The site of obstruction was evaluated by seminal vesiculography in 24 cases of azoospermia with normal testicular biopsies. In 21 cases, obstruction was located at the level of the tail of the epididymis; in 2 cases ejaculatory duct was obstructed; and in 1 case the injected dye was arrested at the level of internal inguinal ring, at the site of previous hernia operation. Vesiculogram in 24 obstructed cases and 4 cases of chronic seminal vesiculities without obstruction showed one normal picture, four with catarrhal inflammation, and the rest with chronic interstitial vesiculities. Radiological study of a patient complaining of aspermia showed multiple congenital anomalies. The high incidence of chronic vesiculities and postinflammatory obstruction is attributed to underlying bilharziasis.

Five male patients mean age 31.8 years with lower ureteric obstruction from urinary tract schistosomiasis have been treated by bilateral uretero-ileocystoplasty. All the 5 patients had bilateral hydroureters and hydronephrosts and 3 had reversible chronic renal failure whilst one patient presented with anuria and acute renal failure. Fibrosis of lower 1/4th to 1/3rd of the Ureters was present in the 5 patients. Four patients survived the operation for a mean of 3.4 years (range 1-7 years). The post-operative complications were hypernatraemic hyperchloraemic metabolic acidosis in 3 and incisional hernia in one. There was one death from septicaemia complicating infected bilateral nephrostomies performed before ureteroileocystoplasty. Ureteroileocystoplasty is recommended for replacement of damaged ureters where the extent of the damage precludes ureteroneocystostomy or where previous ureteroneocystostomy has failed.

We report an 11-year-old boy who was diagnosed with schistosomiasis based on histopathologic examination of an inguinal hernia sac specimen. The child was an immigrant from Liberia and presented to our institution with inguinal and scrotal swelling. His past medical history was remarkable for previous ipsilateral hernia sac repair in West Africa, and at the time of his recurrent hernia repair, he was noted to have a peculiar loculated fluid-filled hernia sac, which was sent for pathologic examination because of its unusual appearance. Histologic examination revealed an intense infiltrate of eosinophils and numerous granulomas with ova of Schistosoma mansoni, and this speciation was confirmed with a stool wet prep examination for ova and parasites. Review of the English-language literature uncovered no prior cases of schistosomiasis presenting as an inguinal hernia. This unusual finding in a "routine" specimen, otherwise destined for the incinerator without pathologic evaluation, reinforces the importance of surgical vigilance in recognizing unusual or atypical features in these specimens.
