Rat HTC
hepatoma
cells were used to characterize the biosynthesis and processing of the renal isoenzyme of the mitochondrial glutaminase. Immunoblot analysis indicated that mitochondria isolated from HTC cells contained two prominent glutaminase peptides of 68 and 65 kDa and two minor peptides of 61 and 58 kDa. When the cells were labelled with [35S]methionine, the glutaminase-specific antibodies precipitated the same four polypeptides. However, when labelled in the presence of 5 microM-carbonyl cyanide m-chlorophenylhydrazone, an uncoupler of oxidative phosphorylation, only a 72 kDa cytoplasmic precursor of the mitochondrial glutaminase was immunoprecipitated. A comparison of the peptides generated by partial proteolysis of the precursor and the fully processed peptides indicates significant structural similarity.
A 71
kDa form of the glutaminase was also observed when HTC cells were pulse-labelled for 2-6 min with [35S]methionine. Pulse-chase experiments indicate that the cytoplasmic precursor is quantitatively converted into the mature forms of the glutaminase. In addition, the observed kinetics established that the 71 kDa peptide is a true intermediate in the import of the mitochondrial glutaminase.

We report a patient with
hepatocellular carcinoma
(
HCC
) who was successfully treated with radiofrequency thermal ablation (RFA).
A 71
-year-old man was admitted to our hospital in August 1996 with recurrence of
HCC
. Partial hepatic resection had been performed in January 1993 for
HCC
that had measured 1.3 cm in segment VIII, and subsequently he had received six sessions of percutaneous ethanol injection (PEI) for treatment of recurrence. Dynamic computed tomography (CT) performed in August 1996 showed two recurrent tumors, one measuring 3.8 cm in segment VIII adjacent to the right hepatic vein, and one measuring 2.0 cm in segment V. Three sessions of percutaneous RFA were performed. After this treatment, most of the tumor in segment VIII and all the tumor in segment V showed low density on dynamic CT, and the right hepatic vein was preserved. However, a remnant of the mass appeared near the right hepatic vein 2 months after the treatment. An additional two sessions of RFA were performed. After the end of treatment, serum alpha-fetoprotein level dropped to the normal range, and no sign of recurrence has been observed until September 1998.

A 71
-year-old Japanese male with myelodysplastic syndrome progressing to overt leukaemia and
hepatocellular carcinoma
developed dyspnea and urticaria immediately after infusion of platelet concentrate (PC). He exhibited an identical reaction following blood transfusion. Serum haptoglobin was undetectable. The patient was determined to be homozygous for Hp(del) by polymerase chain reaction (PCR). Antibody to haptoglobin was detected by enzyme-linked immunosorbent assay (ELISA) and Western blot analysis. No antibodies against human leucocyte antigen (HLA) or platelet-specific antigens were detected. Washed PC and washed red blood cells were effective in preventing the transfusion-related anaphylactoid reactions.

A 71
year old patient presented with a non-ST segment elevation acute myocardial infarction. The echocardiogram showed several masses attached to the interatrial septum. Several days after admission the patient died. A postmortem examination found a large
hepatocarcinoma
with intravascular and intracardiac metastases and several myocardial infarctions of different ages. The infarctions had been caused by coronary paradoxical embolisms through a patent foramen ovale and contained neoplastic cells from the liver carcinoma, which had not been diagnosed. The cause of death was a massive pulmonary embolism.

A 71
-year-old man was diagnosed with giant
hepatocellular carcinoma
(
HCC
) and hepatitis C cirrhosis at a nearby hospital. Image diagnosis showed no other metastasis, but the tumor was very huge with daughter nodules in the bilateral lobe of the liver. He was thus treated by oral administration of UFT (300 mg/day). Two months later, the giant liver tumor had shrunk remarkably, and the daughter tumors had disappeared. Eight months later, the levels of serum AFP and PIVKA-II had also reduced remarkably. Twelve months following the first treatment, the levels of both serum AFP and PIVKA-II began increasing again, and he was referred to our hospital. CT showed 2 liver tumors, 1 of which showed viability with moderately differentiated
hepatocellular carcinoma
and the other evidencing necrosis histologically. Radio frequency ablation therapy was performed for 2 tumors by open laparotomy. It was considered that administration of UFT is a useful and safe therapy for far advanced
HCC
.

