A case of complete heart block occurred after induction of halothane anesthesia in a previously healthy child. The patient underwent repair of an umbilical hernia under general anesthesia. After a standard halothane, nitrous oxide, and oxygen mask induction, complete heart block was noted on the electrocardiographic monitor. Atropine and 100% oxygen were administered, and sinus tachycardia resulted. With the immediate stabilization of the patient's condition, the surgical team agreed to proceed with the case. After deepening of the level of anesthesia, first with halothane and then with desflurane and easy intubation of the trachea, complete heart block again was noted. Oxygen was administered at 100%, sinus tachycardia resumed, the case was canceled, and the patient emerged from anesthesia without further incident. The patient had an uneventful recovery and was discharged to home.

The studied population (1000 children and young workers and 250 controls) were interviewed for health complaints and subjected to extensive medical investigations. Health problems are more encountered among the workers than in the controls, which has been attributed to the impact of work on health and to the low socioeconomic background that compels the children to work, which may constitute a potential health problem from the start. The significant health problems include: a) Respiratory system complaints, attacks of cough, chronic bronchitis, bronchial asthma and reduction in FEV1.0/FVC; b) cardiovascular abnormalities including palpitations, sinus tachycardia, anemia, vertigo and syncope; c) gastrointestinal abnormalities including dyspepsia and parasitic infestations; d) neuropsychiatric complaints; and e) other health problems including urinary tract infections, backache, visual impairment, repair of hernia and nocturnal enuresis. The study has been concluded by recommending the use of primary health care approach to child labour, emphasizing the importance of pre-employment and periodical medical examinations for protecting this vulnerable group from work hazards, and expanding the efforts of family planning to reduce family size.

A 15-month-old boy presented in shock with a supraventricular tachycardia following a 12-h history of worsening abdominal pain and vomiting. The supraventricular tachycardia reverted to sinus tachycardia with fluid resuscitation and adenosine. He was noted to have a distended and firm abdomen. A presumptive diagnosis of intestinal ischaemia was subsequently confirmed at laparotomy when an internal hernia with a distal small bowel volvulus and necrosis was found. Intestinal ischaemia presenting with a life-threatening cardiac dysrhythmia in a child appears not to have been reported previously.
