is a rare and often overlooked cause of small bowel obstruction. We report a case of internal

with an orifice composed of epiploic fat, successfully diagnosed and treated by single-incision laparoscopic surgery. This is the second report of this type of internal
hernia
and the first reported case addressed laparoscopically. Although the use of laparoscopy for the treatment of small bowel obstruction is not firmly established today, it may be beneficial for both its diagnostic value and as a less invasive treatment.

with consequent incarceration or strangulation of the small intestine. An 18-year-old woman was brought to our emergency department with sudden onset lower abdominal pain. She had no remarkable past medical history and took no medications. An emergency laparotomy was performed. On exploring the abdominal cavity, the mesenteric defect was detected in the jejunal region, 3 cm in size. Loops of small intestine had gone through the defect three times in a complex manner and strangulated. Gangrenous intestine was resected and a primary anastomosis was performed. An adult congenital mesenteric defect is rare; however, it should be considered as one of the differential diagnoses in a relatively young patient with bowel obstruction without external

and performed emergency surgery. Laparotomy revealed chyle-like ascites and extensive small intestine with poor color. We recognized that about 20 cm of jejunum from the ligament of Treitz was strangulated behind the pedicle of the jejunum lifted during laparoscopic-assisted proximal gastrectomy. We relieved the strangulation, whereupon the color of the strangulated intestine was restored. Therefore, we did not perform intestinal resection and reconstruction. , total complications, and recurrent IH were meta-analyzed according to the orientation of the alimentary limb. Fourteen studies (13,660 patients) were included. Antecolic orientation resulted associated with a lower incidence of IH and obstructive symptoms, while the route of the alimentary limb did not show to affect the other outcomes. Antecolic orientation decreases the IH. However, the role and the technique of the closure of mesenteric defects cannot be stated due to the lack of adequate data to date. Well-designed randomized controlled trials (RCTs) are needed.