Spontaneous regression of
hepatocellular carcinoma
(
HCC
) is rare. There are few reports discussing spontaneous regression associated with serum lens culinaris agglutinin-reactive alpha fetoprotein (AFP-L3). We describe a case of
HCC
with a high level of AFP-L3, which showed a rapid increase in alpha fetoprotein (AFP) concentration after partial spontaneous regression, and which was then treated successfully.
A 71
-year-old woman suffering from chronic hepatitis C underwent surgical resection for
HCC
. Preoperative concentrations of AFP fluctuated; subsequent to a transient decrease, a rapid increase in AFP was observed. AFP-L3 concentration was extremely high. The resected tissue consisted of encapsulated moderately differentiated
HCC
, subcapsular coagulation necrosis, and chronic active hepatitis. The postoperative course was uneventful. At present, 24 months after diagnosis, no symptoms or signs of tumor recurrence or metastasis have been observed. Although the precise etiology of the spontaneous regression in this patient is not known, we speculate that spontaneous regression of tumor cells with high malignant potential may be related to arterial involvement and insufficient blood supply.

A 71
-year-old man was admitted to our hospital with abdominal pain.
Hepatocellular carcinoma
(
HCC
) had been diagnosed 2 years earlier and he had undergone 7 courses of intra-hepato-arterial chemotherapy (IHAC). We performed gastrointestinal fiberscopy and identified a massive protrusion on the lesser curvature. Abdominal contrast-enhanced computed tomography revealed multiple hepatic masses and an extrahepatic enlarged mass with invasion to the pancreas and stomach. A specimen for endoscopic biopsy revealed adenocarcinoma that stained positive for alpha-fetoprotein. Gastrointestinal bleeding resulting from direct invasion of
HCC
is unusual.

A 71
-year-old man was admitted to our institution having a
HCC
with tumor thrombus extending to the inferior vena cava. He simultaneously had a sigmoid colon cancer. S8 segmentectomy combined with a removal of tumor thrombus in the IVC and sigmoidectomy was performed. He had an uneventful postoperative course. He had a recurrence to the lung 46 months after the surgery, and underwent a pulmonary partial resection. Hepatic recurrence was found in 59 months after the initial surgery. Transcatheter arterial chemoembolization was performed for the treatment. The patient died of esophageal variceal rupture 78 months after the surgery. Although this case was a highly advanced
hepatocellular carcinoma
with tumor thrombus in the inferior vena cava and synchronous sigmoid colon cancer, a surgical treatment and multidisciplinary therapy may contribute to a long-term survival.

A 71
-year-old man with
hepatocellular carcinoma
(
HCC
) presented with intracavitary cardiac involvement detected incidentally on surveillance computed tomography. Tumor with associated thrombus was found to extend from the liver through the inferior vena cava into the right atrium. This intracardiac mass prolapsed intermittently into the right ventricle, causing functional tricuspid stenosis. The mass was resected but recurred after 4 months, eventually causing refractory right-sided heart failure. This case illustrates how intracavitary cardiac involvement of
HCC
can develop insidiously and confer significant hemodynamic compromise. A review of the published research, including postmortem studies, demonstrates that the frequency of intracardiac mass lesions in
HCC
is not insignificant. In conclusion, early detection and diagnosis may have increasing importance in the advent of new therapies for treating advanced
HCC
.

We report a long-term survival case of
hepatocellular carcinoma
with lymph node metastases treated with surgery and radiotherapy.
A 71
-year-old man was admitted to our department at Kumamoto University Hospital in May 2003 for a treatment of local recurrent
HCC
after transarterial chemoembolization. CT showed an infiltrative type tumor, 2.5 cm in diameter, in the segment 8 of the liver. We treated the liver tumor with a right paramedian sectionectomy in May 2003. Postoperatively, lymph node metastases and recurrent
HCC
were diagnosed. Three times of local ablation therapies for recurrent
HCC
, twice lymph node dissections, and twice radiotherapies for lymph node metastases were successfully achieved. The patient survived without recurrence for 60 months after the initial hepatectomy.
