Over a period of 10 years, 60,197 stress and nonstress tests were done in 25,313 patients. There were 229 (0.9%) positive tests. In 47.2% of patients who had positive tests, preeclampsia and hypertension were the indications for the test, followed by suspected intrauterine growth retardation (26.6%) (table I). The cesarean section incidence was 71%. The mean age of the patients was 32 years. Nine per cent of patients were 45 years or older, 7% between 40 and 44 years, 23% between 35 and 39 years, 30% between 30 and 34 years, 20% between 25 and 29 years, 7% between 20 and 24 years and 4% younger than 20 years. There were 13 intrauterine deaths of which 9 fetuses were regarded as too small to be delivered. Of the remaining 4 patients, intrauterine death occurred in 3 patients before cesarean section could be done and in the fourth a macerated twin was unexpectantly found when a cesarean section was done for fetal distress. Routine autopsies were not done on intrauterine deaths. The only external abnormality that could be found was an umbilical hernia. Apart from this abnormality there were 9 liveborn babies with congenital abnormalities. Four of these babies died neonatally, bringing the perinatal death rate up to 50%. Four babies (1.7%) had chromosomal abnormalities. These were trisomy-21 in two patients, a trisomy-18 and a true hermaphrodite. Ages of the patients with the trisomy-21 were 46 and 49 years. Between 1970 and 1982, 13 renal transplant recipients had 17 pregnancies with follow-up at the Medical College of Georgia (MCG). Nine patients received related donor grafts and four received cadaver kidneys; 11 mothers survived their pregnancies. Preeclampsia, urinary tract infections, and preterm labor were frequent complications. complications. Perinatal complications were rare, and 12 of 13 continuing pregnancies produced surviving infants. Only one infant had congenital abnormalities (premature closure of the fontanelles and umbilical hernia). We present maternal and neonatal details, with prognostic considerations for pregnancy in such patients.

The incidence of parturition difficulties from 239 sheep and 21 does from the last seven lambing periods was recorded at a clinic for obstetrics. Without exception the does were housed under extensive conditions by hobby-breeders. The sheep also originated predominantly from hobby-breeders and in a smaller amount from professional breeders, both practising extensive housing. The incidence of manual deliveries (m.d.) in both species was lower (39.3% in sheep; 42.8% in does) than that of caesarean sections (c.s; 57.3% in sheep; 47.7% in does). In a small amount the obstetrics were solved via fetotomy. The practical proceedings concerning the different methods of therapies including analgesia, surgical approaches and postoperative treatments are described. In sheep ringwomb was the dominating reason for dystocia for m.d. (43.5%) as well as for c.s. (73.7%), followed by obstetrics due to fetal abnormal presentation and/or position or posture (25.2% m.d.; 1.1% c.s.), secondary oversized fetuses due to postmortal emphysema and edema (19.1% m.d.; 10.7% c.s.), followed by primary relative or absolute oversized fetuses (1.0% m.d.; 4.8% c.s.) and simultaneously presentation of multiple fetuses (4.2% m.d.). Other causes of dystocia remained scarce (i.e. uterine torsion, hydrallantois, abdominal or perineal hernia). There rested an amount of sheep (7.1%) with preterm s.c. because of continuous pressure and pain symptoms followed by infections or injuries of the prolapsed vagina and/or rectum, pregnancy toxemia and other reasons. The present study reviews the possible role of Se status during pregnancy regarding adverse pregnancy outcomes, with emphasis on those related to diminished antioxidant activity and increased oxidative stress. Studies have reported that Se could play an important role in adverse outcomes such as miscarriages, neural tube defects, diaphragmatic hernia, premature birth, low birth weight, pre-eclampsia, glucose intolerance and gestational diabetes. Also, low Se status has been associated with adverse outcomes among HIV-infected pregnant women and their offspring. Nevertheless, the function of Se in the aetiology of pregnancy complications is yet to be elucidated. Available evidence presents the following limitations: most study designs do not allow conclusions about causal relationships; study populations, selection of subjects, research setting, procedures for defining sample size and analytical methods are often poorly described; many studies fail to adjust for important confounding variables. In addition, population studies assessing the relationship between Se intake during pregnancy and health outcomes are scarce. Further research is still needed to clarify the role of Se status in adverse pregnancy outcomes, especially those related to augmented oxidative stress.

Over-the-counter cyclooxygenase inhibitors are used to relief fever and various types of acute pain like headache, toothache, earache, sore throat, as well as postoperative and menstrual ones. They are also major ingredients in cold and flu mixtures. Unlike well-known organ toxicological profile, their prenatal toxicity was not fully established. For a long time, acetaminophen was considered as a relatively safe antipyretic and analgesic drug during pregnancy. However, a new data indicate that it may increase the risk of cryptorchidism and asthma during childhood as well as preeclampsia, preterm birth, maternal phlebothrombosis and pulmonary embolism. Contrary to acetaminophen, non-selective cyclooxygenase inhibitors (non-steroidal anti-inflammatory drugs - NSAID; i.e., diclofenac, ibuprofen, naproxen) may induce intrauterine growth retardation, ductus arteriosus constriction with secondary persistent pulmonary hypertension, reduced fetal renal perfusion that led to oligohydramion, prolonged pregnancy as well as increase prevalence of intracranial bleeding in newborns. Furthermore, a higher risk of miscarriage, stillbirth and some congenital malformations (cardiac and diaphragmatic defects, celosomy - gastroschisis and umbilical hernia) was reported for non-selective inhibitors, in particular high doses of acetylsalicylic acid (aspirin).

Congenital hernias, frequently misdiagnosed during pregnancy, are potentially fatal and require prompt repair. A pregnant woman with medical history of repaired congenital hernia was admitted with misdiagnosis of preeclampsia. Physical examination and chest x-ray revealed a Bochdalek hernia. Transitory stabilization prompted surgeons to postpone hernia repair, but an urgent thoracotomy was required to relieve a subsequent bowel obstruction that was complicated by an intrathoracic colonic perforation. Emergent cesarean delivery was required with a good maternal and fetal outcome. A multidisciplinary team was present in the operating room. All monitoring catheters were placed in advance in the intensive care unit. During recovery, the patient experienced ventricular fibrillation, presumed to be a manifestation of takotsubo syndrome, which responded favorably to cardiopulmonary resuscitation.
