Lung hernias are rare and are most commonly secondary to blunt or penetrating trauma. Few cases have been reported after video-assisted thoracoscopic surgery and only one case after video-assisted thoracoscopic surgery lobectomy. We report a case of lung hernia after video-assisted, thoracoscopic, right upper lobectomy. The hernia was demonstrated by computerized tomography and repaired by minimally invasive techniques. We believe that the combination of removal of a large lung specimen and the presence of emphysema may predispose to lung herniation after thoracoscopic lobectomy. Thoracic surgeons should be aware of this possible complication.

A 77-year-old man injured himself when he fell heavily on the left side of his chest. He had massive subcutaneous emphysema, bleeding, and left hemopneumothorax. He also fractured his seventh through tenth ribs; a fragment of the ninth rib was displaced into the thoracic cavity. The severity of the damage and the patient's pain was assessed using the Abbreviated Injury Scale 1990 (1998 update) as level 3. He was treated with conservative therapy and discharged on the 16 days after the injury. However, the following day, he had acute upper abdominal pain, his blood pressure dropped to 40 s, and he was readmitted. A chest CT showed the transverse colon was prolapsed in the thoracic cavity. The patient was diagnosed as having a delayed traumatic diapharagmatic hernia. A laparoscopic repair was performed. The rupture was classified as a IIIb-type diaphragmatic injury according to the Japanese Association for the Surgery of Trauma's classification system. It is believed that a fragment of a fractured rib that had been displaced in the thoracic cavity ruptured the diaphragm sharply. Since traumatic diapharagmatic hernia rarely occurs, it is relatively difficult to diagnose at the first examination. This condition has a high mortality rate because of the associated injuries. Surgery is the only treatment, but it should only be considered after a second examination. Herein, I report my experience with a case of delayed diaphragmatic hernia repaired by laparoscopic surgery.

Cutis laxa (CL) is a rare congenital and acquired disorder characterized by loose and redundant skin with reduced elasticity. Three types of congenital cutis laxa have been recognized. Other findings are pulmonary emphysema, bronchiectasia, hernia and diverticulosis. We describe a female neonate involved by cutis laxa syndrome and a positive family history. We focus on the radiologic findings of this case such as multiple bladder diverticulosis, GI diverticulosis and very rare accompanying hypertrophic pyloric stenosis (HPS).

Pulmonary agenesis is a rare congenital malformation of lung development defined as complete absence of lung tissues, bronchi, and pulmonary vessels; it may be uni- or bilateral. The right-sided form carries the poorest prognosis due to severity of co-existent anomalies. Its diagnostic circumstances are variables: first reported cases were diagnosed at autopsy, but early postnatal as well as fortuitous discovery have been reported. In recent years, progress in obstetrical imaging has made antenatal diagnosis possible so that fetal ultrasound and MRI allow early diagnosis and refinement by permitting the elimination of differential diagnoses (diaphragmatic hernia, cystic adenomatoid malformation of the lung, giant lobar emphysema, and situs inversus). This anomaly is compatible with normal life provided co-existent malformations are thoroughly investigated and managed in a multidisciplinary setting. We report four cases of lung agenesis two of which were diagnosed antenatally at 23rd and 30th weeks of gestation respectively. Our aim is to describe the circumstances having led to diagnosis and report both follow-up and outcome of our patients.

This retrospective case review details the use of noncrosslinked intact porcine-derived acellular dermal matrix (PADM) for incisional herniorrhaphy in patients with infected synthetic mesh. A consecutive series of adult patients underwent single-stage ventral herniorrhaphy involving removal of infected synthetic mesh and repair with PADM by a single surgeon (2009 to 2011). Comorbidities, repair procedures, postoperative complications, and hernia recurrence were noted. Of the 13 patients (mean age, 60 years; female, n = 8), seven (54%) were obese and six (46%) had chronic obstructive pulmonary disease/emphysema. Most synthetic mesh infections were polymicrobial (n = 7, 46%) or associated with Staphylococcus aureus (n = 4 [31%]). Six patients had undergone two or more previous repairs. With single-stage herniorrhaphy using PADM, primary fascial closure was achieved in 11 patients; bridged closure was required in two patients. Mean (median) duration of hospital stay was 12 (7) days and follow-up was 23 (22) months. There was one wound infection (drained surgically, PADM remained in place) and one seroma (resolved without intervention) observed during follow-up. There were two hernia recurrences, both in patients who received PADM as bridged repair. PADM yielded favorable outcomes when used for single-stage repair of complex ventral hernias in high-risk patients with infected synthetic mesh.

