resection was carried out,followed by end-to-end anastomosis and closure of the defects of the broad ligament. is a rare cause of obstruction. Among internal hernias, those through defects of the broad ligament are extremely rare. Defects of the broad ligament can be either congenital or secondary to surgery, pelvic inflammatory disease,and delivery trauma. In conclusion, we emphasize that

of the broad ligament should be added to the list of differential diagnosis for female patients presenting with an intestinal obstruction. Early diagnosis and surgical repair reduce morbidity and mortality from strangulation.
