A 65-year-old man was admitted to Oita Medical University Hospital with complaints of right back pain, diplopia and
vertigo
. A tumor, 33 x 25 mm, was found on the right lateral chest wall on admission. Chest X-ray film and chest CT scan showed a tumor, 50 x 30 mm, in the right posterior mediastinum. The chest wall tumor was diagnosed as malignant hemangiopericytoma by biopsy, and the same diagnosis was made for the mediastinal lesion by aspiration cytology. Brain MRI showed a mass at the skull base, which was found to be a metastatic bone tumor from the mediastinal malignant hemangiopericytoma. Abdominal CT scan showed a massive tumor, 65 x 60 mm, in the right lobe of the liver. The liver tumor was diagnosed as
hepatocellular carcinoma
by biopsy. Combination chemotherapy, employing cyclophosphamide, vincristine, pirarubicin and dacarbazine (DYVADIC), in conjunction with radiation therapy, produced no response. The patient died 5 months after admission. Malignant hemangiopericytoma arising from the mediastinum is uncommon. Thus, the clinical features of our case, as well as those of previously reported cases in Japan, are discussed herein.

hepatocellular carcinoma
was admitted to our hospital for
vertigo
after transcatheter arterial chemoembolization. Contrast-enhanced magnetic resonance imaging revealed metastasis in the cerebellar vermis. Although the cerebellar metastasis decreased in size after cyberknife radiotherapy, multiple enhancing nodules appeared in the cerebral ventricles. A diagnosis of intraventricular dissemination from the cerebellar metastatic lesion originating from the
hepatocellular carcinoma
was made. Six intrathecal administrations of 20 mg of methotrexate through lumbar puncture resulted in a dramatic decrease in the intraventricular dissemination. Although intraventricular dissemination recurred 4 months later, these lesions disappeared after an additional 8 administrations of intrathecal methotrexate. Intraventricular dissemination arising from
hepatocellular carcinoma
is rare. Here we report a case that was successfully treated by intrathecal chemotherapy with methotrexate.

In Japan, hepatic arterial infusion chemotherapy (HAIC) using reservoir system is recommended for patients with
hepatocellular carcinoma
(
HCC
) whose hepatic spare ability is favorable. Arterial infusion catheter is commonly detained in hepatic artery via femoral or brachial artery. In our hospital, catheter is often inserted by puncturing the left subclavian or brachial artery considering the patient's activities of daily living (ADL) during long-term detaining. However, it rarely causes posterior circulation ischemic stroke because of the left vertebral artery branches on the path of catheter. We herein report a case of posterior circulation ischemic stroke caused by heparin-induced thrombosis (HIT) after detaining hepatic arterial infusion catheter. A 63-year-old man who is under HAIC treatment for
HCC
was introduced to the department of neurological surgery because of
vertigo
and vomiting. Magnetic resonance imaging revealed sporadic fresh cerebral infarction in the bilateral cerebellar hemisphere. Carotid ultrasonography detected a floating thrombus around the part of the left vertebral subclavian artery bifurcation. Detained catheter was removed and continuous heparin administration was started immediately. However, thrombocytopenia occurred 5 days after the injection. Because 4T's score was 6 points, HIT was strongly suspected. We stopped heparin injection immediately and observed the patient's physical status strictly. After that, platelet value improved naturally. At the late date, antibodies specific for platelet factor 4/heparin complexes were positive and he was diagnosed with HIT.

An incidental liver mass was discovered in a 65-year-old male during a routine ultrasound (US) check-up of his hiatal hernia. The mass, which showed no malignant characteristics, was interpreted as a focal nodular hyperplasia (FNH). Due to normal blood tests and tumor marker levels, as well as the patient's asymptomatic presentation, only regular monitoring was performed. At a check-up 18 months later, CT examination indicated
hepatocellular carcinoma

HCC
). Surgery was no longer possible due to diffuse liver involvement. Transarterial chemoembolization (TACE) and chemotherapy were started. A possible metastasis to the right adrenal gland was detected. The patient started to experience headaches,
vertigo
, paresthesia, and pain of the right jaw. A CT scan of the head showed a mass in the right masticatory space. A CT-guided biopsy confirmed a
HCC
metastasis.
