The number of patients with end-stage renal disease who benefit from chronic dialysis is steadily increasing. This study was designed to assess abdominal surgery in chronic hemodialysis (CHD) patients. A 7-year retrospective study was conducted including all the patients on CHD who underwent abdominal surgery in our unit. These patients were separated into an elective and an emergency surgery group. Forty-three patients underwent surgery. In the elective surgery group (18 patients), the most common diseases were colorectal cancer, symptomatic gallbladder stones, and
hernia
. There was no death related to surgery in this group, and only one patient developed a complication (5%). In the emergency surgery group (25 patients), the most common diseases were mesenteric ischemia and gastrointestinal bleeding from
angiodysplasia
. Complications occurred in 10 patients (total morbidity rate, 40%), and 6 of them died (mortality rate, 24%). Gastrointestinal elective surgery in patients on CHD can be performed with low morbidity and mortality rates. The emergency group was differentiated by the high prevalence of bleeding from
angiodysplasia
and mesenteric infarction, as well as its high surgical mortality rate.

An 85-yr-old male presented with complaints of a 40-lb weight loss and a dull left upper quadrant abdominal pain. He also complained of decreased appetite, generalized weakness, generally not feeling well, and a dull left upper quadrant abdominal pain that was not relieved by food. He had a ventral and a left-sided inguinal hernia. Laboratory investigations revealed iron deficiency anemia, the cause of which was not apparent despite extensive investigation including computerized tomographic scans, esophagogastroduodenoscopy, and small-bowel follow-through examination. , malignancy, and/or mesenteric ischemia revealed an incarcerated
hernia
, and the histopathological examination of the surgical specimen revealed high-grade angiosarcoma. The tumor showed strong positivity for vimentin and CD31 and a focal positivity for Factor VIII and CD34. At that time he was found to have hepatic metastases. He was started on thalidomide as an experimental measure with no change in the performance status and increasing evidence of necrosis in the metastatic lesion.

angiodysplasia
. Gastric angiodysplasia is recognized as an important cause of gastrointestinal bleeding. Angiodysplastic lesion confined to the gastric antrum was first described in 1953 and named gastric antral vascular ectasia (GAVE). The condition is characterized as submucosal capillary dilatation and fibromuscular hyperplasia. The typical finding of GAVE is the so-called watermelon stomach, attributable to vasodilatation. In case 1, a 69-year-old man was introduced continuous ambulatory peritoneal dialysis (CAPD) in July 1997 because of chronic renal failure due to nephrosclerosis. He was hospitalized for severe anemia in December 1997. Gastrointestinal fiberscopy (GIF) showed oozing in the antrum, and gastritis and esophagitis with sliding
hernia
. Famotidine was started and recombinant human erythropoietin (rHuEPO) was used for anemia. However, the severe anemia did not improve. The patient was hospitalized again for severe anemia and hematemesis. Another GIF showed typical watermelon stomach, which corresponded with GAVE. An APC was performed without complications. Three months later, the anemia was improved, and the dose of rHuEPO was reduced. In case 2, a 57-year-old woman was introduced to hemodialysis in 1998 for uremia due to nephrosclerosis. In October 2000, she was hospitalized for rHuEPO-resistant anemia. A GIF showed oozing in the antrum with diffuse vasodilation in the antrum; GAVE was diagnosed. Inguinal hernia is a common condition that usually presents with swelling and mild groin discomfort. Complications include bowel obstruction and strangulation. We report a case of a 50 year-old man who developed lower gastrointestinal bleeding secondary to an indirect inguinal hernia. Colonoscopy showed an ileocecal valve polyp, florid inflammation of cecum and ascending colon and ulcerations of the terminal ileum. Histology showed nonspecific colitis and

of the polyp. Surgical correction of the
hernia
led to the resolution of the endoscopic changes. The trauma associated with intermittent herniation of small bowel probably led to ischemia, resulting in the observed changes.
