A review of the literature on the comprehensive description of depressive patients revealed prominent concern with syndromic subtypes, course of illness, and personality factors, followed by severity, concomitant physical disorders, psychosocial stressors, and adaptive functioning. The descriptive value of multiaxial approaches for depression was illustrated through the application of an extended DSM-III formulation to all 3455 depressive (bipolar depression, major depression, dysthymic disorder, and atypical depression) and 7837 nondepressive patients of all ages and sexes presenting for evaluation and care at the Psychiatric Institute of the University of Pittsburgh during a period of 53 months. Twenty-six percent of the depressive patients received an additional diagnosis in axis I, the most frequent of which were substance use disorder, anxiety disorder, and condition not attributable to a
mental disorder
. In axis II, depressive patients presented a differentially higher frequency of dependent personality disorder and the "anxious/fearful" cluster of personality disorders. In axis III, 47% of the depressive vs. 40% of the nondepressive patients had a positive diagnosis of physical illness, with a significantly higher frequency among depressive patients attained by acquired hypothyroidism, migraine, essential hypertension, unspecified abdominal
hernia
, and unspecified arthropathies. Specific stressors differentially more frequent among depressive patients were those of conjugal, parenting, and occupational types and those reflecting the impact of physical illness. Overall stressor severity was at severe, extreme, or catastrophic levels for 42% of the depressive and 31% of the nondepressive patients. The highest level of adaptive functioning in the past year was good, very good, or superior for 44% of the depressive and 29% of the nondepressive patients.(ABSTRACT TRUNCATED AT 250 WORDS)

In a North West England community of 4880 males, data were examined from 40 adult males on a general practice list who chose to be sterilized. The most popular age for vasectomy was 30-34 years; there was a sharp drop after age 40. Olny men with children were vasectomized, with average number of children per family 2.53. There were no professional men in the group but may semi skilled and skilled workers, a reflection of the industrialized area. In contrast to other reports, the results indicated that men of the lower classes are choosing to be sterilized whereas previously the professional and skilled workers represented the majority of those undergoing vasectomy. There is no evidence that vasectomy significantly effects general health. 75% of the group studied saw their general practitioner, on average, once every 3 months after the operation. 2 patients have been admitted to the hospital since their operation, 1 for arthroscopy and lateral release of the left patella and the other for
hernia
repair. There has been 1 case of marital breakdown and 2 cases of
psychological disorder
. 3 patients have suffered from recurrent low back pain which is probably related to their weight problems and occupation. Nearly 60% of the people in the study completed and returned their questionnaires. The success of the counseling technique was reinforced by the fact that 95% thought that enough information was given. The effectiveness of the operative procedure is shown by the 100% success rate of sterilization. 74% of the men required only 3 days or less off work; 23% required some treatment or advice from their general practitioner subsequent to the operation. Only 9% found the procedure more painful than expected. All the men would recommend the operation and were glad to have had it done. 40% reported better relationships with their wives.

There are reports providing scientific evidences for hypotensive, anxiolytic, neuroleptic, hypolipidemic, immunomodulatory, antioxidant, anti-inflammatory, bronchodialatory, antihyperglycemic and hepatoprotective activities of this plant. A wide range of phytochemical constituents have been isolated from this plant including sesquiterpene lactones, eudesmenolides, flavanoids and essential oil. A comprehensive account of the morphology, phytochemical constituents, ethnobotanical uses and pharmacological activities reported are included in this review for exploring the immense medicinal potential of this plant.

It is uncommon to find trauma to the flaccid penis in medical practice. We report a case of penile invagination complicated by incarceration of the herniated penis through a defect in the scrotum following manipulation of his flaccid penis. A 7-year old boy presented with his penis herniated and trapped in a defect in his scrotum following penile manipulation of three days duration. His testicles were intact and all his system was normal; there was no feature of
psychiatric disorder
in the patient, and none in his family. He was resuscitated and appropriate antibiotics given. Under general anesthesia, the
hernia
was reduced and circumcision performed. He made uneventful recovery and there were no complaints during follow-up. Conclusion: Penile invagination is an uncommon condition which requires urgent surgical intervention to avoid complications, hence the need for this case report in order to highlight the imperative for emergency intervention.