We report the clinical details and imaging findings of a case of perforated Littre's hernia presenting as lower abdominal wall cellulitis in a 50-year-old male. Findings included herniation of an oral contrast opacified blind-ending pouch arising from the anti-mesenteric border of the distal ileum into the right inguinal canal, with extravasation of oral contrast. There were adjacent inflammatory changes with subcutaneous emphysema extending up to the right anterior chest wall. Meckel's diverticulum is rarely diagnosed preoperatively on imaging. We highlight the importance of computed tomography in imaging Meckel's diverticulum.

Subcutaneous emphysema (SCE) mimicking diaphragmatic hernia on X-ray is hitherto unreported and this case has been presented with the aim to alert the clinician about this unusual presentation of SCE to avert undue panic and initiate appropriate evaluation and management. This case also reemphasizes the importance of a thorough clinical examination, which probably could have revealed the subcutaneous emphysema earlier.

In this study, we aim to investigate the imaging appearances of congenital cystic adenomatoid malformation (CCAM) of the lung, and to enhance the understanding of this disease. A total of 11 cases with CCAM of the lung were confirmed by surgery and pathology. Preoperative chest computed tomography (CT) scan was performed in all patients, and high resolution CT scan was performed in lesion areas of 7 cases. Our results showed that there were 3 cases involving left and right lung, 5 cases involving right lung and 3 cases involving left lung. CT scan showed 6 cases with single or multiple air-filled cavities (> 2 cm in diameter) and 5 cases with multiple honeycomb-like cysts (< 1 cm in diameter). The cysts of CCAM contained air in 9 cases and a small amount of liquid in 2 cases. The complications of CCAM included different degree of emphysema in 7 patients, mediastinal hernia in 5 cases and congenital pulmonary sequestration in 1 case. All lesions have certain space-occupying effect. In conclusion, CT manifestation of CCAM of lung has certain characteristics and can provide reliable information for diagnosis of the disease.

Congenital pulmonary malformations represent a broad spectrum of anomalies that may result in varied clinical and pathologic pictures, ranging from recurrent pulmonary infections and acute respiratory distress syndrome, which require timely drug therapy, up to large space-occupying lesions needing surgical treatment. This classification includes three distinct anatomical and pathological entities, represented by Congenital Cystic Adenomatoid Malformation, Bronchopulmonary Sequestration and Congenital Lobar Emphysema. The final result in terms of embryological and fetal development of these alterations is a Congenital Lung Hypoplasia. Since even Bronchial Atresia, Pulmonary Bronchogenic Cysts and Congenital Diaphragmatic Hernias are due to Pulmonary Hypoplasia, these diseases will be discussed in this review (1, 2).

The purpose of this case is to investigate a case of obturator hernia leading to right thigh abscess on 68-year-old woman of China. A 68-year-old Chinese woman was referred to China-Japan Friendship Hospital of Jilin University with abdominal pain, bloating, exhaust, stop defecation in 2011. She had chronic bronchitis, emphysema with a history of 20 years. This patient did not have any bad habits, such as smoking, alcohol consumption, etc. In this surgery, CT was used to diagnose the basic condition of the patient. Surgery was used for treatment of patients with diseases. In addition, this operation was performed by the china-Japan Friendship Hospital of Jilin University. The results of this case showed that the cervix of rectal right anterior wall can hit a funicular neoplasm, toughening, smooth, with tenderness, considering for the external pressure bowel loops. The inside of the right thigh showed obvious swelling, skin slightly bruising, and tenderness. Chest radiographs showed that patients had emphysema, multiple planes of fluid and air in the abdomen. Patients had been successfully operated, but she died because of severe infection.
